Oak Park Place

The care staff is trained and if they are seeing someone that is just not picking up on it quickly enough - especially with new staff coming and going all the time they address it quickly and work to train them efficiently. They have good communication and that is something we have noticed. We do think the value for the money is good and we would highly recommend them to others.

I would tell others come walk through and visit. It's the greatest! Talk to the ladies who live there and sit out front in the nice weather - they'll tell you. They're friendly and greet you as you come. I was impressed walking in the door how clean and organized it was. And it smells good, which is unusual and a big deal. The facility is spotless. My mom loves her girls who come in and take are of her. She loves them, and they love her as well. The care services have been great, and they've even prevented a few negative health outcomes. I honestly don't feel she would still be with us if those had occurred with her still at home. We really don't have anything negative to say. If she doesn't like a meal, they'll make her something else. But she says the food is usually excellent. Activities have been a little limited due to Covid, but she loves playing Bingo and tells everyone about it. They do take an elderly waiver, which is great. But even without it, I think it's a good value. It's everything we need it to be for her. And that's what matters the most - that she's getting what she needs.

When we first moved in, it was awesome. They had a slight issue with staffing retention, but it did not last for long. They take good care of my loved one and the community is clean. They should improve in making sure the residents look presentable, as I have noticed my loved ones hair has not been combed. There are photos online of the residents engaging in activities, but their hair is not up to standard. My mom always had her hair done and took care of herself, so I do wish they would do that as well.

6 months ago, I was asked to take a Lymphedema case at this facility. I was beyond impressed the moment I walked in. The atmosphere was so inviting & the staff was extremely welcoming. I knew from that first day, I wanted to establish a relationship with this company. Every one was approachable & I never felt like I was inconveniencing anyone. Management works so hard to make sure the care needs of all of their residents are met. [name removed], Director of Healthcare Services & [name removed], Director of Housing are a dynamic duo!! I can’t say enough about the activities department…one of the best around! [name removed] the beautiful lady at the front desk is one of my favs! [name removed[ Therapy is now one of the preferred therapy providers at this location & we are blessed to have a strong relationship with this fabulous company! If you’re looking for a top notch Assisted Living Facility in this area, look no more! Make that call & schedule a tour with [name removed] DOS! You’ll be glad you did.

I would recommend you check out the community. The staff and friendliness are amazing and if I could rate them a 10 I would. [name removed] was our first contact and [name removed] who took [name removed] place was just as good and we were thrilled with the transition.

I would say to forget this community and to never even think about it. I would not recommend this community.

This community was nice but it was too far away for me to access my loved one.

My mom had an emergency and after her hospital stay, we were able to get her into Oak Park Place rehab. The staff took great care of her and was always in communication with us. [name removed] , our case worker went above and beyond to make sure we knew everything and actively coordinated her doctor's office, therapists, and her new memory care residence. thank you to all the staff for getting our mom strong again.

My mother was sent to Oak Park Place for rehab after being in the hospital for a fractured pelvis. We then had to search for an assisted living facility and Oak Park Place graciously let her stay until she could be placed in her new facility. They took very good care of my Mom and our family is very grateful to the whole staff.

I would recommend you take a look at this community. Oak Park Place went above and beyond for my loved one. I was very pleased with the care my loved one received. My loved one was able to make friends with the staff. It was a great experience for my loved one. Most of the residents were older than my loved one., as well This is a very clean community. Oak Park Place worked with my loved one and their finances.

The community is very accommodating and the staff attentive to the residence. the room sizes are good and they offer different room sizes also. I would recommend this community to others.

During the time our loved one was they were very good, and the staff members were excellent. Towards the end we had Unity come in and they very informative, which was very nice. From what we could see the activities seemed good, and the management there was very good. When our loved one passed they made sandwiches and desserts for our family.

The community and the layout was very nice, and it was very welcoming for the guest. The administration at the facility was very good, we were very impressed with how the organized things to get the families and the Green Bay community involved. The folks who worked directly with our loved one were wonderful, we would highly recommend this community to others. They had happy hours where the families were encouraged to come. During the holidays like Christmas, they invited families and the entire Green Bay area for meals, cookies and hot chocolate. They even had a reindeer petting station.

Oak Park Place Madison did a wonderful job with the care of our loved ones. Everything was taken care of and available for our loved ones in the community. The exercise classes was one of the many activities the community offered for the residents. Our loved ones enjoyed the programs in the afternoon that the community had prior to the pandemic. My loved one really enjoyed the community and everyone was friendly. All the staff were easy to talk to, and answered all of our questions during the move in process.

We thought Oak Park Place was very good for our loved one. The community brought in musicians weekly and my loved one enjoyed this very much. If you can afford this, you should go here. I felt they did a better job in this community than one in a big city.

The staff was wonderful. Take the corner room. It has great windows on two walls. They have different floor plans for you to choose from. The food was pretty good, they have lots of menu options. They have an outdoor space for residents to use.

Unfortunately because of COVID 19, I could not tour the facility in person. I enjoyed the "Skype" tour with [name removed]. I was able to ask questions before, during, and after the tour. There were times when the feed broke up, but she was able to repeat what she told me during that time. What was nice is that I could ask questions right away when they came up while I was touring the facility. She was very friendly and informative. It lasted about 40 minutes and was good.

Have many great activities, clean with friendly staff

Oak Park Place has been excellent in every thing we have needed for care for our loved one. They communicate well with each other, patient’s doctors, nursing staff etc..beautiful facility with a very bright and clean environment. They have great staff who seem to enjoy working there..

So far we have been very happy with the care and rehab of Oak Park.
