This Home is managed by a R.N. who also has many years of experience with wound care and long term care of clients with multiple medical issues. The caregivers are selfless, loving and nurturing. This Home has been a God send for my mother's care. I am very happy to let others know about this quality of care.

Mom has been there for about two months. So far, so good. She is alert, clean and seems happy. The facility is open and bright. It is like a home with the smell of good food. Sunlight abounds. The home smells fresh and clean: no urine odor which is so common in these facilities. The caregivers seem to know their residents and they are very positive. We really like the openness of Alexandria Care and we are encouraged to visit frequently. Maureen is very responsive. Our questions are answered promptly. At this time, our family is happy with our decision to change our Mom to Alexandria.

They could use more activities like chair yoga or something where the residents can get a little more active. It is conveniently located for my family. The room is big, bright and maintained very clean. The staff schedule is really good they have 8 hour shift instead of the typical live in for a few days or week at a time. This way the staff is not burned out by the end of the week or a live in shift like at other communities.

My feedback is based on only a 3 week stay. The house is beautiful. But a few things could improve. There aren't really any activities at all. There isn't a provided cable/TV service. Asking an 87 year old to learn a Roku is ridiculous! My mom would have been 100 xs happier if she had been able to watch tv and her regular TV shows. For the price, I thought this was stupid. Also, twice I had to call them about mom falling by the bed. I had installed a camera and we just happened to be watching. The staff is busy and isn't able to monitor residents very well.

Facility was very clean and presentable. Nursing staff was very helpful and compassionate. My family member said food is bland. Activities were not consistent with calendar schedule.

If I look back on the experience he had at the facility, I realize his acuity was too high to be there. He fell frequently because they had their hands full with the staff they had to care for everyone. I had hoped it'd be okay but even if they went down the hall to help someone else, he'd get up and fall. Alarms etc were not enough. Most of the staff were very friendly and caring about [name removed]. They spent time reading to him and sitting with him when they could. They would call me when something happened. I think my perception was skewed about level of care and expected too much at times.

They are very friendly, helpful people. They need to improve on the food choices but based on the experience we have had so far, we would recommend Silver Leaf community to others who may need a place for their loved ones

My loved one is currently a resident at Silver Leaf Assisted Living at Church and the overall experience has been good for her, so far. However, there is no way to easily contact the staff for any issues, even emergencies. Also, they removed the daily nurse after we got there. This was a selling point for us.

This home is a well kept, recently remodeled (within 2 years) facility that is bright, clean and spacious. The staff is very attentive to the residents needs and schedules for medications and treatments. Routine support such as laundry and stocking of necessary supplies is good. The food is great, being set by a professional chef. Due to the number of residents (there are 8 different rooms for the residents) and two daytime staff, the amount of recreational activities is limited. Additional stimulus and planned interaction for the residents would be beneficial. The overall score for this group home is above average and is recommended.

My loved one is currently a resident at Silver Leaf Assisted Living at Church and the overall experience was good. However, they changed management companies and the level of overall care has decreased. We are currently seeking other options that might provide more activities and interactive experiences.

Unfortunately my mom passed away before she had even been at Silverleaf a month. But while she was there I found the staff so kind and helpful. Not as impressed with the manager but I didn’t have many interactions with her. The facility is beautiful and the food was good. My only negative was that there weren’t any other residents up and about that could interact with my mom.

The staff changes a lot so no continuity in caregivers. People need to spend time with their loved one and get to know the staff. Don't be afraid to speak up if you have a concern.

The staff is caring and patient. We picked this community because of the location. I would recommend to others.

Our family had an in person tour with Silver Leaf Assisted Living, and overall it was a nice tour experience and nice community, but we just liked the pricing and staff at another community for our loved one

Except for the turmoil with the staff, I would recommend this facility to anyone with a parent needing 24/7 non-nursing care. It was the only facility that I could find that would care for a person with a peg tube without a RN on-site 24/7. The interaction/communication with the staff was great.

Lake Point is a facility with 16 rooms which I think is a plus. They say they have 4-1 ratio caregivers to residents but that's too often not the case. There are no activities and no real stimulation. They do try very hard to make it a "family " feeling. The management is easy to talk with and they try to help with any problems that arise. All in all, it's the best place for my husband.

Monticello West seems to be the perfect place for my friend. She has dementia and the staff is always very helpful to keep her engaged in activities, both physical and mental. My friend is very happy and her brother came for a visit and he met with the staff and is very happy with the place.

Silver Leaf is a very small community. The staff I met on the tour were very nice and accommodating. It seemed like it would be a nice place to be. But my mom would not have liked that the temperature control is not in each room - the temperature is set for the building as a whole. And the rooms were pretty small.

The people at Silver Leaf are very nice and patient with our loved one. They may not fully have a menu set up to handle diabetic dietary restrictions but we would still recommend this community to other families.

I had a very good tour experience at Lake Point Assisted Living. It was a very nice facility. We saw the food and it looked incredible, the meals are all chef prepared. We just did not think they had enough activities to keep our loved one busy which is why we did not choose them.
