My father was in acute rehab in Care One for a few months..the biggest problem is they are UNDERSTAFFED resulting in NEGLIGENCE! Although the staff is relatively friendly, they are overworked and do not have the adequate time to give each patient timely and appropriate care. Sometimes after hitting the call button my father would have to wait over an hour to be changed, sitting in his own feces. And this is when I was there trying to find help in the hallways! It does no good to bring up your complaints internally because they will simply "Yes" you and say they are doing something about it yet they never do. My father suffered bruising from being roughly handled and was sometimes in bed for over 18 hours without getting. I WOULD NOT recommend CareOne for accute rehab, especially if you are not there all the time to monitor the care or lack of!! Although the PT staff was good, they often would have to wait for my father to receive the help to get up out of bed in order to have his time with them which sometimes he would miss because of they did not have the time/staff to get him ready in time. Pitiful. There is no doubt in my mind if I were not there every day he would not still be with us today!

We did not have a good experience here. They had a real problem when it came to communicating with families. You really had to stay on top of them if you wanted any sort of updates regarding your loved ones status or care. They would reassess your loved ones care level without even a discussion, and the only way you'd know is a letter in the mail telling you that costs had gone up, effective next month. After we got one of these letters it was very hard to get in touch with anyone to discuss it. They also have issues with staff turnover, and inconsistent care quality because of it.

Though Care One at Wayne was an adequate facility for my grandfather’s recovery, their handling of the Covid-19 crisis was absolutely unacceptable. Two weeks into my grandfather’s stay, they began barring visitors from entering the facility, which they did act early on, but on FaceTime calls with my grandfather it was obvious that social distancing practices were NOT taking place within the facility. The physical therapists who assisted in the FaceTime calls were within inches of his face, wearing NO masks. Who knows where employees go after work, what family members they come in contact with, and what they may be carrying. Temperature checks may be effective for symptomatic carriers, but with a long incubation period for presenting symptoms and asymptomatic transmission, it is useless. Not until the last week of March did employees become mandated to wear face masks and PPE. (Side Note: During a FaceTime call, the PT who was managing the call for my grandfather was wearing a mask but sneezed centimeters away from his face and did not bother to move away nor cover her mouth additionally.) We relentlessly brought up our concerns to the social worker, [name removed], and other staff members as cases in nursing homes and long term care facilities rose in the state. We were constantly reassured that the facility was ‘air-tight’ and taking all necessary precautions. Speaking with another employee, we were also told that there would be no new admissions of patients to further prevent bringing the virus in. When cases grew dramatically, we moved to get him discharged, but were met with resistance from the staff even though he was cleared to leave within a few days. He was in perfect health and had no reason to be there, besides his own willingness, and potentially be exposed to the Coronavirus. The additional time that he was to spend in the facility was by his own choice of extension and not necessarily recommended by the health professionals or mandatory. He simply enjoyed the friendly staff and had good insurance that would cover it. In my opinion, patients were overly comforted about the risks of contracting Covid-19 in their “safe little bubble”. As Gov. Andrew Cuomo said, ‘it only takes one ember to start a wildfire’, and one ember it did. On [date removed], our worst fears were confirmed when we received a call from Care One alerting us that there was a positive case of Covid-19. We were told that the positive patient had been brought in from the hospital days earlier with an already suspected case of the virus. Why would a long term care facility, already vulnerable, accept a symptomatic and presumed case of Coronavirus? This was entirely shocking as we were assured no new patients were being admitted from the hospital. Staff informed us that this patient was being isolated and on the opposite side of the facility that my grandpa was on and that any potential outbreak was under control. At this point, it was absolutely necessary for my grandfather to return home, even though employees had told us that they felt ‘at ease’ with the situation. During the discharge process, we asked the social worker to be informed of any outbreak arising in the 14 days following his departure to help us better monitor him and assess the level of isolation he’d need when returning home. She said she would and made note of it, but never did. We thought maybe it was a good sign until the New Jersey Department of Health released the numbers for infections and deaths at every long term care facility in the state. We were appalled to learn that Care One at Wayne had 12 cases, and as of writing this their total is 30 cases and 5 deaths, likely not including employees or those patients who were discharged amid the outbreak. It is absolutely disgusting that within the 14 days of my grandfather’s discharge he was not notified of any outbreaks to take further precautions with his health. He did receive a strange phone call, in which a staff member questioned him on his health a little over a week after his departure. He is 91 and lives with his elderly wife, who could have also been put at risk. For a healthcare facility to sweep cases under the rug and hide it from patients and family members is irresponsible and could prove fatal. This is exactly the type of behavior from these types of facilities that led the Department of Health to publish every case and death in long term care. Trying to hide and diminish infectious outbreaks to avoid negative publicity does exactly the opposite. Grocery stores, including local Trader Joe’s and ShopRite’s, have done more to notify customers of possible contact with positive employees and shoppers than Care One at Wayne, a healthcare facility did. Just to make this clear, the issue is not that Care One at Wayne contracted and had an outbreak of the virus, as no facility is immune. It was inevitable. Instead, the issue lies with informing patients and their families about possible risks to their health during a global pandemic. The 14 days following possible exposure are CRUCIAL and to go into it blind, especially at the age of 91, is reckless on part of the facility. The patients they care for are part of the most vulnerable population and the utmost diligence should be performed when caring for them. If there was exposure to a possibly fatal illness while a patient resided within the facility and they are still within the incubation period of contraction from when an outbreak or community transmission occurred, they SHOULD be notified and followed-up with.

When shopping for a home for my mother I was told by Lie #1-[name removed], in admissions that CareOne Moorestown was an "age in place" facility. After 2 months I got a call saying they could no longer care for her, she needs to leave. Lie #2- they report to The County Office on Aging that they do not accept Medicaid, but told me they do. Lie #3- [name removed] told me there is a "bed hold rate" if residents have to leave for hospital or rehab stays. This was a reduced rate from the regular rent, separate from personal care costs and med management. My mother fell while in rehab, they transferred her anyway to assisted living and the next day ended up in ICU receiving 2 blood transfusions. After one night in A.L we "took possession of the room" and none of her belongings had been moved in. She was in CareOne rehab until her 100 days were used up, so this enabled them to collect room rent from our private pay and and Medicare for rehab. They refused to take responsibility for transferring her to A.L instead of sending her to the hospital.

My father, 88 yrs. old, had Alzheimer's and laryngeal cancer. I dealt with the social worker at St. Mary's Hosp. in Passaic to help place my father. I asked if Care One would be able to accommodate his needs. She spoke with someone there & explained his situation, so I felt they could take care of him. But after getting there late evening the respiratory doctor told my brother & I to go home, get some rest, they know their jobs, they'll take care of him. They didn't have two important medicines he needed when we got there & I had to drive home and bring them back! He needed one-on-one care and they could not accommodate him, expected me to live there 24/7. At one point when he tried to get up during the night when I wasn't there in the beginning the nurse or aide tried to get him to go back to bed & injured the top of his hand really bad, scraped the skin off! When I requested several times to speak with [name removed], the director, she was never available. He had a trach & could choke at anytime. All they did was give him meds, check his vitals. The PT girls were very nice & helpful. The head nurse would just tell me they can't take someone off the floor to stay with him. Well, they knew his illnesses and should have just said in the beginning they couldn't accommodate him! Also, at one point I tried the suction machine and it wasn't working. "I" had to inform the nurse and she was able to fix it but that took time and if my father was choking he wouldn't have made it or would have suffered lack of oxygen and maybe passed out. If your loved one is able to be on their own it might be a nice place. If they really need to be cared for, find somewhere else. Some pluses would be the snacks they offer in the front foyer and there were some very nice people who worked there. The facility is probably like most of these nursing home/assisted living places where the bottom line is money. After 8 days in this place I requested how do we go about discharge immediately and they offered to have an aide from an agency stay that night so they could work on discharging my father with whatever they have to do for that. I reluctantly agreed but went back at 10 pm to bring clean clothes & meet the aide, and while I was helping my father with things I looked over at the aide in the chair & her eyes were closed like she was sleeping. I said, "excuse me, what if you are sleeping and my father gets up?" She said, "he has an alarm on." I felt she was useless, getting paid to sleep on the job. I was totally exhausted at that point and went home to try to get some sleep because I was bringing my dad back to my house where he could get decent proper care. No, I would not go back there.

My father was here for around a month. It was one of the worst experience we have ever gone through. He did very well from a heart valve surgery, but once here he had a sore that festered into a stage 4, two fist size to the bone bed sore!! Do not trust this place to care for your loved ones. He never made it to his hip surgeries, because he ended up not surviving all the infections acquired due to this sore. Care One relinquished all responsibility. I remember a doctor yelling at the nurses for something and at the time I did not know what. We found out!!

It was a horrible experience for my father at this place, While he was in rehab they came and assessed him. They told us they would be able to take care of him. Within 3 hours of moving in, he fell and ended up in the Emergency Room. They did not have an alarm for him in his room, he fell several other times. I even had to pay extra to have a caregiver with him 12 hours a day. I would not recommend this place to anyone.

Year 2 - Gave it a second chance - mom came home malnourished and crippled. They were supposed to walk her 15 minutes a day and feed her. My stepmom had been sick for a long time suffering for the effects of a massive stroke 5 years ago, that surprising she survived. Comment: This is a review for the Care One Nursing Home - which comes up as Care Morris AL when you search for it on this website. I have been visiting my mother who has been living here for about 7 years. I have to say that the one consistent thing you can count on at this facility is that it is always chaotic and unsupervised. There are certain staff members who try their best to support the residents but THEY are not being supported by management. There has been a new Administrator almost every year she's been there and every Administrator fails to lead the staff well - the current one included (Tim) whom I've never seen once, has never taken the opportunity to get to know the residents and does not respond to any questions or concerns. This facility has been appropriately written up to the Dept of Health numerous times and guess what happens? All of the "staff" you never usually see crawl out of their offices during the DOH visits and put on their best behavior. It's atrocious. The Board of Officers and Owner should be ashamed of themselves. Why can't they get supervision in line and give the residents and staff the support they deserve?

I wouldn’t send a dog or my worst enemy here! You’ve heard the old saying, “Don’t judge a book by its cover.” Well… don’t be fooled by the nice appearance of this facility! They know how to “talk the talk” here, but they sure can’t “walk the walk.” We visited the facility and were impressed by the beautiful setting and appearance and we were assured that they would have no problem handling a patient who came to them for therapy as a “max assist” after a stroke. What we found was that this lovely facility on the outside was a real “hell hole” when you were on the inside. There is a serious lack of respect in the way patients are treated. Mom came from an acute rehab center (Marlton Rehab), where she received excellent rehab and was treated respectfully. Whenever she needed to go to the bathroom or get into or out of bed, someone was there to help her and it was no problem at Marlton. She was taken to the bathroom when she needed to go and was helped in and out of bed several times a day at Marlton, so she could change positions and rest. Upon arrival at Care One, we suddenly found that my elderly mom was expected to stay in bed for 12-14 hours, then stay in a wheelchair for the remaining 10 to 12 hours. Ridiculous! NO-ONE stays in the same position for that long and remains healthy. Suddenly, at Care One, Mom could no longer get a shower, but could only have bed baths. She could no longer use a toilet or even a commode, but had to use a bed pan! This was demeaning and a move backwards, when in the immediate days and weeks after her stroke, she was able to do these things! The difference was not in Mom’s abilities, but in the abilities of those around her. Response time to call bells was long. Though some staff members were polite and helpful, there were staff members who treated mom roughly and who were extremely rude and nasty to Mom and family members. Mom was initially placed in a double room, but her portion of the room was no bigger than a closet! The hospital bed was against the wall, leaving her paralyzed side to the wall. Next to the bed, there was not even room for a chair! We had to stretch/crawl across Mom to try to prop her bad hand on a pillow and staff could not properly get to her to care for her. It was a completely inappropriate setting! Therapists were unskilled and negative. They continually gave the same cues to patients, when those cues were obviously not working. There was a serious lack of positive feedback. Physical and occupational therapy were lumped together into one marathon, exhausting, session, rather than being split into morning and afternoon sessions. Therapists work with numerous patients at the same time, walk away from patients while they are supposed to be working with them, even leaving the gym area. Patients spend much more time “resting” in a session than they spend doing therapy. The gym is very busy, packed with therapists and patients, making for many distractions for both the therapists and particularly the patients. Patients were not given a schedule for therapy and were often left sitting for hours, being told that therapy would be “soon.” We started trying to get Mom into another facility after the very first day, but unfortunately, it took two long weeks to find another placement. Every step of the way was a battle here! We met with director after director, but the place was still horrible!
