It was a beautiful facility. But our loved one is a peritoneal dialysis patient, which became an issue. In the process of determining if he was a candidate to move in and over a span of many weeks, we did our best to allay their fears regarding his PD treatment and provided them with solid medical refutes to their concerns. In the end, I feel like they came up with all sorts of other problems instead of being honest with us that they didn't feel comfortable with him as a PD patient. They simply didn’t want him there.

One of my relatives moved into Salem Woods but had to move out after only three weeks due to very dangerous and chronic mismanagement of medication by their staff. What’s worse is their blatant attempts to cover this up and blame others for it. We’re now filing complaints with numerous state regulatory and oversight agencies to ensure that The Residence at Salem Woods fixes their many problems. We urge anyone who might need assistance with medications to avoid living at Salem Woods until this is resolved. My relative ended up in respiratory failure after Salem Woods personnel failed to give her critical daily medication for over 3 weeks. First they blamed the patient’s primary care doctor for this, then they blamed the patient instead of taking responsibility. Separately, they admitted their staff failed 10 times in just a 3 week period to log the administration of narcotic pain medication, seriously putting her at risk of receiving a lethal overdose. After we audited their logs, we further discovered that a third medication had not been given for at least 5 days in a row. But when we caught this weeks later and brought it to the attention of the director of nursing, [name removed], she claimed that she spoke to the on duty staff at the time. The staff was then allowed to retroactively log the medication weeks later, claiming that they simply had “forgotten” to log it but “remembered” giving it. Subsequent checking of the medication showed that in fact there remained too much medication in the vial for it to have been administered that many times. My relative complained to the nurse on duty that she was having shortness of breath and wanted to go to the emergency room. Instead of staying with the patient, the nurse ([name removed]) went downstairs and called me to ask what to do. This wasted precious minutes as we found out later. I asked what medications had been given that day. She responded that the medication log from the other nurses was too hard to read so she couldn’t tell. I then had to tell her to call 911 and get back to the room to be with the patient. By then the patient had lost consciousness and needed CPR. She spent the next 2 weeks in and out of hospitals, including ICU where she was intubated. I met with the management of the facility about these issues. They admitted they had issues but offered no actions to remediate them and to date have not addressed the issue with staff. I left the meeting totally dumbfounded and told them they had serious legal and safety liabilities. My relative moved out in [date removed]. When we requested the return of her security deposit, the executive director [name removed] sent my relative a release of liability agreement and stated we would only get the money back if we agreed not to pursue legal action against them. This is extortion on top of what was already terrible injustice to a vulnerable senior citizen who almost died while in their care! There is much more to this incredibly heartbreaking story. I feel terribly for the existing residents of Salem Woods, especially given the Covid related restrictions make it difficult for loved ones to see what’s really going on there. I also feel badly for the many staff who truly seem to care but are managed by people who are incapable taking responsibility for making a safe environment. I feel obligated to speak for the people who can’t and make sure Salem Woods is held responsible.

My mother in law was a resident for 3 1/2 years. During that period, we saw a steady decline in activities and food service. Staffing was chronically short-handed. During the pandemic, the shortage made things worse. Residents were isolated and depressed and needed more care not less. In the the period of 3 months, she lost more than 40 pounds (we only found out when she was hospitalized). We tried to get updates on her weight and other matters but got no responses. She fell at least twice that we know of (we had to rely on staff to tell us) and were told that she was checked out and unhurt. When she was hospitalized for the last time and we saw her, we found swollen knees, recently healed scabs, and black and blue marks that showed that they lied to us. When she fell the final time and didn't return to Mattison for three days, nobody even contacted us to check on her status. She died a few days after the last fall. It took more than three weeks for us to get her possessions. When we finally got them, thousands of dollars in jewelry, clothing, walkers, shower chairs, and other items were missing. When we reached out to management, we basically got a shrug of the shoulders.

My elderly parent is at Brookside and the communications are TERRIBLE! I have easily requested emails SIX TIMES on events so I could attend public holiday celebrations and never an email or call. Unbelieveable! The phone isn’t always answered, no recording machine! Finally in the current virus situation updates would be helpful, what are they doing to ensure the health of the residences??? Keep the families INFORMED! For the price I would not recommend Brookside, the communication is not happening- very unprofessional. A fee of 10k monthly for the one bedroom, 10k a month for the 24/7 aid.

My mother was there for three weeks. She exited the building 5 times although they said it was a secure facility. They wanted us to pay for round the clock care at our cost. When we placed her there we told them she had exit seeking behavior yet they insisted they were secure. She was constructively evicted and we moved her to a SECURE, WELL STAFFED facility. Poorly staffed and very few activities for dementia residents. Phone calls were rarely returned and generally sent to voicemail. DEFINITELY not a place for dementia care.

To call the directors/managers/people in charge of Selleck Wood's liars, thieves, and the most unprofessional people I have ever had the pleasure of dealing with would be the understatement of the century. They are in it completely for the money and could care less about your loved ones. Here is our story in brief: We transferred my father to their assisted living facility 3 months ago with the following care plan: 2-3 weeks of 24/7 care and observance to see if his condition improved and to see if he could handle living independently - if not, Selleck Woods would happy to put him in their memory care unit. About 2 weeks in, we realized that he would be unable to care for himself without full time help and while he sometimes enjoyed his interactions with the community, it was clear that his condition required a memory care unit. Not only were we then denied entry into the memory care unit, we were provided no plan going forward other than, "just keep doing what you're doing and we'll make another assessment in two weeks". When pressed about what the details of that assessment would be and what they were looking for, we got nothing - "oh we'll figure it out when we get there". [Name removed], the director of nursing, is incompetent at her position and relies entirely on her nursing staff to do her job. During this entire episode, the 24/7 caregivers provided by them through a 3rd party were constantly late, slept on the job, or spent the entire time on their cell phones. Selleck Woods had no advice or help - we ended up hiring an agency we found ourselves although my mother spent every day basically babysitting them. O Another incident involved one of the nurses dispensing the medications who got into an argument with my mother one day because it was clear she was not following the medicine timetable - she showed up an hour early because it was clear she wanted to get off work. When asked why she wasn't following the chart, she yelled at my mother and finally after getting her supervisor involved, she sullenly waited to dispense the meds at the correct time. Luckily my mother was around to catch the incidence. Fast forward to today. My father had an episode that Selleck Woods decided they could not handle so we were forced to check him into Silverhill where he caught pneumonia and had to be transferred to the hospital. I'll be posting about Silverhill and the physical abuse my father received while there. Sufficed to say, once he was allowed to leave the hospital, there was no way we were taking him back to Silverhill. When my brother went back to Selleck Woods to explain that my father had been officially discharged, he was told that they were refusing my father entry and that we had been "evicted" despite a contractual obligation of no less than a 15 day notice for removal from the property, not to mention a MORAL obligation as an assisted living community to help my father and family. When my brother tried to stand his ground, [name removed](one of the directors) called the police on my brother and threatened to have him arrested. We will be reporting the police officer who was completely unprofessional in his behavior towards my mother and especially brother. We are still owed our deposit and the last half of the month we paid for. But that's just money. The humiliation, threat of arrest, and so much more were more damaging than anything else. And the cherry on top, my father currently has nowhere to go for his condition. This is the short version of our nightmare. And we will not be silent on this matter for the sake of anyone else considering this community for their loves ones. Don't. Stay away.

We found Atria Cherry Hill to be a very difficult facility to deal with. Their office staff was unprofessional and hard to work with. My mother in law was approved to move into the Assisted Living section on [date removed]. We paid the month in full at the beginning of March. The management met my mother in law at the time we gave them her check. We filled out piles of required paperwork and submitted all required documentation from her PCP, insurance company, bank. However, the facilty had to send their nurse out to my mother in law's home to evaluate her. They admittedly forgot to do this, and the nurse didn't arrive until mid afternoon the day prior, [date removed], to my mother in law moving in. After completion of the evaluation the nurse told us, as a family, that the facilty could not take my mother in law into their Assisted living section because she needed Memory Care. However, they did not have a vacancy in their memory care. She was completely moved out of her house, her belongings had to be put into rented storage until we could decide where she was going to go. Atria never reached out to us. Atria never offered to return her money. We had to fight with them, getting their corporate office in another state involved. It took over 6 weeks for them to give our money back. My mother in law has been living in another Assisted Living, for 2 months now and she is flourishing. She is not at a stage where she ever needed memory care. We would not recommend Atria!

As a current resident I do not recommend Wingate. We have been promised renovations for almost a year. The place is falling apart. The chairs in the dining room aren’t even safe. The common areas have dirty, out of date furniture. Wingate doesn’t seem to care about anything but the bottom line.

If your love one falls and breaks a bone and needs surgery beware of a phone call from administration advising you that your loved one is not welcome back as their evaluation will advise they are not equipped to take care of your loved one. They will kick your loved one out to the street. Family trusted and selected their place because we believed in them that they would offer the best available care. We were sold a bill of goods. They are hungry for the money and when they feel it is to costly to care for your loved one they throw them out in the street. They definitely not care givers and should be ashamed of themselves.

I am very disappointed with the Mattison Crossing for mistreating my mother. She fell and broke her hip while under their supervision and did not call for an ambulance to take her to the hospital. I found her sitting in the wheel chair in the social area. I wound up taking her to the emergency room following the ambulance the night of her fall where they found a broken hip and she needed HIP replacement surgery. She is in rehab and the social worker contacted me to advise that she will be discharged and The Mattison Crossing Executive contacted me while I am out of town on a cruise to advise me that they are not accepting her return and it is my responsibility to remove my mothers furniture and personal belongings this week or will be charged again. Clearly while I am out of town, this will not happen. In addition, they are trying to throw her out in the street but they had no problems taking her money. My mothers home was sold and has no place to go. They should be ashamed of themselves acting this way as a healthcare provider.

When we moved our grandmother in, we were astonished by the facility. The building was new and bright with lots of friendly families. As time went on, the community grew sour. We were highly suggested to move our grandmother to the "Reflections" part of the facility by the staff. The staff in the Reflections community did not care for the residents. One time, a staff member grew impatient with a resident during lunchtime, so she went to the kitchen and forced the resident to eat what she brought back. There have also been numerous reports of the staff stealing from residents. Whatever you do, do not go to The Residence.

The good - the nurses are amazing. They are caring and considerate and really look after the residents. Could not ask for better nursing staff. The bad - since the new management has taken over services have been cut tremendously. The staff is overworked, food has been downgraded. Bubbe's cafe has nothing in it - for the amount that we are paying - they could bring back the plain cookies, milk and yogurt. It was a nice social opportunity to sit with relatives and guests. The ugly - (1) No one monitors the phone between 10 pm and 8 am. If your family member calls you and they need help and you need to reach someone at the facility, you can only leave a message. (2) the level of Kashrut is being changed with only a poorly written notice in small type in the resident's mailbox. We know that other's are unhappy from talking to people, but when we discussed with management they said it was only us. The representative had little or no information and what they told us was wrong. This was done in a deceptive manner and appears to be reflective of management style. And there are no decrease in pricing to offset this change.

If you care about whoever you are thinking of placing there, DON'T. Please taking this as a warning, they are nothing but a business, and what do businesses thrive on, MONEY . They are NOT a patient care facility. Do not be fooled as we were about how much they care about the resident. My father spent more time at the emergency room with unsupervised or unwitessed falls in 2 months than he ever has in 3 lifetimes!!!!!! PLEASE DO NOT SEND ANYONE TO THEIR FACILITY. There is not a rating low enough to post.

I cannot give this facility a rating. I speak from first hand experience, no measure are taken to correct issues. The degree of uncaring is overwhelming. I am planning to move my mother out as soon as possible.

If I could give to zero stars I would. The residents of this community do not receive proper care from the nursing aides. Staff is extremely rude especially the management team. I moved my mom out immediately after two months of hell. The facility has a major bedbug issue and the food is terrible. Mom is now in a new facility! I do not recommend this place. Think twice before sending your love ones to live at the hearth.

I don’t recommend this place. The residents don’t get the proper care for the amount of money they charge. Major bedbug issues, and the employees are very disrespectful especially the management team. I moved mom out after two months. Picking this place was My worst decision ever.
