If your loved one has any behavior issues this is not the place for them. They are definitely way understaffed in my opinion. We explained my parents issues before they took them but they assured us they could handle those issues. 2 months later they told us they could not care for them after all and we had 30 days to find a new place. Most of the staff is really nice but some were unkind and kinda mean to the residents. I am sure the assisted living is nice but the memory care is only for those alzheimers patients that will follow rules....unfortunately my parent was not wanting to be a rule follower.

The facility isn't great at providing what they say they do... they make communication very difficult. The activities team does the best they can at providing activities - and sometimes it's just difficult with what they have to work with in staffing levels and resources. I wouldn't pick this facility if it wasn't for the fact that my loved one needs Medicaid/low income spend down at some point. They promised a 30 day, 90 day reevaluation and health check in with me and my loved one... it never happened. An occasional phone check in with family member would be helpful. Having more front desk support or call-in support after 5pm would be helpful. And new carpet in the halls. The transportation options are so so limited.... at least one outing a week would be nice. Some months it's only been 1 a month. Even a "trip to the bank, or some basic errands" support would be nice. Middle of the night "on-call" button response was a maintenance man, not an actual nurse support. They gave vaccine clinic and vaccinated my loved one without knowing for sure if she had been... I was mad about that. So they don't keep up very well with medical records when someone is more in the independent living side but still struggles. They are very nice to her and would never do her harm, it just it not what we had hoped for. Another positive is someone that did not need to help stepped in to help my loved one use her phone. I would just like to see some incremental improvements.

The community is constantly making excuses when I have a problem. They keep saying it is because they are new. That should not be an excuse to make mistakes. I feel that the staff does not communicate very well I have to prompt them for any answers. The community has also changed the dining by taking things off the menu and making it smaller so that they can speed up the time for dining. Timing on meals did improve but it is a very limited menu. Matt Director of health services been great with me but the resident says he never has time. The other medical staff is constantly making mistakes, that affect residents health. I would not recommend my friends or family to this community.

We had a very disappointing experience with The Gardens at Barry Road. They were one of the facilities we visited while looking into memory care options for our loved one. When we visited they were very encouraging and said that they were completely willing to see if things would work out for our loved one at their facility. We scheduled an assessment with them, but when the day of the assessment came they called us and told us that they refused to do the assessment since my loved one was beyond the sort of care they could provide. This was a huge letdown for us, we wanted to get assessments at a few different facilities and we were counting on them to be one of them. Based on our experience I would not recommend them, they did not seem professional or reliable.

We took her out after about 6 weeks. Inconsistent care. She fell one night and no one responded to her buzzer. Medications administered at random times. Since some medication needed to be given 30 minutes before a meal and medication was given late, she missed some meals. Those are just a couple of things. I live in California and had to fly out to Texas to take her out of the place.

After only 2 months we moved our mom out because the assurance we were promised never happened. She was helped to shower once while living there. She could hardly eat the food because it was too hard to chew. Her meds were not given consistently and even got the wrong medicine once. Very disappointed.

Bethesda Gardens was good the first month my mom moved in. Then things started to go downhill. The facility is called Assisted Living but has become more of a Nursing Home/Memory Care facility and this place is not equipped for it. Biggest issues are with limited staff and issues with the dining room. If this place was truly Assisted Living then current staffing might not be an issue but since so many residents require way more needed attention than Assisted Living more staff is definitely needed. The place lacks efficiency with the way it runs things and has no plan for when Caregivers call in sick, take vacations or resign. Dining room has very limited staff and even when they use some Caregivers and Med Techs to fill in which is most of the time it still is not enough. Then Caregivers and Med Techs are not available to do the job they were hired to do. The food in the dining room is not great. My mom barely eats what is offered so I end up bringing her food just so she gets some nutrients. She does like most of the deserts and chicken fried steak and taco salad when they are offered. Games are the main form of activities but I hardly see the residents playing them. An outside volunteer group comes in about 4 times a year to set up holiday parties which my mom and many of the residents seem to enjoy. Overall, I am disappointed with Bethesda Gardens.

My mother lived there for several years and recently moved out after the rent was raised repeatedly and services, staff, amenities and activities were cut to the point of being unsafe and detrimental. Theft off porches and patios was common place, rides to critical appointments cancelled regularly and finally stopped all together and the food service became awful with rotten lettuce in salads, stale bread and under and overcooked food served at most meals. Just not the place you want your loved one to live out their golden years.

Lifestream only cares about money. They raised rents on January 1, 2024 and I had only been there a couple of weeks. Social security doesn’t go far and this increase really hit hard. Just recently they started renting units at much lower prices than what current residents were paying. Also they were allowing smoking on the property even though the rental agreement says it’s non smoking. It’s not enforced. Main office administration don’t return calls to residents that leave messages or put requests in writing. I’ve moved out and would not recommend Lifestream to anyone. I’m sure the newer residents will see a price increase as soon as they get fewer vacancies.

Staff does not follow Doctors orders. Staff does not regularly check on residents. Aids do not go in and offer clean cloths, showers in mornings and does not go get residents undressed for bed in evenings. Leave them in same cloths for days. Take room oxygen off and does not put resident on portable tank to go to meals. Directors answer to complaints and criticism is "well maybe you should find some place else for them to stay". assistant director provides nothing but LIP service. We have had multiple meetings with these idiots and all it does is get worse. I wish someone from Corporate would spend time in this place and investigate all of their employees from top down. Director has no business being a director of anything!!! Stay away from this place. You will regret placing your loved one here. They don't care about anyone, just collect a pay check. Not only do they not provide the care they charge they are raising the rates. State needs to investigate them!!

The facilities are getting run down. The dining staff and food is less than subpar. We had to have fast food brought in because they couldn't provide us meals. The worst mistake of my life was entrusting Mother to the care of Director [name removed] at Collinwood. While the facility is nice enough to look at, a few of the seriously overworked employees are making near heroic efforts to take care of some of the residents, and apparently the director (Director [name removed]) is good at making her management happy (by making them as much money as possible, including spending as absolutely little as possible on the residents), when it came to giving residents like my mother an optimal quality of life, Directory [name removed] failed miserably. Mother deserved better than the treatment she got there. When I respectfully pointed out to Director Jacoby and her staff specific issues that should be addressed, e.g., cooking chicken so it was not raw, not having hair (human?) in food served to residents and their guests, allocating more than $1.67 per resident per meal (their numbers) (out of the thousands of dollars per month residents like my mother paid), OUT OF

The marketing sales person gives you a lot of information that ends up not being true by the nursing staff. The assisted living community is supposed to be assisting the residents in my opinion. We were told that they would check on the residents every 2 hours; however, the nursing staff stated that they wouldn't be checking on the residents every 2 hours, and that the resident needs to be advocating for themselves. The staff would check on the residents during the medication drops which was approximately 2-3 times a day. The staff saw that my loved one was on the floor, but my loved one said they didn't fall. They staff didn't try to assist my loved one one back onto their feet either. My loved one ended up having to go to the hospital. The community stated that they would make arrangements to the hospital, but the family would be responsible for making arrangements to get my loved one back to the community. The assessment paperwork wasn't signed by the nurse that did my loved one's assessment. I found this out afterwards.

When we first got here we had a very hard time finding parking. There was a little side yard off of the front door and the staff were all standing there smoking, it just felt unkept. They did not look or seem professional at all. We peeked in the front door and the wheelchairs were lined up in the hallway and people were just sitting there slumped in their chairs and that really bothered me. When the director came out to talk to us I felt she was very unprofessional and kind of rude. I got a very bad feeling from her so we ended up not continuing with the tour.

This is where our mother currently lives. They are horrible. Three Executive Directors, three memory care team leads & managers in an year in a half. Currently no Activity Manager or personal. The are using employees from the Assisted Living side to fill in. No training on memory care. They do not know the residents and their needs. We would not recommend.

I would want others to know that we had a bad experience here. They do not care at all for your loved ones especially if they are combative. When I went into his room they were not cleaning so it was very messy, I had to give him showers and clean his room myself. His clothes went missing when we were here, when he would go to the bathroom or dirty them, they would toss them out. They also changed management 3 different times during our stay here. I also witnessed abuse twice when I stayed here overnight when they did not know I was there, I contacted their director about it and they said they reported it to the State. She was only suspended for 2 weeks. I would not recommend this facility to anyone.
