Most of the staff are very good. The other residents are super friendly and made my loved one feel welcome immediately. The activities alone make it a great value and my loved one loves the food, too. The communication has not been consistently stellar, but once in a while they hit the nail on the head. I would recommend the facility. Once we got over the bumps, things have been great.

The majority of the staff (front desk and office ) are so friendly. You can tell they are passionate about their residents and community. I believe the housekeeping is okay, don't let all the bling and beautiful remodel fool you. Communication with the family was amazing in the beginning, however possibly due to staffing, it has gone downhill rapidly. The move in process was pretty simple, they assisted. The food downstairs seems wonderful, however my mother was moved upstairs to the Memory Care floor shortly after moving in. The food upstairs does not seem to be the same or as good as downstairs. At this point, I am not sure if I would recommend The Falls.

My sister has rapidly declined since being admitted. I was very concerned so had a meeting with staff to find out they have my sister on schizophrenia medication. Not dementia medicine, which she has. My only conclusion is that they are chemically restraining her? Because at one point she was in a zombie state. Since our meeting they have reduced schizophrenia medicine.

The reason we gave MiddleBrook Farms a 3 is because housekeeping was lax. Also we took our loved one to the hospital on a [date removed], we had gotten sick in a garbage can, when we went back on [date removed] the vomit was still there and not cleaned up. With the housekeeping being improved we would definitely recommend this community to others, the rest of the care was excellent.

• There are 2 elevators 1 of the elevators was broken for weeks. • Doesn't seem like they can retain employees • The food is awful and there are no snacks out during the day i.e. cookies, fruit etc. for a resident to just grab • The "all day" menu does exist but it's a huge "imposition" for them when you want something from the kitchen • Activities are sub par. • I try to go once a week and use the kitchen to make a decent meal for my mother and another resident and her husband. Generally the cabinets in the kitchen are locked so you have to hunt someone down. I need to bring my own pots/pans, napkins, serving bowls etc. because they don't have any. Sometimes there are dirty dishes in the dishwasher because it hasn't been run. I've had to unload the dishwasher (not a big deal). • On two occasions I've been there when a resident has "gone missing". I was at the front desk one time and watched as three staff talked about it. Not one of them went outside to look around. Turns out the resident made his way to Starbucks to get coffee because he doesn't like the coffee at the falls. • Often times the front desk is left unattended • If you call for an attendant it can take 30-40 minutes for somebody to come. There is no way to differentiate between an emergency and someone just needing something non- urgent • The system to call an attendant has been down at least 2 times that I know of in less than a year • Parking is extremely limited for guests • when they have "happy hour" it's at 2:30 in the afternoon • The new bistro seems to be for looks, I've never seen it used • The staff is generally very friendly however, you can see some of the staff are just burned out and don't seem to enjoy their job • They seem understaffed yet keep taking new residents

My family and loved one have had an average experience with Crescent Point. The community was clean when we would visit our loved one. There are meals and activities to assist the residents with socialization. I would say the nursing staff and CNA's could show more motivation, interest and personal focus on residents under their care. People should not just drop their loved ones there and think they're ok - they're not. You get very little for the expensive price tag! It's a shame.

My loved one likes the food. They have a lot of activites, and things to do. We picked this community because it was close, we were in a hurry to find a place, and they had good reviews. Their tranparency with prices/cost was not good. They were very expensive. They need to improve on letting people know the actual price upfront, opposed to changing it afterwards. Other than that, he care was good and they called when it was any problems.

We picked this community, because of the proximity to home, services offered, it was comfortable, and welcoming. Very good care, but overly expensive. Need a Recreational Therapist- the one who was there was exceptional but she left - sadly not under positive conditions and and was not replaced. More activities are needed for residents. More training is needed to teach the aides and require them to de-escalate behaviors of some of the residents whose behaviors cause disruptions in the dining room during mealtime.

For example, there should be low-salt, no-salt, vegetarian, and non-dairy options available even without a doctor's note imposing a strict diet of that nature. Within the last month, there has been a huge turnover in staff, with upper management. There has been a lot of inconviences, due to renovation taking place. There needs to be better commnucation with the residents, with any changes going on with the renovation process. Someone needs to be more sensitive, to the needs of the residents. I am really hoping that the renovation process is nice, and the staffing issue gets resolved. The current staff is friendly and hardworking.

Benchmark is understaffed. When we came to them with concerns, I think they listened but couldn't really correct the understaffing issue quickly. Maybe they don't pay well or employees leave because of the understaffing. I'm not sure. We moved our loved one out of Benchmark because of the understaffing issue. Her care was sub-par and we had several instances there.

This facility is a good place, I really liked it. Mom was happy here and she loved it. They were good to her. There were a few times where I had to advocate for her to get things done. And I feel like her Physical Therapy could have been better. But it all worked out. They had some really good Aides here who worked with mom to help her be mobile often, she loved to walk. They also had a lot of good activities and always had something going on each day. Mom loved the exercise class. I would recommend this facility to family and friends.

My loved one has a debilitating illness, and we were disappointed by the lack of understanding from the care providers. For example, he can't move his legs. And when they moved him, they continued to tell him to move his legs, over and over again. They just weren't willing to take the time to listen to what he had to say. He's as easygoing as they come, but they didn't have compassion for him living in a body that just doesn't work, which is frustrating to him. I didn't see the nurses doing things to meet his needs, nor was I educated on what I could do to help meet his needs. The front desk staff were very friendly, but the care providers not so much. The cost was higher, yet the quality of care was significantly less in terms of respecting him as a human being. When I would talk to them about my concerns, they responded positively. But there seemed to be a disconnect between administration and caretakers, because we just didn't see results.
