Location was perfect, caretakers were wonderful, but room available was too small for our needs.

The house was clean and inviting. It covers everything i was looking for.. I have two concerns. 1. Not enough interaction, 2. cost what happens when the VA or other funds run out.. will medicade cover ..?

Director was a lovely lady and very helpful and the home was beautiful. Her prices were much higher than we expected and because of that we have ruled this location out.

The staff was great. Experience was very positive for 2-3 months. Then management stopped doing upkeep and repairs. They lied to me more than once regarding things they were in the process of resolving. Food qualities went down, microwave broke and was never repaired, oven temperature would not register. It was a mess. It then came to a point when they stopped paying the staff. This lead to more false promises and lots of attrition, I was personally in Touch with the COO and registered complaints with the ombudsman. Unfortunately my mother passed but I have been told the house shut down and residents / Staff were moved to various locations. The 2 rating was because the staff was a 5 but everything else registered at a ZERO in the last few months.

Poor nutrition options. Too much starch. Too much staff turnover. Poor communication between staff members. They left Silver Leaf a few months ago and went to [facility name removed]. It was much better.

Avg. expected more in terms of people listening and acting upon requests. Avg food. Best part was Home Health Services (physical therapy). We picked this community because of the location, furnished room, and price.

Our loved one was not really happy with the food, and this community was not just stimulating enough for our loved one.

They tell you what you want to hear but the quality of care isn’t what they say it will be.

I have had nothing but problems with this facility from the get-go. Before my Mother was supposed to move into this community she was supposed to have an assessment done. Well, the evaluation ended up never happening. The other problem I had is the staff ended up giving her the wrong dose of medications. I feel like there was just one problem after another. One of the reasons why I had to bring my Mother here is because she wasn’t eating or drinking and this facility doesn’t even monitor any of that. After my mom passed, I spoke with the owner and he said that he is supposedly working on these issues. However, all of the staff members are great with the residents. [Removed], the marketing director was great and helped us out a lot. I just think that there is a management issue. I would not recommend this facility to other families.

We did not have a good experience in this home. It is a very nice looking home, they do a good job with keeping the home nice and clean. I feel like the staff needs to be more compassionate towards the residents. He wasn't satisfied with the food, they need more of a variety to choose from. The communication with the staff was not good as well. The director did inform me that he was going to try to improve things.

Jeff is a passionate owner and cares about his residents. The house is obviously still not totally renovated and i wonder if he can maintain the house as a business. I would look for his history as an owner before placing my husband there.

I would recommend you stay away from the community. We moved my loved one from a hospital setting due to being discharged. There had been a COVID outbreak prior to my loved one being there, and the community didn't let us know. according to a caretaker The community didn't make sure to change my loved one's clothes. I felt uncomfortable with sharing our experience. The community would just restart the clock on any complaints we made. The community was able to provide vaccines against COVID for the residents. There is a community fee here. It was a nightmare for us.

The owner stopped paying bills and employees were not getting paid. The staff was nice and the home was beautiful but the owner let every thing go. No maintenance and not paying bills. Most residents moved out when they tried to tell us when we were getting legal notices from unpaid vendors

We moved out after 2 days. They gave him the wrong medication multiple times and never responded to calls for help in the night. Medication- no excuse! Night help- before I moved him in I was told someone would always be awake and available. One person was working and she was sleeping x because she had to work the next day! Horrible management.

[date removed], my mother passed. [date removed], the facility withdrew $[removed] for payment of November. Requested a refund and told we signed a contract, no refund if the contract is terminated. Posted online of the deceiving wording and received response they would make it right financially. We have not received a refund, told they would only refund if we removed the review and provided a 5 Star rating. Filing complaints with [removed] for deceptive wording in the contract and violation of the Consumer Review Fairness Act. Investigation determined rules or regulations were violated. It also included additional incident complaints and investigations on record for this facility from others. Silver Leaf Assisted Living (Lake Point) has 30 facilities in the Dallas/Fort Worth area. They have no intention to honor guarantees, no professional courtesy or integrity to acknowledge or respond to messaging, they violated the law please tell me how can you trust the care and safety promised?

All style, no substance. My dad was there for a week in "nursing care". He was never given a shower. The food never included a green vegetable. He fell twice and no one could help him, 911 had to be called. I had to change him and his bed every time I visited. It is a beautiful facility and the senior living area may be different, but nursing was a nightmare.

Our family had an in-person tour and Lake point assisted living was not going to be a fit. The woman giving my tour rambled extensively about residents getting into each others rooms and "having fun" and residents walking through the woods on their own. I was not impressed with the staff but the building is beautiful. Real shame.
