Best for Independent Living or Individuals with Lower Levels of Care Needed

I do not recommend Hearth and Home for loved ones they've evaluated/determined as needing Level 6 care. Everyone I interacted with was friendly, respectful and responsive. There was a calendar of events/activities provided as an example of what was offered, but we did not experience any of them, perhaps again due to COVID and the short duration of her stay. Unfortunately, the facility had a COVID outbreak the very week [date removed] we moved our grandmother in. As a result, she was isolated to her room and never had an opportunity for socialization. The employees commented regularly about being understaffed which appears to have also impacted the level of care she received versus the level of care she truly needed.

I would never go there again or recommend that anyone go there. The food was subpar, one night our supper was peas and scallop potatoes that was it never any alternates they said there was alternates, but they never had them. Staff never helped the people who are more handicapped, cut their food or anything. Many people did not eat just because they couldn't cut up the food or they would pick up the food and one big chunk and bite off pieces.

Not so great

Very unorganized. Some of the staff is great, some of the staff does not have any critical thinking skills and are just there going through the motions. If you want anything done, you have to ask a million times, and it probably still won't get done. They can't manage to do laundry and bring the correct clothes back. They constantly are changing their management, so if you need to talk to someone you don't know who to talk to. Most of the meals are junk food and are full of sugar. Just sickening. I would not recommend.

Despite plenty of opportunities to improve

Inexperienced staff with dementia, they allow them to refuse meds, showers, and neuro checks following falls, but they are suffering from dementia and do not fully understand what they are refusing. No activities for those with dementia, they are left to sit in chairs with no interaction. Missing items for months. Consistently late meds. Meals are not palatable and are very small and you are expected to use nutritional drinks to replace their lacking meals.

Not a great experience

Unhappy

Totally unimpressed with the level of care, attention, and cleanliness! For the 5200$ a month plus pharmacy fees of over 200$ a month charged, I dont feel the fees are worth the appearance of the facility. My mother "escapes" rather routinely and administrators and "nursing" staff never call, when I was informed of her walking out, it was "if we go walk with her, she calms down and is different "! I appreciate this,but I dont appreciate no calls, same trash ( candy wrappers), under her bed for 3 days in a row, the fact that she is incontinence her trash not being emptied for 3 days! Truthfully I 'm in the process of trying to find a smaller facility with less "patients"! I feel all Mom is happens to be a payment!

At first, we were happy with Glen Park. But, when the mice began running around my mother's room we became unhappy. Unfortunately, in my opinion, the administration did not deal with the rodent problem in what I considered a timely manner. Needless to say, I moved my mother immediately. In my opinion, Glen Park is not a facility that should be considered when looking for a rodent free environment for your parent.

Very disappointing experience

This assisted living does not adequately take care of memory care needs for residents with Alzheimer’s or Dementia diseases, although they charge them with higher level rates. My mother’s rent was increased three levels in three months, housekeeping did not keep her room clean, my mother was not kept clean, and the food is awful. Very disappointing!

Not a good option for memory care.

We did not have a good experience at Hearth and Home at Vandalia. I found that the staff were not trained to deal with the challenges that Alzheimer's clients present.

Poor Care , Inexperienced staff, high turnover, poor all around

My mom died at Wyngate 5 weeks after entering. We paid for a higher level of care that we did not receive. Poor management, inexperienced staff, high turnover, sever staff shortages, poor quality of food (arrived to her room cold every time) . The twice a week showers did not happen & the bed linen was not changed for 2 weeks (supposed to be weekly). They did not assist my mom to bed & did not help her dress. Finally, she fell at 11pm getting up from her chair, fell & died 4 days later. She should of been in bed by 9pm. No one checked on her from 430p-11p as the food tray was still in the room & food was all over the place after she fell. $6400 per month, self pay for this level of care. Don't do it!!

Poor Value

Very poor care with very little effort. Items missing from loved ones and cannot be found. Drop off very poor meals, might as well be meals on Wheels. Don't try to help residents feed themselves, just drop and go. Buyer Be Ware before you put someone in here that you Love. All Blow and no show!!!

A sad place to put someone

I was sitting with a friend who got very little care that so called residence manager was a rude lying two-faced women who didn't help him at all he never was clean told her and those so called caring nurses but no one cared I saw so many up sitting things going on here l wouldn't put anyone in here it's made to look good but believe me a lot of things go on here that's not right beware just beware

Not Happy with this facility for mom

My mom was not happy here. Her condition declined and she was not given her medications. The staff was not equipped for my moms needs. She was taken to several other locations and when she came back to this facility she was not given medication and we were informed that she could not be controlled all due to incorrect medication.The location was not equipped due to her condition decreased They did have alot of activities for her to be able to keep busy, but was limited due to incorrect medications. We had to take her out of this facility and place here somewhere that she gets better care and is now doing very well. I would not recommend this to others

My mother was in the Madison house for almost a year. Her room was always a mess. I had to clean and put laundry away every time I visited her. Her hygiene declined once she was placed there due to staff members not taking proper care of her. She was given the wrong medications on two separate occasions. There is a major breakdown in communication at the Madison House. Management needs a tremendous amount of improvement. I can provide two examples, although there were many more. On one occasion, just before my mom went on Hospice the manager had a LPN call me so I could hear my mother screaming for help. My mother had end of life anxiety, dementia, and was also bed bound, so she screamed out a lot. But isn't this what nursing homes are for? Another example is my mother went for a doctors appointment and came back with that night with a dialysis port (surgery) that the staff, management and my family had no idea she was having performed. When I called the hospital that performed the surgery they were able to provide me proof they had faxed all the information to the Madison House about the surgery. When my mother was turned over to Hospice she remained at the Madison House. Hospice cleaned her room and cleaned her up. Only then did my family and I feel like she was receiving the care she deserved.

We did not have a good experience with this community at all. It was not as secure a community as my loved one needed, and the staff did not handle the memory care well. My loved one had wandering issues, and after only a few weeks they decided to send him to the hospital after an escape attempt and told us he could not return.

My mother lived in the Madison Senior Community Center for almost a year. Over that time period we had quite a few issues with this facility. The communication and follow through of the staff and management was dreadful. She was given the wrong medications on two different occasions and underwent surgery without the staff or our family even knowing it was going to occur. The overall communication between her physicians, and the nursing staff there was always hit or miss. Her hygiene went down tremendously over this year, with the staff members dressing her in clothing that did not fit, or that had stains, or sometimes not dressing her at all, leaving her in her night clothes all day. Her clothes would come up missing and get mixed up with other residents clothing. Her room was a constant mess. Our family had to clean and sort laundry every time we visited. When there was an issue, it usually had to be addressed with management at least three to four times before it was rectified, if at all. The money a loved one pays to stay there is significant and does not match the quality of care they receive.

Very poor care

My 89 year old father was in Wyngate respite care for 6 days. Staff let him fall twice in that space of time, and when I insisted on a bed alarm they did not respond when it alarmed. There was no intake assessment so that they knew what he was capable of when he came in, so they tried to force him to do things he has long been disabled from doing, like getting in the shower and getting out of bed and going to dining room for meals. Conditions were often unsanitary as used gloves were left on sink and counters, wet linen brought back to his room, and flecks of poop on his blanket. My grandmother got excellent care there years ago, but things have drastically changed.

NOT A GOOD MEMORY CARE FACILITY

After a few months mom's prized feather pillow with her name on it disappeared, as did her glasses and pricey hearing aids and a lot of her clothes. She was involved in an altercation and even though other woman lived down separate hallway and had no problems walking, they made my mother walk to opposite dining room. They never checked for UTI after her personality changed. I requested a check, they never did. They told us she had to be moved to their other facility which does NOT accept Medicaid and tried to fine us for moving her to a different facility even though they already told us she needed to be moved. Prior to move, and again instead of getting her properly diagnosed, they made us get a very expensive 1 on 1 service. So not a good place for those in need of memory care.
