Not the best, not the worst.

My loved one has lived there almost since it opened. It has been disaster after disaster. They start with one director -- she tried but seemed like she had no support. Then they brought in a new director. Simply amazing. She cared about us, listened to us, followed through on our requests, ect... Then I come to visit mom one day and she is gone. No warning, goodbye, anything. They said she was laid off to move another direction. Well, here we are again starting over. HUGE mistake on their part. As far as I know including my family there are 5 others that are currently looking to move. Since moving in in June we have had 3 administrators, 3 nursing directors and now 3 memory care directors. IN 10 MONTHS! The CNAs are good for the most part, however most do not stay. Nursing is okay -- not the best but also not the worst. All in all I would suggest finding a different place. One with more consistency and stronger roots.

Nice facility, but disappointed in level 2 & 3 care.

The facility is decorated beautifully giving a very comfy feel as you enter. It is clean, food is good, offering a variety of options. The staff they have is dedicated and friendly. The problem is there is not enough staff. One nurse and a CNA for each 63 residents in Assisted Living. If you are a level 1 resident, you would probably do well here, but level 2 and 3 do not get the services you pay for with any regularity. Call lights are not answered in a reasonable timely manner. My loved one fell and it took over an hour before anyone came. When the nurse arrived, she said she was doing the best she could, she was the only one there. We found this to be unacceptable. We were there for 9 days before a bath was given and only then because we insisted since we were paying for two a week. The billing department and the maintenance staff were excellent to work with. I would recommend this facility if you require minimal care, but if you are level 2 or 3, Recommend you look elsewhere. Also, we were told the activity department was really good, but no one ever encouraged our love one to participate. We would like to give Kudoes to two night nurses who were more attentive, [name removed] and [name removed]

Our family's experience

The room size was small but okay, but the bathroom was too small for a walker. The activities were wonderful, and the food was pretty good. We rarely had anyone offer to clean dads room, but the rest of the facilities was mostly clean. Some of the staff were wonderful, but several were terrible and had little if any training. They claim to be equipped for dementia patients, but they are not. The only reason my dad stayed as long as he did is because we hired private caregivers to sit with him during the day.

Was not impressed. Facility appeared neglected, dreary. Your guide was very nice but said she was new, so didn’t give me the impression she knew much about the operations. The patients there were overly happy, as if they were “dosed”. Yet workers did not smile. Too much concentration taking us through the section of people having dementia, which we were not interested in, which made us uncomfortable. PLEASE do not contact me further about this place.

Communication is the biggest issue

Communication is awful, CNA's are lazy, we have to Constantly get on the the staff for updates and care. One time mom was sent to ER - family didn't even find out until the arm band was seen on her nightstand! Some things have been worked on others a "work in progress"

The price was half the other recommended. In less than a realized this was not the place I wanted. Odor horrific at times. Wet clothing left in room. Floor left dirty after meals. Food trays brought into room and not set up for totally blind person. If a patient did not wake up to eat, the tray was left , then picked up untouched later. Patients sitting in communal areas were not checked for patients needing attention. 90% of the time the food was good. Frequently, short staffed with aides working overtime. Water cups Frequently not reachable. Do not remember the staff ever being disrespectful.

would not recommend

The building is not wheelchair friendly no doors on bathroom or bedroom to accommodate standard wheelchairs Call light response always greater than the 7-8 minutes they advertise. looks great in the lobby and dining room but the actual cleanliness of the rooms is poor we had to wait 3 weeks to get a housekeeper just recently so they didn't clean her room until I complained and then 2 weeks no cleaner ok we put her on the Thursday schedule again not done ok we will rotate to Friday and I leave for 2 weeks come back her flats aren't done again I talk to the desk oh she has been moved to a Monday rotation for cleaning so unless I complain it doesn't get done. that is how we choose them but the dining help, move in specialist people at the front desk the activities coordinator who are the friendliest aren't the ones giving care many of the aides are short and rude not all ones like [name removed] and [name removed] are great but you don't get them every day. she is now on hospice and not near the end but they are providing aides Monday thru Friday because of the lack of care from GlassPeaks staff and to be honest if she was my mom and not someone my dad married I would move her They also provide nursing 2 times of week because their nurses don't even check on her except quarterly when they evaluate how much they can charge me for her care If I go to the office to complain they will check on her but I shouldn't have to do it constantly. When I asked for the facility severe weather protocol the head nurse didn't know what it was had to contact the administration and even that took over 30 days to get. you must email them if you can't catch them in the office and except for the lead nurse and of course billing they take sometimes up to a week to get a response. she likes it most of the time but because she is in the bed most of the time and it takes 2 people to get her she hasn't been to the dining room since March but they charge her an extra 5 for each meal they deliver to her room. they are good and [name removed] tries to get people involved. overall for almost [removed] a month it is 1. Like I said some of the issues have developed over time as [name removed] is hard to deal with and manipulative and the staff is not encouraging to her to get up and do things because let's face it is easier to go into a room and say would you like to shower and for her to say later at 10 and then never come back to do it. Some aids have gone as far to tell new staff don't do anything for [name removed] unless she asks you to do they just come in once a shift or so "do you need anything" and of course her answer is no until I get there

I have to rate it a generous two.. In fact we are moving my parents out at the end of the month.. The apartment was not move in ready and it took a week for them to address the issues.. It took them over a month for them to get their medical needs taken care of and in wasn't until we had to get the general manager of operations involved to get it done. Communication on laundry and house keeping was not given.. My father has had to do their laundry and only one of the four washer/dryers actually works.. I'm giving it two stars because although inefficient, most staff were friendly and the food was fine..

My elderly friend doesn’t really care for the food, except for a few items. She also feels it is dirty and the housekeeping staff do not do a good job with her room. One time, one of the laundry staff didn’t show and she was without clean clothes for ten days. She said most of the staff is not friendly. I must tell you that she can be difficult and is very hard of hearing and has poor vision so that limits her interaction with others. The building is 40 years old and is worn. It shows. She is not happy.

There was never adjustment for special meal requirements- mom needed soft food and the liquid Food in general was just ok, there was not a cook they just reheated prepared meals Having nurses in charge of this facility should of made the facility better but they did not like to be questioned or corrected, which is what occurred between them and the hospice nurse. When we did a review of moms care they lied about moms ability which was a last straw but nothing to be done because I was aware that we were in moms last weeks. Mom was assaulted by another resident last fall, which was stopped by a staff member but it was not handled well. They then offered my mom a space in memory care which was ok. The memory care has a good reputation with hospice and the aides and med techs for the most part were excellent. They like all places had problems with staff shortages. I did hire sitters for mom and they covered every afternoon into evening for 4-5 hours. The facility told me they (the sitters) were not necessary but they were my eyes when I wasn't there so I'm sure they improved the care. If there would be any thing that I would say about future residents - they should not have problems and be able to move themselves (use wheelchair with out assistance).

My experience with Oasis at 56th Street was extremely unpleasant. I always went outside to smoke but due to having Covid, I was confined to my room. I lit one cigarette and a nurse walked in on me. I was evicted and they kept my rent refund. [name removed] on the 2nd floor and [name removed] on the 4th floor smoke openly every day and go unpunished. My watch was stolen and they took my laundry and it was never returned. The food leaves a lot to be desired and it's always cold. One day when I was sick, I ask a nurse if she would take my temperature and she sent me on a wild goose chase looking for a thermometer. None was found. BED BUGS throughout the community. They took every article of clothing I own and kept it for 2 weeks. I showered daily but had to wear the same dirty clothes. Privacy is out of the question - they freely use a pass key to walk in on you.

Review for Independence Village of Greenwood

I would say there are a some great caregivers who were. Good to my mom. Alot of turnover of staff and short on nights. Do not particularly care for upper management and [name removed]I feel I had to be very present and do many things for my mom myself. I would not necessarily recommend the facility.

Our Experience

My mother was approved to go there in Assisted Living. That surprised me and I told them that, but they said she had been evaluated and qualified. She was there for basically a week and was sent by them to the hospital and was not allowed to return because she could not stand and pivot on her own. The facility itself is wonderful.
