- next door to nursing facility that takes Medicaid - most likely be on 3rd floor (common areas visually confusing -room hallways too plain - can't tell what door is hers) - most people looked [removed] and wheel chair bound

My loved one was previously a resident at Lorien Harmony Hall and the experience was not great. My loved one was not receiving any therapy and I was unhappy with her lack of progress. I ended up moving her to another facility. Based on the experience, I would never recommend Lorien Harmony Hall to others.

Lorien bills [business name removed] as a Skilled Nursing Care center and not as a rehabilitation Center. Consequently if you expect [name removed]to pick up any deductibles after Medicare they don't! You will be liable for [cost removed] not paid by Medicare!! What skilled nursing care did my wife get? Well the so-called "Skilled Nurses" most often would just call for an aide to help her get to the toilet as they walked away! As far as the 5 days a week of both Physical Therapy (PT) and Occupational Therapy (OT) she was supposed to get. The sessions were often for far less than the requisite 45 minutes and many week she did not even get such services for more than 4 times,

It is heartbreaking because this used to be a really great place! My mother was there for rehab and AL a few years ago and the staff was so caring and compassionate. The process to get her in AL went smoothly and you can tell they cared. My friends mother was there recently and it was a horrible experience. Breaks my heart because I recommended the place.

My mother is at Harmony Hall and I am not happy with the facility or many of the people on staff from management treating me as if I work for them to the arm twisting to pay more. Many of the employees are not happy due to the conditions which they have to work from too many people that they have to service to not enough support for the residents. The management does not answer many questions of family members and the director always states that she will get back to you when you have a question, but never does. The answer is "I will investigate it." This is a constant complaint that I have heard from other family members of residents and from current and past residents. I have asked for the last two years the cost break down for the level of care and I have not received this information in writing.. I am investigating other facilities due to the management of this facility and the constant harassment to pay more without a clear definition to what the cost will cover. Something must be done. Just because a person is aging doesn't men that they should be taken advantage

This place is clean but, that's about it, it's not personal at all it's like a huge hospital, no one know what's going on. to many residents. When you ring for a nurse wait 20 minutes for a nurse to get to the room, it's not cozy or homey at all, you get this lost feeling for your loved ones ??

My loved one was living here for some time until I found the community was not able to handle residents with dementia. They should try to understand people with dementia. It also was expensive, they were the most I have spent for care.

The lack of care is depressing. If you have a loved one do a hard pass on this facility. They are understaffed and and ADL's are not being done. Showers may or may not happen and residents in skilled nursing are left dirty. Also, no memory care help yet they have a dementia population

From the management to the food service to the RN’s to the support staff, this is a place to avoid for people you care about. If you want to drop someone off, never visit, get overbilled, see your family experience malnutrition/weight loss, stress, isolation, assistance calls go unanswered, temporary staff being rude and unavailable, then this is your place. For everyone else with a conscience, with realistic funds, with expectations of care, compassion and services that match the exorbitant prices, there are better options almost anywhere. Do not believe the lies you get when you have an initial visit, as the owner (nepotism at its worst, as his father is the ceo) is only about $, not your needs, health, property or happiness. The few staff that are nice are leaving daily, replaced by untrained temps that don’t do their jobs. This perspective is shared by many residents, past and present, but fake reviews fill these sites. Just say no to this place.

Horrible. I regret ever falling for the bs sales pitch. Yes it all looks so pretty but the staff is [inappropriate phrase removed] of the administrator, who cares about nothing but having an easy job and money coming in. He does not care about his staff or the residents. Everyone in contact with my parent complains about him and the way things are run. Food is pathetic and the dining staff offers non-defrosted food and wonders why anyone complains. Aides and nurses don’t help each other and residents suffer by being forgotten, not getting meals or assistance for the bathroom or showers. Turnover is weekly; anyone good at their job leaves. The staff is condescending and obnoxious and treats elderly like children. I think the people that write great reviews are not at Encore often or have relatives not disclosing. After a few months, I am taking my parent out. Anywhere is better than this place, and for the insanely high costs, untrained and uneducated staff, it’s been a waste of money and time.

We’ve encountered so many problems in this place. Here are just a few: 1) food is awful. My mother, who is a vegetarian, asked for a vegetarian menu. The menu lists many meal options. Yet she has been served the same meal (over and over and over) - a dried out black bean burger. 2) some (not all) of the aides have been condensing and impatient with my mom, which is demoralizing to her and her family. 3) there is no written schedule for physical therapy or occupational therapy. It’s impossible to know when the PTs and OTs are going to show up. My mom has not had PT for 5 days straight and there is no excuse for that. 4) my mom did not receive a shower for 5 days straight and now only receives a shower 2 times per week. 6. When we push the button for an aide or a nurse, it usually takes at least 30 minutes or more for anyone to arrive. There are some caring, nice and sweet staff. For example, [name removed] has been kind and responsive. But overall, this place not good.

As a nursing home they are severely understaffed. When your loved one has issues and you bring it to the administration's attention they say it will be addressed, but one day later it is right back to the original issue. Once the issues start adding up and it begins to become highly egregious the administrator will no longer address the issues and he will not take or return calls. To top it all off, our loved one was moved to hospice and when we went to get their belongings they could not find them. When we tried to bring this to the administrators attention, he refused to answer the phone. Never have I ever encountered such an inept individual in an administrative role in my life. For your loved ones sake please avoid this place at all cost.

I was a patient at Encore for several weeks and I don’t think anyone should have a loved one stay here. The night staff consists of two people to service one floor level of a wing (approximately 50 patients). I concur with other comments posted here about the food and medical care.

Aides and nurses object to doing their job. Immobile Patients rarely receive a bed bath (for example, I only had one bed bath during my 27 day stay). Loud fighting goes on in the hallways any time of the day or night (usually involving staff who have disputes),. Patients are frequently left to lie in their own excrement for over two hours after a call button has been activated, staff frequently dispenses medication improperly and many staff members speak abusively to patients (I hear this from the halls or when i am passing by other rooms and I have experienced this personally).

My mom lived at Lorien Assisted Living for two years. The facility is nice, but the staff is overworked. Some are very caring, but many see it just as a job. My Mom usually has to wait 15 - 20 minutes for an aide after she rings. If she rings again, they complain that she rings too much. There have been times my mom missed meals because no one came to get her. Additionally, the highest level of care is no different than the lowest level, but costs 3 times as much. The administration is terrible and will charge you extra every chance they get. If a new medication is prescribed, sometimes it takes up to 3 days to get it. The food started out as decent but the quality and taste has declined. The activities are few and uninteresting. I would not recommend this facility if your parent needs any kind of additional attention or help.

This is not a very good community when it comes to care. They seem to only care about the money and not the residents. They were giving my loved one mashed up food that was just like paste. They did not get them up and moving around they just left them sitting all day. I was paying for around the clock care and all they do is sit them at the nurses station and most of the time no one was there. I really never dealt with the nurses or doctors I dealt with the person in financing. This place was a terrible experience for myself and my loved one.

Do not put anyone you love here!!! Do not think they do what they say they can do!! We chose Encore because they said they were certified for Parkinson's patients. The facility did nothing for my husband and in fact he had serious anguish issues being so isolated for such a long period of time. Thank God we were able to Face Time him and witnessed the many mean nurses and care givers who were abusive. They did not realize my husband kept his phone on when they came to deliver any care (not much of it). He showed us bruises on his arms and hands. The care at Encore is subhuman and there is nothing I could do to change a thing. My husband needed thickened liquids and they kept giving him thin liquids. He had to correct them. He had a swallow test done and no matter how many times I called I was not provided with the result. Family members and I were put on hold for 20 minutes or more at a time. They knew why we were calling and would not take our calls. I spoke to the Administrator and he was extremely rude and would not listen to my issues. I am not sure how or why he has a job in public health. He certainly does not care for the residents. I will say the physical therapists, speech therapists, and occupational therapists were good to my husband but that was only minutes in the few days a week they worked with him . Please do not subject to any one you know to this horrible facility.

My mother was a resident of Encore in July after suffering a stroke.Visually the facility is wonderful, the care or lack of care she received was absolutely HORRIBLE!!They basically wrote her off the day she arrived, she was incapacitated and needed a great deal of care.My family was there every day,on a daily basis I had to ask if she would get therapy, had she been bathed, basic needs were not being met. [name removed] the head nurse was so nasty any time we would approach him it was as if he was being bothered. Not once in the 30 days she was there did she get a shower, she was to be rotated every few hours, that wasn’t being done,we had to practically beg to have her changed,I don’t think their staff had the proper training nor did the majority of them care enough.I walked in one day to find that whoever turned on her feeding didn’t connect it so she was covered in formula,no idea for how long. Her arm began to curl because they weren’t working with her as they should. When we asked to see notes on what was being done with her it was like pulling teeth,we went through every chain of command and our concerns were never addressed. The social worker never reached out to us when my mom arrived after several days we had to reach out to her only for her to brush us off. They were not administering medication properly, she was in a great deal of pain and refused to examine her,[name removed]laughed it off, after I pushed and asked the Nurse practioner to examine her she had to be rushed to the ER.,.The Adminstrator was very rude and combative when we tried to bring our concerns to him I couldn’t believe it!I filed a complaint with their compliance division and they never followed up with us.At the hospital it was discovered that her feeding tube was completely clogged up,she was severely dehydrated, she was in bad shape. They contributed to my mother’s demise. She is now deceased. There are so many other incidents but too much to get into.I would say stay as FAR away from this place as possible!!

We chose this assisted living for the beautiful location, and cleanliness of the facility. All went well for a few weeks then an obvious decline in my father was apparent. The staff seemed indifferent and uncaring. Unhappy with the care we took my father home with home care. Messages with supervisors were left unanswered, but the billing office made sure they called. Would not recommend this facility.
