Do NOT place your mother in this complete private pay facility. There is a high turnover with staff. Understaffed at night. Admitting high acuity residents without proper training to Care for them. Offer add on services but not enough staff to cover what is needed for proper level of care. This is the perfect example of a wolf in sheep's clothing. The activity director quit. No social worker on site. Business is run by an inexperienced team. If you Care about your loved one do not go here if they have any type of cognitive deficits.

At first glance it was beautiful. Then after awhile you get a full understanding it's all about the money. They get involved in family disagreements which creates a larger scale of problems. There is routinely only 1 to 2 persons on night shift if you can find them. They accept high acuity residents without having adequate staff trained to care for them. There are literally Prairie size dustballs under the furniture even after deep cleaning. There's a high level of turnover with staff. There sometimes is not milk for 2 weeks at a time. There isn't enough staff to provide the extra add on assist the resident pays for. The management isn't receptive to any of your complaints and in turn threatens to have you band from a facility that is private pay. Do not fall for the sales pitch if you love your family. This facility is not appropriate for assisted living or memory Care. The director is terrible, the activity director quit, in house select rehab is short staffed. Terrible

This was the facility that I was leaning towards placing my parents, however, after the tour, there was NOWAY! The Director had to stop multiple times to herd confused memory care patients back to their apartments, we saw no other staff on the floor. The available units they showed us were dirty and contained biohazard bags and boxes. It was not a good overall impression. It is not a place I would feel comfortable leaving family. If that’s what they are showing us on a tour, who knows what’s going on behind the scenes.

My mom was a resident at Symphony at Mentor for nearly ten months. During that period, management changed twice. Nearly all of the staff changed. Communication was not good. I think mom was like many memory impaired seniors and either believed she had showered and refused when asked if she'd like to shower. The problem is, these folks figured that was okay and weeks and months passed between showers. Any time I complained, she would get a shower, but we were right back to long spells without them until I complained again. Housekeeping was unreliable. Once I noticed a stain on the bedsheet and two visits later it was still there. I changed the sheets and taped the date to a corner. Several weeks later, they hadn't been changed. I complained and it seemed to improve. The floors were never fully clean. The move in was difficult, with no pavement between the walk and the door. The thresholds of the doors to the courtyards are too high for a wheelchair to cross without a struggle. I imagine a person with a walker would need to be careful. The security procedure to enter and leave the unit discourage visiting. There is a doorbell that summons someone to come and punch the keypad to unlock the door. When you're lucky it happens quickly, but many times it can be a 4 or 5 minute wait before someone responds. There was next to no communication. The receptionists were the best sources of info and they were more proactive than management. The meals were average quality. The menus looked interesting. There were more pink and yellow beverages than I would expect. I would recommend people away from this facility. The price was the lowest I found, but I found that to be a false value.

We thought it had a good reputation and we trusted the individuals we went on tour with, two of which are no longer there.Terrible place didn't take care of our sister. There was a lack of communication and not enough staff.

First, I would like to know why my comments keep getting removed - My wife and I have been visiting my grandmother who was admitted about 3 months ago and each time grow more concerned about the conditions of the facility. We continue to grow deeply disturbed and appalled by their inhumane treatment of patients. It is with a heavy heart that I feel compelled to share this negative review. One of the most distressing aspects of my visit was witnessing the way the staff interacted with a patient who is wheelchair bound and needed to use the bathroom. We could hear the interchange as we sat in the hallway. The patient was clearly in distress and afraid to move from the chair to the toilet. The caregivers displayed an alarming lack of empathy, and understanding - berated the patient, argued with her, taunted her and cursed at her. It is clearly evident that the staff members are undertrained, and their frustration and impatience were palpable.

Medications are not often given at night. There are nights when you cannot contact anyone there on duty. This place is not about care it is about making money. I do not recommend this facility at all!

Overall worse experience for my parents and family. What was told they do for memory care residents was not even close. They are not trained for Alzheimer's patients very bad for my mom and dad. Instead of redirecting her they would sent her back to her room, my dad never got to leave it was the same as them both being in their home! Awful place do not refer other families.

Place should be shut down! Took mom out of there in less than 3 weeks!!!! Absolutely did not meet expectation's. ! Not enough staff so they lock patients out of their rooms so they can locate them easier

My mother moved to symphony in [date removed]. Yes, there were staff issues but those were worked out. Even during the issues you could tell that mgmt was doing the best they could. In [date removed] 'corporate' fired most of the management staff. Most of which still haven't been replaced. Many other employees have also left-only two of the staff i know are staying. It is musical chairs with agency help. More often than not, residents are parked in the dining area sleeping in their wheelchairs. Now about cost changes. Our rate started out at $[removed] per month. Went to $[removed]. Ok. Has been $[removed] to $[removed] until this month. Received bill for over $[removed]. No advance notice, no communication-nothing. Tried talking to corporate-so far useless but will keep working on it. I understand mom needs more care-two people to help from sitting to standing and for bathing but other than that it is all a money grab. If it's up to you, go to a good nursing home for full care-symphony is not a good long-term option.

Don't lie to people. Fire anyone caught lying. Make there employees do there job. Or fire them.. get off the [profanity removed] point system. Up front let the people know what they will charged!!

We felt that the services they offer were very over sold and they were not able to carry out those services. This was a very difficult time for our family and loved one and they assured us they would be able to handle and deal with our loved ones certain behavior's associated with the condition and they were not. We were very impressed in the beginning with the tour but once we moved our loved one in it was the complete nightmare and nothing met expectations and it was just a compete nightmare. ,

I toured this community. Horribly unprofessional and I would never consider them again.

This place looks lovely on the outside, has great food, but was nothing but a horror inside. My mom was there for 3 weeks and we moved her. My mom was full of grab marks and even left in her room in an unlocked wheelchair. The help is extremely rushed. When we were moving out several items were missing including her DVD player. I just hope they corrected these issues. The medical care is attentive.

Nothing else to say other than this is a conflict of interest.

We did not like this community and we knew it was not a fit for our loved one.

I JUST moved mom in to Symphony of Delray Beach - having BIG issues - but I'm not getting ANY response to my questions! [name removed], our sales girl (bubbly & responsive) - one reason we chose this place - was fired and we were left to deal with [name removed] - the Director - she was non-responsive, cold and deservedly was let go the following week! I'm told a woman from "corporate's in charge" - I called and asked to speak to her (no one knows her name?), I was told "she'll be in on Wednesday"! I've asked for the number to "corporate" which no one knows or will give to me (no info on website). Desk xferred me to HR, phone rang and rang - when answered they hung up! Staff doesn't understand they're dealing with the elderly who needs their time and patience, which they have NONE to give! The one good thing is my mom enjoys the food. I would NEVER recommend this facility for anyone who is looking for a home for someone that they love - I am making plans to move my mom out AS SOON AS POSSIBLE!

This place is horrible. The workers are lazy the office people are very rude. Their food is nasty and they are very under staff. And it not very clean in their. I took my love one out the facility. This place need to be reported..

Just moved my father out, Administrator number 3 since he got there 8 months ago. Nurse [name removed]is horrible, uncaring and lazy. The sales person was nice enough, but was not very caring once you moved in.

We are so unhappy with this facility. The food is disgusting. Our family member is now eating frozen dinners while paying for 3 meals at this place. Dinner is practically non existent. Lunch is hard dry meats. Vegetable soup this week was one pea, one bean, one carrot. Potatoes are hard as a rock. Staff leaves for better working conditions and pay all the time. Paying for services we seldom receive. Pharmerica, the drug company delivers much needed prescriptions days and days after script sent in. Even sent wrong dosage once. Sometimes a week or more goes by without meds being sent to patient. This place needs to be reported
