My Husband had a short stint at Symphony Manor. He has Alzheimer's....... One weekend he was a little more agitated than usual, Symphony Manor called an ambulance and had him taken to the hospital for behavior stability. When his stay at the hospital was completed and he was calm, we had a patient review with the hospital. To my surprise, Symphony Manor (who was there in attendance) told us he was not allowed to come back. I was in shock, there was no warning whatsoever. I had to start the process all over again to find a new home for him. This is not only a huge set back for the patient, it's also detrimental to family members. I felt so incredibly helpless, we had no one to turn to for help, I now know what it feels like to be homeless. In other words, your loved one had better be totally compliant or they will be tossed to the curb.

Don't let your loved one experience [Removed] here

My mother has been a resident of St Augustine Plantation for over 3 years and the quality of care has declined to a point that I will no longer recommend the facility. The care-givers and nurses have good hearts but the administration is terrible showing [Removed] and caring that simply can no longer be tolerated. The food served is frozen vegetables and processed entrees like pot pies. Despite my constant requests for fresh fruit, the residents rarely see anything except canned peaches or fruit cocktail. Tonight a resident was crying because despite numerous requests for meat and potatoes for dinner, all he was offered was grilled cheese or cold lunch meat. Families have called and complained and met with management and assured of changes but the situation is not improving. My mother had an unreported leg wound that was [Removed] and became horribly infected - the infection opened a stage 3 wound with infection half an inch away from the bone - an infection that 3 months later we are still fighting. The facility is pretty but until the administration is changed at the top, I urge you to avoid putting anyone you care for in the director's hands.

Woodholme Gardens Experience

My experience at Woodholme, unfortunately was not a good one: Constantly sending emails, no follow up, no showers on shower day, room not cleaned on housekeeping day, having to be reminded to wash clothes, missing items, "Short staffed" (per the staff) which DEFINITELY affected patient care!!! For the price, very sad!!! Every employee does "not" have patience, and some simply lack compassion and empathy. In society as a whole, you have some people work in a field simply for a paycheck and others who love what they do and have hearts to serve... you can tell the difference. I'm grateful for them. We have witnessed quite a few very unsettling things there, which is why our family was constantly there. My advice would be to 1) check on your family member "regularly." 2) Document everything, take pictures if necessary! 3) Assure you are receiving the services you are paying for, you may just be surprised. 4) write your loved one name on everything (when it comes up missing...nothing will happen. In my opinion, they were nonchalant about it) The family was left purchasing the items AGAIN! It was "always" something! Just because a place is expensive doesn't mean it is better! I personally would not recommend anyone. Some employees were absolutely wonderful but the overall "care of our loved one".... Very disappointing! Activities...plenty, but what you dont realize is that the residents "sit for hours." If they dont have the mental capability to go to their room (on their own) to lay down and relax, then they will sit ALL day until its time to go to bed. Maybe my expectations were higher, maybe it was due to being short staffed... it's your love one, so of course you want nothing but the best for them. What we were offered is NOT what we received. (promises & excuses with no resolution, no change) It was very frustrating and overwhelming. I thought it would be easier, but it wasn't unfortunately! Many times, I felt as if I was still caring for my loved one. In closing I just want to say, it takes a special kind of heart to care for the elderly, especially dementia patients. It takes extreme patience. and love. You hear it in a person's voice and see in a person's actions. My prayer is that those looking for a place for their loved one, that the Holy Spirit will lead them to such a place, filled with love, and compassion, where their needs are always met and the staff have the joy of the Lord (because it strengthens) in Jesus name Amen. I know its not easy, try to remember "with God, all things are possible (Matt.19:26) & that you "can do all things through Christ which strengthens you". (Phil. 4:13) Grace and peace be multiplied to you all.

our family's experience did not meet expectations and would not recommend.

It's a beautiful place and it fooled us at first, but I didn't leave my parents in there a full month and I would never recommend it to anyone. They removed my Mom's thyroid medication from her chart (still not sure how or why that happened because my Mom has Hashimoto's), often took over an hour to respond to the emergency pull, and generally were the opposite of helpful

Not the place for my love one

It is very expensive to move your family member from one facility to the next. But in my mother's case, the move was needed so give me a couple days and I'll think about it. Each month they added different cost to my mother 's bill. When I questioned the extra charges .Each person gave me a difference answer. My mother was moved to the memory center about three months after she was an assistant living, which that didn't surprise me. The staff was not trained properly to know how to handle people with dementia. I went every week to see my mother, and I witness this with other people in the memory center. So I can only imagine what they were saying to my mother when I wasn't there once the agitator and she would act up then it would cost her more money at that point I had it.

They could improve on staffing. Communication with the families. Put the dining hall in the middle of the room instead of all the way at the end of the room. Not leave residents soaking wet in their own urine and lying in the floor for hours until the have rhabdo. My favorite thing about the community was when my dad left in an ambulance and never went back.

Misguided lies

Not a well run organization and will be one pricey mistake if you move your loved one in here. The food is horrible the nurses are horrible and the sales pitch is all smoke screens and the executive director is as phony as they come. Medication is either never available or “late” / never received.

Mom not taken care of

Unfortunately, not too long after my mom moved into Symphony she started wandering. Symphony made it clear the only way she could stay there is if she had 24 x 7 Aides. Once that happened, they did not pay attention to her at all. The room was barely cleaned, medication was not always on time. Plus, many of the residents were rude to my mom. Shortly after, I moved my mom out. Bottom line, Symphony is OK For seniors that are self-sufficient. But for seniors that need help, I would look elsewhere.

Review for Symphony of Valley Farms

Had add on's and was fine during the day but not for dementia residents. Hardly any staff after 6 when sundowners occur. Lots of falls, out of milk regularly, not chef prepared meals appears frozen food. Activity director quit and not replaced. No social worker, no Medicare or Medicaid beds to transition to. If a person is alert and has no dementia and only needs minimal assistance, it would be fine. I did have items come up missing. No assist bathing at times.

Symphony at Valley Farms

Do NOT place your mother in this complete private pay facility. There is a high turnover with staff. Understaffed at night. Admitting high acuity residents without proper training to Care for them. Offer add on services but not enough staff to cover what is needed for proper level of care. This is the perfect example of a wolf in sheep's clothing. The activity director quit. No social worker on site. Business is run by an inexperienced team. If you Care about your loved one do not go here if they have any type of cognitive deficits.

Nightmare

At first glance it was beautiful. Then after awhile you get a full understanding it's all about the money. They get involved in family disagreements which creates a larger scale of problems. There is routinely only 1 to 2 persons on night shift if you can find them. They accept high acuity residents without having adequate staff trained to care for them. There are literally Prairie size dustballs under the furniture even after deep cleaning. There's a high level of turnover with staff. There sometimes is not milk for 2 weeks at a time. There isn't enough staff to provide the extra add on assist the resident pays for. The management isn't receptive to any of your complaints and in turn threatens to have you band from a facility that is private pay. Do not fall for the sales pitch if you love your family. This facility is not appropriate for assisted living or memory Care. The director is terrible, the activity director quit, in house select rehab is short staffed. Terrible

This was the facility that I was leaning towards placing my parents, however, after the tour, there was NOWAY! The Director had to stop multiple times to herd confused memory care patients back to their apartments, we saw no other staff on the floor. The available units they showed us were dirty and contained biohazard bags and boxes. It was not a good overall impression. It is not a place I would feel comfortable leaving family. If that’s what they are showing us on a tour, who knows what’s going on behind the scenes.

My mom was a resident at Symphony at Mentor for nearly ten months. During that period, management changed twice. Nearly all of the staff changed. Communication was not good. I think mom was like many memory impaired seniors and either believed she had showered and refused when asked if she'd like to shower. The problem is, these folks figured that was okay and weeks and months passed between showers. Any time I complained, she would get a shower, but we were right back to long spells without them until I complained again. Housekeeping was unreliable. Once I noticed a stain on the bedsheet and two visits later it was still there. I changed the sheets and taped the date to a corner. Several weeks later, they hadn't been changed. I complained and it seemed to improve. The floors were never fully clean. The move in was difficult, with no pavement between the walk and the door. The thresholds of the doors to the courtyards are too high for a wheelchair to cross without a struggle. I imagine a person with a walker would need to be careful. The security procedure to enter and leave the unit discourage visiting. There is a doorbell that summons someone to come and punch the keypad to unlock the door. When you're lucky it happens quickly, but many times it can be a 4 or 5 minute wait before someone responds. There was next to no communication. The receptionists were the best sources of info and they were more proactive than management. The meals were average quality. The menus looked interesting. There were more pink and yellow beverages than I would expect. I would recommend people away from this facility. The price was the lowest I found, but I found that to be a false value.

We thought it had a good reputation and we trusted the individuals we went on tour with, two of which are no longer there.Terrible place didn't take care of our sister. There was a lack of communication and not enough staff.

Inhumane treatment of patients - staff is too young

First, I would like to know why my comments keep getting removed - My wife and I have been visiting my grandmother who was admitted about 3 months ago and each time grow more concerned about the conditions of the facility. We continue to grow deeply disturbed and appalled by their inhumane treatment of patients. It is with a heavy heart that I feel compelled to share this negative review. One of the most distressing aspects of my visit was witnessing the way the staff interacted with a patient who is wheelchair bound and needed to use the bathroom. We could hear the interchange as we sat in the hallway. The patient was clearly in distress and afraid to move from the chair to the toilet. The caregivers displayed an alarming lack of empathy, and understanding - berated the patient, argued with her, taunted her and cursed at her. It is clearly evident that the staff members are undertrained, and their frustration and impatience were palpable.

Very bad facility

Medications are not often given at night. There are nights when you cannot contact anyone there on duty. This place is not about care it is about making money. I do not recommend this facility at all!

Mixed Bag

Overall worse experience for my parents and family. What was told they do for memory care residents was not even close. They are not trained for Alzheimer's patients very bad for my mom and dad. Instead of redirecting her they would sent her back to her room, my dad never got to leave it was the same as them both being in their home! Awful place do not refer other families.
