The building has been thru a tremendous amount of changes with staff as well as management it’s no consistency within the building. I observe staff members on the cell phones while visiting on the floor and then sitting. Staff members are being over worked and burned out. Whats advertised is not what they have to offer. The level of care can be adjusted with the level of compassion for someone love ones. When having conversations with management things get brushed off as if you’re delusional. I often visit and see residents left unattended and soiled. Activists is amazing and the food have its hits and those two departs being ran by some amazing people

It was a rip off. Mom was there 3 days and we removed her for inferior care. They did not refund us the $[removed] that we paid. $[removed] for 3 days!! The staff were rude, incompetent, lazy, money grubbing, greedy. Also a very high turnover rate. They had no regard for their patients.

My mother was there for two weeks with dementia. She did not understand the band and why she should push the button for assistance. BECAUSE she had dementia. My brother and I were with her from 7:00am until she went to bed. While there, we tended to her needs. We dresssed her, took her for walks around the campus, took her to the dining area and other care she required. Whenever we pressed the button for assistance, no one came, so we took care of the matter. She fell out of bed three times while she was there and the third time put her in the hospital with all kinds of bruises. I removed her from Inspiritas the very next morning and brought her back home. I don't know if the fall had anything to do with her injuries, but she got weaker and passed in one and a half months. Because she was only there 2 weeks and not cared for as I thought she would be, I did ask for the deposit but they declined. It's a beautiful facility and the staff were nice, but they were short of staff.

I honestly don't know where to start when giving Capital Square (which then changed to Silver Sands, then changed again to what is now Your Life at Tallahassee) a review. It was a very sad experience for Mom and I. Dad was transported there from the VA in Orlando on Friday before Easter wknd. Mom had paid the [fee removed] entrance fee and was told that he would have a room but he didn't when he arrived. Sadly Dad was there for only 3 1/2 hours when we received a call that he had passed away in their livingroom. Mom and I were devastated. My 84 yr old Father had Alzheimers/Dementia and needed full time care that they agreed to give him. They couldn't even provide a room that was already paid for in advance so I question now if they even have the capability of caring for others loved ones. Mom has tried to contact them numerous times for reimbursed or atleast retroded back the money she put forth so we can pay for my Father's funeral. They have neglected to return her calls. Please reconsider.

The transportation van was broken and the salon was closed during his 6 week stay. The areas where public is much cleaner than resident areas. There was black mold in the ceiling in the dining area. Staff at front desk more friendly than resident area. Over medicated resulting in falls, incontenence and misbehavior. There are a few resident care techs that try to do a good job, but majority have their nose stuck in their personal phone. I visited facility several times when the care techs did not know where he was located. I h a d to get a care tech to let me out of floor. Two care tech in common room nose in phone. A resident in a wheelchair was leaning forward and would have fallen on her head if i had not yelled at the care giver. The residents did not receive enough food. [name removed] was fed many meals on a saucer or small bowl. He lost 10 lbs 1st week there. They didnt ask if they wanted more food or drinks and when they served the residents proceeded to serve themselves. On one occasion a worker was fixing a go box with more food than was served to 4 people. Some residents did not have an appetite and did not finish their meal. The underserved residents would eat their leftovers. When i was there iIwould bring it to the servers attention and reported it to on duty RN. The lady in charge activities was very nice and helped me with other issues like finding his lost clothes' that were in laundry. Tom seemed to enjoy activities. The management is very lackadaisical. There needs to be more supervision and guidance in resident area.

My Mom was a resident for just about three years, having moved in about 6 months after they opened when they were known as Your Life of Coconut Creek. The aides were mostly wonderful. They all knew Mom's name and were typically helpful but seemed overworked. I often questioned the staff-to-resident ratio but was always told that the facility met the minimum state requirements. While at Monarch, my Mom experienced two unaccompanied falls, the last one, in [date removed], was life-ending. Safety protocols appear lax and there is no way for a resident to call for assistance during the night when both falls occurred. We were told that our Mom could have yelled for help. I found this to be a totally unacceptable, unprofessional, and uncaring response. There was no way of knowing exactly how long my Mom was on the cold bathroom floor, by herself, scared and in pain, until the next bed check. The security bracelets issued to residents are totally ineffective unless a resident is near the door.
