My mother stayed there in November of 2020 and the care promised was not provided. Her belongings were never unpacked although I was told they were all hung on her walls. Her fridge was not cleaned in 4 months of living there. I found soiled underpants in her closet and under her bed. Her soiled lines were left for weeks after she was hospitalized and worst of all when she caught COVID while hospitalized they refused to let her back into the community and forced us to move her to the only skilled nursing center near by. She no longer walks because of this event. I could go on and on about the promises made to her and I but it is of no use. Just do not bring your parent here if they cannot fend for themselves. The staff turned over so often I should have realized there was a problem. The truth is not something they tell you here. the sales pitch is nice, the place looks great but...buyer beware...this is not a place for MOM.

My mom is a resident in this assisted living facility, The Residence at Stafford. She is not in memory care. Unfortunately she gave up her home, most of her possessions to go there. Although they "advertise" residents are "free" to partake as they wish, that is a lie! They have these "managed care plans" residents MUST participate in, if they don't they are asked to leave. They also use this as a way to increase prices, if resident asks for help with something not in their "plan". My mom moved there for a little help, when SHE required it, not to live by their gestapo rules.

Not anything like what was advertised or discussed prior to my mother arriving. Been here 3 months and the care is horrendous. No nursing staff after 11 pm. Assistants don’t give nurses issues they encountered. Prescriptions orders from doctor have to be constantly checked. Sometimes meds don’t get to resident for 3 days. Food is Bad. Nice menu but really bad. If you parent needs assistance; this is NOT the place. Very disappointing.and very expensive for the non-care received.

This place is god-awful. Do not send your parents here if you care about them. They are left all day long with untrained and uninterested people. Unless you think tossing a balloon around is a viable activity or watching tv, they do nothing with these poor people. My mother never hurt herself before coming here, now she is sent - alone - to the hospital on a regular basis. "Falling" all the time. The communication with our family is abysmal. It's an extremely expensive place and they are put out if your family member doesn't conform to their rigid schedule and can't be cared for by their untrained staff. They will toss your parents out without even telling you until after it has happened.

My mother lived in the memory unit at Traditions for six months before I moved her out. I had so many issues with her care which I repeatedly brought to the attention of the staff. In the six months she lived there there were 2 different memory care directors and a period of time with none. They never let us know when one left, came etc.. My biggest problem was that they had a calendar with listed activities but the activities did not really happen. I live nearby and would stop in at different times of day multiple times a week and they generally had the residents lined up in the hallway sitting on soiled chairs with paper towels on them (I assume for incontinent residents) watching a movie with one staff member sitting there completely unengaged oftentimes texting or with headphones on. They have no activity room in the memory unit that would even fit all the residents if they chose to actually run activities. I spoke to the staff and the director about this on multiple occasions. The cleanliness also was a problem. I noticed her sheets had not been changed in weeks and spoke to an aid about it. She acknowledged they had somehow forgot - how long would this have gone on if I had not noticed the second sheet set never moved? Another time her room and bathroom were not cleaned in weeks. Even their schedule on the wall showed this not that the unpleasant smell would not have told me. Weeks after she moved in I saw they still had the previous occupants emergency information on her door which in the event of an emergency would have been a big problem. It was very sloppy and unsafe. They should not have a memory unit. It is clearly an afterthought and very small,dingy and badly designed. The outdoor area is cramped and not visible from inside so they can only access it with aids so they spend most their time inside usually in the hallway watching movies. I have little knowledge of the traditional side of the building and can only speak for the memory unit. On many occasions I spoke with staff and emailed or spoke to the Director of the entire building. I also told them I was looking elsewhere due to the issues. When I moved her out they tried to charge me for two extra months. They really should have had to pay her moving expenses. It is very difficult to move a loved one with dementia and we never would have done it if her care had been better and we had not been forced to make the move for her well being. If you have a family member in the Traditions memory unit drop in often during the week and keep track of their care and activities as well as the amount of staffing. If you are looking for somewhere to put a loved one I would look elsewhere!

The care in the memory unit is AWFUL. Phil told us as well that they were up to Industry standards but they hide under the assisted living standards. Ratio of 1 uneducated aide to 10 dementia clients is just wrong. Some of the aides like Jodi and Teresa worked well. There were some who were lazy and uncaring. To have 2 people doing laundry, serving food and assisting people is ridiculous. The group that built this unit must not have had any knowledge of what dementia and Alzheimer's is about. The building is a huge circle and with a yell one cannot be heard from one side or another. Many, who can move their loved ones, have done so. One last comment, all the activities promised in the beginning lasted about 2 months. Last time I was in I noticed a huge decline in awareness and involvement of the people living there and NO ACTIVITIES. just sitting. Please be aware.

The analogy I would say, is it looks like the Ritz Carlton, but it's the service of a Howard Johnson. It looks beautiful and would be okay if you just need a place to say like a hotel, but if you have any needs as far as skilled nursing, it's not the place. They try to get by with two people doing laundry and serving the food, and caring for the residents. If you just need a bed or something to eat it's fine. They didn't even know that she needed to go to the hospital. Then they only wanted her back if she was weight bearing and could transfer, and even then we would have to provide our own CNA. For their tagline to be about how we treat you like family, well I wouldn't want to be in their family. She needed to go to the bathroom, and they said that she just went two hours ago, and that she would have to wait. She had never been to a place where she said someone was rough with her, and the group at night had a couple of people that didn't know what they were doing with a transfer. It was a really nice place but it was so deceiving. I don't think half the staff is qualified. We had to bring toilet paper, and gloves, they wanted us to pay for all of the little extras, even thought they said that it would just be the monthly fee. Phil the executive director told us that they base their staffing on the demand not the industry. When I told him that they needed more people, his response was that they were up to industry standard. Even with the new management it seems to be the same, and I have heard almost all of the staff are looking for other jobs.

My loved one did not have a good experience at this community. They are very short staffed. They treat people like they are numbers instead of treating them like they are human beings and this can cause a lot of harm to a person living in that environment. Don't put your family there. They are severely understaffed, they did not fulfill the level of care we were paying extra for. There was a constant fight with them to do their job; they were careless in the care of my mother, resulting in her ending up in the hospital. In a critical situation, one of their aides didn't even know what a DNR meant. She told me, "Well your mom has a DNR". I had to inform her that a DNR only stands if she's having a heart attack, but for a stroke, they should call 911. We moved her out of there because they are incompetent, understaff and failed to take proper care. Looks are deceiving as the community is pretty and clean. It looks comfortable, it's homey, small and intimate. Everything about the looks of it are very attractive and inviting, that is why we chose this place. After having her be there, that's when all of the other problems started to arise. We were told, "We can't do that, we'll need to raise her level of care". So we raised the level of care and paid more, but they still weren't providing that level of care. They weren't even fulfilling the doctors orders in regards to level of care.

There was a time that my loved one had to call me that my loved one had gotten sick, and then I had to call the community to clean my loved one up, since my loved didn't get assistance after a few minutes. We were paying for housekeeping and this wasn't getting done. The community washed my loved one's cell phone in the laundry. It was a beautiful facility. But our loved one is a peritoneal dialysis patient, which became an issue. In the process of determining if he was a candidate to move in and over a span of many weeks, we did our best to allay their fears regarding his PD treatment and provided them with solid medical refutes to their concerns. In the end, I feel like they came up with all sorts of other problems instead of being honest with us that they didn't feel comfortable with him as a PD patient. They simply didn’t want him there.

One of my relatives moved into Salem Woods but had to move out after only three weeks due to very dangerous and chronic mismanagement of medication by their staff. What’s worse is their blatant attempts to cover this up and blame others for it. We’re now filing complaints with numerous state regulatory and oversight agencies to ensure that The Residence at Salem Woods fixes their many problems. We urge anyone who might need assistance with medications to avoid living at Salem Woods until this is resolved. My relative ended up in respiratory failure after Salem Woods personnel failed to give her critical daily medication for over 3 weeks. First they blamed the patient’s primary care doctor for this, then they blamed the patient instead of taking responsibility. Separately, they admitted their staff failed 10 times in just a 3 week period to log the administration of narcotic pain medication, seriously putting her at risk of receiving a lethal overdose. After we audited their logs, we further discovered that a third medication had not been given for at least 5 days in a row. But when we caught this weeks later and brought it to the attention of the director of nursing, [name removed], she claimed that she spoke to the on duty staff at the time. The staff was then allowed to retroactively log the medication weeks later, claiming that they simply had “forgotten” to log it but “remembered” giving it. Subsequent checking of the medication showed that in fact there remained too much medication in the vial for it to have been administered that many times. My relative complained to the nurse on duty that she was having shortness of breath and wanted to go to the emergency room. Instead of staying with the patient, the nurse ([name removed]) went downstairs and called me to ask what to do. This wasted precious minutes as we found out later. I asked what medications had been given that day. She responded that the medication log from the other nurses was too hard to read so she couldn’t tell. I then had to tell her to call 911 and get back to the room to be with the patient. By then the patient had lost consciousness and needed CPR. She spent the next 2 weeks in and out of hospitals, including ICU where she was intubated. I met with the management of the facility about these issues. They admitted they had issues but offered no actions to remediate them and to date have not addressed the issue with staff. I left the meeting totally dumbfounded and told them they had serious legal and safety liabilities. My relative moved out in [date removed]. When we requested the return of her security deposit, the executive director [name removed] sent my relative a release of liability agreement and stated we would only get the money back if we agreed not to pursue legal action against them. This is extortion on top of what was already terrible injustice to a vulnerable senior citizen who almost died while in their care! There is much more to this incredibly heartbreaking story. I feel terribly for the existing residents of Salem Woods, especially given the Covid related restrictions make it difficult for loved ones to see what’s really going on there. I also feel badly for the many staff who truly seem to care but are managed by people who are incapable taking responsibility for making a safe environment. I feel obligated to speak for the people who can’t and make sure Salem Woods is held responsible.

Keep the families INFORMED! For the price I would not recommend Brookside, the communication is not happening- very unprofessional. To call the directors/managers/people in charge of Selleck Wood's liars, thieves, and the most unprofessional people I have ever had the pleasure of dealing with would be the understatement of the century. They are in it completely for the money and could care less about your loved ones. Here is our story in brief: We transferred my father to their assisted living facility 3 months ago with the following care plan: 2-3 weeks of 24/7 care and observance to see if his condition improved and to see if he could handle living independently - if not, Selleck Woods would happy to put him in their memory care unit. About 2 weeks in, we realized that he would be unable to care for himself without full time help and while he sometimes enjoyed his interactions with the community, it was clear that his condition required a memory care unit. Not only were we then denied entry into the memory care unit, we were provided no plan going forward other than, "just keep doing what you're doing and we'll make another assessment in two weeks". When pressed about what the details of that assessment would be and what they were looking for, we got nothing - "oh we'll figure it out when we get there". [Name removed], the director of nursing, is incompetent at her position and relies entirely on her nursing staff to do her job. During this entire episode, the 24/7 caregivers provided by them through a 3rd party were constantly late, slept on the job, or spent the entire time on their cell phones. Selleck Woods had no advice or help - we ended up hiring an agency we found ourselves although my mother spent every day basically babysitting them. O Another incident involved one of the nurses dispensing the medications who got into an argument with my mother one day because it was clear she was not following the medicine timetable - she showed up an hour early because it was clear she wanted to get off work. When asked why she wasn't following the chart, she yelled at my mother and finally after getting her supervisor involved, she sullenly waited to dispense the meds at the correct time. Luckily my mother was around to catch the incidence. Fast forward to today. My father had an episode that Selleck Woods decided they could not handle so we were forced to check him into Silverhill where he caught pneumonia and had to be transferred to the hospital. I'll be posting about Silverhill and the physical abuse my father received while there. Sufficed to say, once he was allowed to leave the hospital, there was no way we were taking him back to Silverhill. When my brother went back to Selleck Woods to explain that my father had been officially discharged, he was told that they were refusing my father entry and that we had been "evicted" despite a contractual obligation of no less than a 15 day notice for removal from the property, not to mention a MORAL obligation as an assisted living community to help my father and family. When my brother tried to stand his ground, [name removed](one of the directors) called the police on my brother and threatened to have him arrested. We will be reporting the police officer who was completely unprofessional in his behavior towards my mother and especially brother. We are still owed our deposit and the last half of the month we paid for. But that's just money. The humiliation, threat of arrest, and so much more were more damaging than anything else. And the cherry on top, my father currently has nowhere to go for his condition. This is the short version of our nightmare. And we will not be silent on this matter for the sake of anyone else considering this community for their loves ones. Don't. Stay away.
