My parents lived at Inspired Living at Lakewood Ranch from the time they opened. When they opened we had great hopes for the facility. In the almost 4 years they've been open, they are now on the 9th Executive Director. They even changed Executive Directors during the pandemic. This once again cause loss of key personnel. I've had many issues over the years, including my parents getting stuck for 45 minutes in the elevator during a power outage! Their cleaning crew has never been good but I've found out this week they are horrid now given th pandemic and noone is inside checking on them. After the new ED came in and their best nurse left I started looking at other facilities. I found that they are gouging me on cost. I've been able to cut the costs by about 40%, and the new facility provides a higher level of care. I moved my parents into a brand new full memory care facility with amazing care and attention this week. They are so much happier already. I was not allowed to go in and pack and move them, had to have their named movers do it. When their belongings arrived at the new facility I was called by the Executive Director to tell me their beds were in awful condition, I needed to buy new ones (theirs I had bought 2 years ago but they weren't keeping the mattress pads on it). Also that their clothing was in awful condition, smelling of urine. The furniture was caked in dust. Before the pandemic I went in 2-3 times a week and picked up their apartment, always telling them what wasn't being done. It appears since I haven't been in there they have not done their jobs. Also, in the time that I gave them notice they ran out of one of my dads meds and I had to complain to get them to fill it (I pay for them!). I also found that some other things happened that they should have called me about and never did. I have called the parent company CEO in the past to voice my issues but they were never addressed properly. This place has had ups and downs since they opened with all the turnover, but this is outrageous what is going on now. Don't let the nice pool fool you, your loved ones need much more.

I had my mother there for approximately three years between assisted living and memory care Over the three years the turnover of personnel here is just Not believable Numerous managers nurses CNA’s etc. every few months different personnel Although the facilities is quite nice management in nursing care is pathetic Please do yourself a favor and look somewhere else I’m not sure where these are the reviews are coming from probably people that work there but again look elsewhere

If you are considering Autumn Leaves for you loved one- run the other way - fast. My husband was there for 2 weeks. Prior to his admittance I had toured twice, the staff was convincing, the place looked well planned. When he moved in on March 19, 2019, he was walking with no assistance, had no sores or wounds on his body After the first two weeks (they don't want you to visit the first 2 weeks) he collapsed, was sent to the hospital with pneumonia, multiple pressure sores on ankles, heels (stage 2), hip (stage 3), buttocks and back (stage 2), swollen legs and feet, rash, irregular heart beat and could not walk or stand on his own. With out my knowledge he had been given a drug that has a black box warning from the FDA that says "do not give this drug to elderly demintia patients"

Run the other way. My husband moved in March 19. He could walk without assistance and had no sores or wounds on his body. Two weeks later, he was in the ER with phneumonia, mulitiple bedsores (stage 2 and 3), swollen legs and feet, rash, irregular heart beat and could not stand or walk on his own. They do not want you to visit the first 2 weeks so you can imagine my shock when I saw him in the ER. This place looks good and sounds good but there is a definate lack of care.

They were warm and sweet and friendly and listed all of their amenities and boasted just how lively and wonderful this place was. Petrified to make this next step for my parent, I felt this place was thriving and the answer to our prayers. Sadly it was not. The staff is denfensive and moody the minute you have a problem. You can never get a hold of anyone as the aids and staff that take care of your loved one are not consistent. There are limited activities. The worst part- items missing, stealing, and staff members on their phones. While there are some lovely people, the charm and excitement to get my loved one was enough to sign the deal, and I haven’t seen any of that love since. There’s a lack of humanity that I’m not sure is a trait one can fix.

This is a horrible facility. My Mom was not taken care of well at all. She had to be placed into a nursing home because of this god-awful place. She had 2 bad falls. At the hospital they found her malnourished and dehydrated. She had to have a feeding tube placed. She had horrible edema in both her legs. The edema was so bad she has permanent tissue damage. One of the falls cut her leg so badly that the skin will never fully heal because of the tissue damage. I can't say enough bad things about this place, please please stay away. They are truly only it for the money.

This place was great at the beginning, but as staff turned over and our needs changed, their care became more of an issue. To get you “signed up”, they will lie to you and tell you you can "age in place" but if your loved one is too much trouble, they will ask you to leave...or worse, admit to the hospital so they can deny your loved one re-entry. Staff turnover is very high and not nearly at the ratio it should be for the cost (5k+ a month). There has been 3 executive director changes and 4 health and wellness director changes since we have been there...nice, clean facility but keeping staff is really an issue. many promises and we had high hopes, but the turnover has caused the situation to be such that I can not recommend at all.

This started off as a great option for our mother. In the beginning the lured us in with a "special" rate. After medicine and level of care additions the rate went up by half. The bills would come and we would have constant rate increases that eventually more than doubled our original fee. We decided to move our mother to another facility the day we watched a resident who was in the memory care unit fall, hit her head and get back up with NO reaction from staff.

My mother and father have been there for many months they have never bathed my mother. My sister had to get to other people to put my mother in the shower. Because she just became accustom to not bathing. The staff pressures my father to get my mother to eat. And now pressuring him to do something about the 'stink.' They are pushing him to give care that he can not possibly provided. He is getting more and more stressed out.

This place is the worst ever. Don't put your family member in there. It's nothing but a front just to get your money. I give this place a ZERO rating. Reaping with urine, food sucks, workers sitting around on cell phones, loud talking amongst t h.g themselves, patients not being properly watch. Male patients walking around trying to get in doors. Items missing from rooms. They are a bunch of money hogging staffers and rude liars about this place. Nothing they say is true. I suggest you look elsewhere their poor rating speaks volumes.

I am unfamiliar with the other Chelsea ALF but I can tell you that this location is poorly run. There is frequent turnover of administration as well as staff. My loved one has fallen several times over the past few months. Response time is 20-30 minutes each time. I find this unacceptable with regards to quality of care and it is atrocious for the price they charge these residents. We have had issues with missed showers, missing prescriptions and roaming residents stealing from my parent. Attitude is poor from the top all the way to the bottom. It is obvious by the way occupancy has doubled in the past 18 months that they are more worried about the bottom line than the well being of their residents. My loved one was in an ALF in a different state - very different and a very enjoyable experience. Do your homework before sending your loved one here.

The executive staff there is EXTREMELY rude to deal with. I would not feel comfortable leaving a family member there.

This facility has been renovated but what lies under beauty and decor is another story its what o call "plastic" "phony" there is two realities I actually toured the facility the marketer Cheryl was smiling yet her tone of voice and poor eye contact showed me she was not wanting to tour us she had no knowledge of the speciality care memory impaired unit which was nicely decorated due to holiday yet smell of urine overwhelming there was two workers sitting conversing to each other one had cell in hand few residents sitting alone no interaction a activity going on with group of 9 by bubbly older woman named regina who introduced herself Cheryl did not there was trash stuck in handrails on walls the nurse [removed] thin physically eating standing over med cart On tour of second floor I was introduced to the director of nurses who appeared irritated unfriendly in corridor I heard loud conversation by large sized african [removed] in wellness where she stated "im tired of these residents and their [removed]" She was talking to a young man I was sad to see how Chelsea a well known healthcare company had lowered standards but as I sat in lobby I did have insight by conversing with a resident who informed me the place was the pits and blamed administration stated she does nothing we have no rights well clearly id never put my mom here now
