We feel most fortunate that Sylvan Crossings - Fitchburg had an opening for our mom, Orla Erickson. We (speaking also for my sisters) were impressed with the staff's friendliness and caring attitude toward the residents of Sylvan Crossings. Ryan Wagner, the director, is very professional. Easy to work with. Staff has gently worked with our mom to orient her to her new surroundings. We are very impressed with the quality of the food that she is served; Mom has had swallowing issues. Staff are careful to watch over the food she eats. Cleanliness: wonderful. Also, Nathan, Mom's caregiver, is very good to Mom. Understanding. Accepting. Thank you to all staff here.

I toured Sylvan Crossing at Westshire Village and was impressed.

My wife is being cared for very well at this community. For the most part I'am comfortable with her move here. It is a very nice place, everything is very well maintained. They do a very good job with the cleanliness. I really like the staff. They have been very helpful, and great to work with. I know she is safe, and in good hands. I don't see why I wouldn't recommend this community, it is a nice place.

She is doing very well at Sylvan Crossings of Stevens Point. Once she got settled she adjusted really well. She is making friends. The staff help her a lot. They are informative when I call and ask questions. Everything is going well so far. She is getting along very well here. A lot of the family goes and visits because it is a good location. In the facility she was in previously she stayed in her room a lot. Now she is helping with flower arrangements and is really getting involved there.

Sylvan in Waunakee was by far the best of three facilities in Dane County that cared for our Mom, who had dementia. Rising prices forced us out of the first, the second had staff that was young, overworked and unguided. Sylvan is small, personal and well-run. It was marginally cheaper than the other facilities, but it surpassed them in providing services that Mom actually needed. The staff genuinely cared about Mom, enough to take the time to coax her into taking a shower if she didn't want to, or to go have lunch in the dining room to socialize a bit. Mom met and interacted with other residents in the living room, a first for her. Mom started to thrive again at Sylvan, which took the family by surprise. Then, when mom's physical health took a nosedive, we received timely weekend and after-hour phone updates from Sylvan staff, followed by hands-on aftercare in her room from people who already knew her when we were afraid that we would have to move her to hospice. Sylvan isn't big or glamorous, but it turned out to be the perfect fit for our family.

We're incredibly happy. The enthusiasm and eagerness with which the entire staff there deals with the residents is beyond belief. I never expected that in a nursing care facility. It's not as sterile as I expected. It's very homey. Jack is very happy. They're extremely friendly; I would rate them as 6. I was quite shocked at how reasonable the cost is. I don't specifically know about activities. He doesn't use recreational facilities there. He's too sick, and he's introverted; doesn't tend to participate in games and such. They've been very responsive when I've raised a question or concern with them; they've responded immediately. The location is just down the road from us, and we're there every day. I would recommend this to anyone who is looking for a safe and happy place for care beyond assisted living.

I'm very pleased with them. I think the value is first rate! I like the neighborhood and the cleanliness, the fact that it is small and the staff work independently and together. If you have a need and the administrator happens to be there the administrator will step right in and help. We haven't encountered any problems. The level of expertise is relatively high, I think. The caregivers are well trained and they show concern for the people. They have several entertainers come in. My husband enjoys playing bingo and doing the exercises. The food is very good and they make adjustments for people with allergies.

The staff is very patient and gentle. The residents have a wide range of needs: paralysis, dementia, young people with mental deficit that require supervision,...The outdoor common area needs updating and paint. Showers and room cleaning are once a week. Bedding is kept very clean and fresh. The facility needs maintenance. I am repairing the closet door that was off when mom moved in 2 months ago. I'm concerned that the window screens won't keep mom in. They're probably fine because she never opens the window.

There have been a lot of issues finding and maintaining staff just like everywhere else right now. They do their very best - but it has been a struggle. They are having a hard time finding an activities director so there is not a lot going on. The cook that they have right now is really good. The director is amazing, she is great as far as communication with the family and addressing issues as much as she can.

This communities was okay and it was much more homey. It seemed a little disorganized but we had already fell in love with another community before we came here so it was hard.

I took a tour of this community for my father, and his needs. The staff I met was kind, and informative. My questions were answered. The community looked nice, and everything appeared to be well-kept. The experience was a nice one overall. I did find another community that suited our needs better, but my tour of this community was a good one.

Unfortunately my father in law was only in this community for 11 days. He wasn't happy, so he moved back home. However I did feel this was a nice community. Its a smaller community compared to others, but it is a great facility overall. I would recommend it.

Staff was friendly and you could tell they care for their residents. residents seemed happy there. bit on the expensive side and rooms were studios. we are looking for 1 or 2 bedrooms so my parents can stay together. wall paper in entry is a bit busy and home is locked which is good for those who tend to wander but more of a hassle for those more independent. staff seemed very outgoing and used residents names. director has dog there which gives a family /house type atmosphere

Sylvan Crossings does a wonderful job of personalizing their care to each resident. They have a unique and special point-of-view that when loved ones with dementia are exhibiting behaviors, they are trying to communicate their wants and needs. They take the time to assess the situation, and make any necessary environmental changes in an effort to make that person more comfortable. I will definitely refer more patients here in the future!

When we toured this community, we liked it but it was small. There were only about 32 residents. Also, it was far away from where I live, so I would have to drive a distance. They referred me to the one in Westshire Village and I really liked that one.

We toured this community for the increased needs of a friend. The individual rooms were very spacious and bright. The staff seemed friendly and knowledgable. The common living areas seemed small and cramped.

My family and I are pleased with the support and care being given to our family member. The environment is comfortable and the staff have been caring and helpful. We needed a place that understands the difficulties around advancing dementia and the staff here have worked well with our family member. We have been kept updated and are included in the care planning.
