Everyone was kind to my family, and I during our tour of this community. The community looked nice, and everything was nice, and clean. I did feel this would had been a nice community to consider for my father, and his needs. The staff was friendly, and my questions were answered. Everything went well overall.

Caring For My Mom Well

My mother is enjoying her time here at this community. It is a very nice place, and I'am pleased with her move here. It is a good, safe place with kind people. I know she is being cared for very well. There are some activities available for the residents, but I think they could use more of a variety for the residents to choose from. The meals are good. It has been a good experience for us here at this community. It is a community I would recommend.

This community looks nice and up kept. My tour was with the director who gave me full attention and stayed focus through out the tour. I did like this community but the distance and not being a new development changed our decision.

Great nurses but unexpected costs

Right now I would say it's fair. The price ended up being much higher than I expected and what we had discussed. There were some optional things added on without asking us first. We're trying to lower the bill now that we are more aware of what triggers more costs. The nursing staff seems good. It's a nice building/apartment. If the bill stays that high we won't be able to afford it.

nice community, needs more educated staff and more nutritional food

There aren't really activities. The meals are chinssy- cheap frozen things. A lot of times they're is only one staff aid. They are currently painting. Not real tentative... the staff needs to better trained.

Our Experience

First impression is everything. I think they could do better at keeping the windows clean when you first come in. I've noticed some staff don't speak very good English. The person that helped with getting my loved one into the place was wonderful to work with and very helpful. I know the food was good. I know they've recently changed, so we'll see what happens to the quality.

Less than advertised

Against mom's wishes my brother moved her from another facility. Her room was nice and clean. We did notice one time that there was a large puddle of water in the dining room, mentioned it to staff and was told that someone would clean it. An hour later the puddle was still there and the staff I spoke to was mopping it. My mom was always at the same dining table. When table cloths were removed we noticed that her table looked like something that had been stored in an unheated, moldy garage. Her leftover food was left on the table for over an hour. Initial visit was during a supper. Aides were sitting around, one on the counter in the open staff area. Mom had few teeth, food should have been cut for her, instead it was left uncut which made chewing very difficult for her. She was in her 90s. Initially staff seemed hostile to us, only by being gracious to them did they treat us well, one staff spied on us, grateful to nurse who helped us get our mail to mom-brother kept mail f

Nice place but room for improvement

Chandler is a nice place, but they charge for every little thing. (Ex. Billed for a Band-Aid, billed for confirming a bruise was healed, etc.) Their response time to my parent's needs is not the best. Timing for my mom's meds at night is supposed to be around 8pm. Sometimes she doesn't get them till after 11pm. I do think they're short staffed. When my mom fell and fractured her ribs, there was minimal communication from Chandler to me. I needed to keep contacting the hospital to find out when my mom would be released and how she would be transported. Also, It would be nice if someone/the staff engaged with its residents more, made them aware of the activities going on, and encouraged their participation

Disappointed

When we were looking, the main criteria was that the facility accepted elderly waiver - that was one of the biggest deciding factors. They said they did after 2 years of private pay. We signed the lease and I wanted to get more information on it. I called and they said they had no availability - so if money runs out and there are no openings we would have to move her again. That is a very big deal and it felt like a bait and switch to us. They never told us there were only limited spots available and that they were full. I think they could have communicated that to us better.

Darkly decorated. Consistent smell of deodorizer throughout the building. Room available was too far away from elevator. Very close to where I live. More expensive for less sq footage.One bedroom with den available. However, it is an apartment further from our dining room. No in unit laundry. Indoor parking included

Mom is settling in

I was pleased with the appearance of the place but kinda seems discombobulated at times. I'm sure they are working on the issues. Like the time that my mom and others wait on their food it has been up to 25 mins just waiting on the food. But when she gets it, it is good. The staff is helpful. It is kept clean and the grounds are well maintained. She enjoys the activities like playing cards and games. They had a fundraiser not long ago that was lots of fun and the directors were welcoming . I would recommend this community.

Homesread of Maplewood

I have found that the " Homestead of Maplewood" does a pretty good job of engaging their Memory Care people in activities during the week. They have a lot of people who come in and entertain the residents. Which is wonderful. The thing I find most frustrating is on the week- ends - their staff is so much younger and they don't pay attention to what is going on with the residents. My sister's Walker has disappeared for the second time in the 7 weeks, she has been there. It has been gone for 6 days this time. It has her name and her apartment number on.

Beacon at Mapleton

A lot of times there's only one person working and so the residents don't get looked after as well as they should be. They don't have an activity director anymore so the only thing the residents can do is watch TV or color. The food is not the best they they did remodel or I should say they did some painting. We had a camera in the room so we could see how early they'd put her to bed and they wouldn't even get her up for breakfast. We were not impressed at all.

Lack of Communication

During COVID I wouldn't recommend this community. The isolation and lack of community for our loved one wasn't good. They did allow virtual visits. The in person visits were very sporadic and not set up very well. This didn't make it comfortable for everyone.

Avoid this facility like the plague!

Let me tell you every reason why you should never put your loved one in Pleasant Manor (PM). My mom only lived there under 2 months before she passed away, & the trouble started the first day. First off, the facilities are not clean, there is literally junk and garbage everywhere, it smells horrible, and you are lucky if you can’t get help from staff. We even witnessed a resident in a wheelchair get stuck in the door at one point with no help to get out. The majority of the rooms are very cramped with little space- they’re shared between 2 people with a curtain as a divider so you have very little privacy and bathrooms are shared between 2 different rooms. There are no phones in any of the rooms. They are EXTREMELY understaffed. My mom would press her call button for a nurse/aide and she would wait anywhere from 20-40 minutes for anyone to come to her room and help. I witnessed the long wait times myself on multiple different occasions and witnessed multiple different patients in the hallways asking for help from the CMA's and aides and they were always told there would be a wait. Second was their lack of communication in all areas. I can give multiple examples of this. They failed to schedule a parascentesis for my mom. Her discomfort got so bad that she was barely able to eat anything. They finally scheduled it but failed to communicate to them the medications she was on so when she arrived they weren't able to do it. Third- Her dentures were thrown away by a staff member at Pleasant Manor. She had set them in a cup next to her bed & apparently they thought it was garbage. They never called me to tell me about this, and I had to find out from my mom the next day. I made numerous calls to the Director of Nursing and the SW at PM to make complaints as things were going wrong right and left, and was always told “we are sorry, we are short staffed, we are working on it, we are sorry we threw her dentures away, we will get her in to the dentist to get her another pair which we will pay for.” She ended up getting her replacement dentures from them no less than one week before she passed away, and they didn’t pay for them like they said they would, they billed her dental insurance. Fourth- During her stay at PM she had 2 hospitalizations, 1 because she ended up getting an infection (not PM’s fault), and the second time because she needed stitches on her leg after they scraped it doing a chair to WC transfer (PM’s fault). Neither of these times was I called to be informed. During the hospitalization for her infection, PM called her hospital room to ask us if my mom would be coming back & if they should hold her bed. I don’t know if this is standard practice by a SNF, but I thought this was extremely unprofessional and upsetting. After being discharged back to PM on hospice, 2 days later my mom called me crying saying the nursing and staff at PM weren’t helping her with her pain management saying there was nothing more they could do. I ended up finding out through hospice that she ended up back in the ER and needed stitches because of the WC transfer incident I mentioned earlier. She only lived for another week and a half. On the day of her passing, one of the RN’s from PM stated “Has hospice called you yet about her expiring?”, “Oh I thought they would have called you about her expiring.” I am aware that ‘expired’ is a term they use in the medical field when it’s related to death, but with how they treated my mom & the experience we went through with them, it just felt like she was an expired piece of food to them and it was very unsettling and upsetting. Was my mother very ill and didn’t have much longer to live? Absolutely. Did they make her last months of life extremely difficult? Absolutely. Did they contribute to her passing quicker? You can make that decision after reading this review. I would NEVER in my right mind put any loved one in this place & I am so glad I never have to speak with them or step foot in this facility again. I have told everyone I know about our experience and will continue to do so.
