Well, needless to say the covid lockdown has changed things a lot here lately. But overall we have been more disappointed gradually since the new director/dictator [name removed] came to power. This lady is always trying to sneak around or hide in the front office. She lets people freely in and out of the building with NO masks. No respect for us and that we are able to catch the covid easier than most. We get weekly letters which up date us on the situation, but no follow through. Now she wants to reopen things. The actual building is beautiful and has lots of amenities, a gym, aerobic room with chairs, a very nice bar and rec room. But some things have fallen off. The activities were never really good even before the covid, now my friend and I just see them push a cart around all the time like something was just thrown together. The food was delicious in the beginning, went down hill significantly, but has recently gotten better the last month or so, better dessert selection WITH sugar free options finally! We will probably be looking to move out if things continue in this direction. My friend that lives a floor below me told me her daughter put a review and that it did not even get posted. Real honest business practices I see, wonder what else [name removed] is hiding?

I’ve been here for a while now, almost 9 months. This place has significantly declined. It was very good in the beginning, the most disappointing thing is that after speaking with my neighbors- we are concerned that the reviews are being filtered/doctored. There have been several of us that have left reviews on google and they do not appear, and magically all these new fantastic reviews are popping up at the same time? Why hide the true resident reviews and create false ones about the executive director? We all gathered at the beginning of the covid epidemic and collectively wrote her a letter advising we we’re going to petition her out after the covid. She has been allowing visitors in under lockdown, not enforcing masks or social distancing, as well as not reporting the death from covid and the true number of cases to the public health department. THIS CONCERNS US!!! Over 2 weeks ago a resident death- NOTHING reported!! She hides the truth on all accounts, even down to review censoring I can’t believe, so deceptive! We are a high risk population and deserve respect and the truth should be told to others in the community as well that are looking for a new home.

I have been living at Clarendale for a about 8 months now. Things were great at first. All the staff was friendly and the executive director, [name removed], at the time really knew what she was doing. Things have drastically gone downhill since the end of last year. We got a new director, [name removed], who doesn’t seem to have a clue about anything. The way she speaks to us is very condescending. She has no compassion for us residents, especially during this tough time. She lets people gather in the front, doesn’t mandate masks and tries to cover up people having the COVID. We have already lost one of our fellow residents due to [name removed]'s lack of social responsibility. It’s very unfortunate. My neighbor told me her daughter put a truthful review on Google, but it never showed up. [Name removed] must be filtering these reviews so people don’t know the truth. I think it’s important that people know how Clarendale really is so they don’t get their hopes up by the pretty building. I have started looking at other places to live. The Radcliff seems to be very promising and the man running the building seems very friendly. He actually sat down and talked with me, which is something I have yet to experience with [name removed]. I am looking forward to the day I can move out.

My parents lived here for four months. The building is beautiful. The executive director could not solve the poor food quality. She hired a new executive chef and the quality declined further. Also if you have animal allergies, this is not the place for you.

My mother lived here for 5 years. At first, it seemed to be OK for staff and care. Management changed after a couple of years, and things went downhill. I would find her needing her teeth brushed every single time I visited. Complaints to nurses and even management did nothing. Signs in her room did nothing for reminders to brush her teeth. An overwhelming smell of urine often greeted me when I entered her room. I asked nurses why her underwear wasn't changed more often since she took diuretics. They claimed it was changed every couple of hours. Her hair was always dirty and smelly. Her fingernails were always caked with food. I requested that her hands be thoroughly cleaned after meals, and showed the staff her dirty fingernails. Nothing changed about that either. I left toiletries there regularly and discovered they were either being used for other residents or someone was stealing them. They would disappear within a week of my bringing them in for her. I complained about that, and nothing ever changed. Her clothes were all ruined with their laundry facilities, and I often found other people's very large sized clothes in her room. I had to search in their laundry facilities for lost clothing, and they had a bin that literally reached the ceiling full of clothing that was never given back to the rightful owners. It was overwhelming to search through that mess. A visiting nurse told me she would never put one of her family members in Walnut Place. That convinced me it was time to leave.

This place is horrible . I went to visit my mom , her room smelled of old urine like it hasn’t been cleaned in days. My mother who wasn’t able to feed herself was left alone with a plate of food in front of her and no-one cared to check on her. The staff made me very uncomfortable they kept asking me if I’m ready to leave . Met a woman there who was visiting her husband , the poor woman was crying because she couldn’t get these lazy woman do anything. My mother was elderly and sick but she died at this facility of neglect . Her legs turned black from knees to toes in 3 mos They wouldn’t let me speak to her when I called . I complained to [name removed], the manager but it went nowhere . If you care about your loved one don’t sent them to Freedom Square.

I would give 0 stars if I could. My mom lived here for almost a month last year. While she was here, she frequently complained that she would be left for hours without being checked on. When I would visit, I constantly heard people's call buttons going off without anyone responding. The last straw was her last night there when her oxygen dipped to critical levels. The nurse with her that night told my mother that she had too many patients (more than legally allowed) and that many were sicker than her so she would have to wait. My mom ended up in the ICU with fluid in her lungs the next morning. Since moving out, I received one bill that was literally handwritten with no itemized charges. My mom then passed away, and her bill was sent to collections. The facility will not give me itemized information on my mom or let me pay it off. The financial person takes days to respond. It's like a never ending nightmare that my mom ever had to live here, and I can't even get her correct balance paid off.

After many years of uneventful life in independent living (where the only problem was how the food went downhill after Brookdale took over), my mother had a stroke, suffered paralysis, and spent her final 6 years at the "Inn" - skilled nursing. Staffing was a problem, the nurses overworked, the aides as well (some of whom were desperately in need of more training). A supposed slip from a chair (mind you, she was partially paralyzed) broke her leg, and 90 days later she was dead. The wound was not cared for properly and became necrotic. Opiates were necessary to keep her painfree. The aide who "found" her was covered up for by the staff, even though the ones who knew us gave me the wink wink, nudge nudge when parroting the corporate line. Avoid skilled nursing there at all costs. Your loved one's life may depend on it.

You will be sold a lifestyle in the lap of luxury, with the "assurance" that your continuing needs will be met as you require more help. You must pay MUCH MORE for that and it is not worth it. My loved one was in Harbor Health rehab, on sight at Southport Square. My loved one was misdiagnosed with a swallowing disorder and fed pureed food and lost about 30 lbs.and was finally hospitalized after loosing conciseness for dehydration, which had happened 3 times before in their care. At the hospital we asked that a swallowing evaluation be done on our parent to put to rest what we already knew. Our parent did not have any kind of disorder as it was checked by the hospital over several days. Southport Square, Personalized Living and Harbor Health Care Pay their hardest workers the lowest. I have often heard employees being reprimanded for taking a little extra time with a resident and told,"I hope you are not putting in for overtime", can you imagine! In Assisted living, room, board and activities are included, but everything costs extra or Al la cart. Your loved one or family end up paying upwards from $4,000 a month for basically a studio without a kitchen. Add on CNA or Personalized living and you are looking at Thousands more a month on top of the $ you already spend. Shop wisely.

The room was very small in rehab - if the bathroom door was open you couldn’t use the recliner and vice versa when the recliner was open you couldn’t open the bathroom door. Same with the closet door and the room door. The cooling/heating system was broken and continued to be broken though the service man was called multiple times, the nursing staff was unable to meet medication times and the staff did not bathe the patient as was ordered. The dining room for residents was continually empty when we went to visit the Bistro to purchase a light meal. Inquiries with residents provided reviews that the food is terrible in the beautiful dining room. Therapy and therapists were excellent. Common areas are opulently decorated. Gorgeous property.

WARNING !! to any family member who has a person in the Park Lane side of Bellingham ! I had to get my mother out via 911 for a fractured leg when the nursing staff and administrators did nothing to help her after several requests. I reported them to the Pa Health Department, so they retaliated with the administrator stating " I am sorry HONEY you are no longer a daughter your a trespasser" when mom came back after three days in the hospital. They had threatened me and my mother prior to this to stop reporting them or "We will make sure you two never see each other again" I even gave them a cease and desist letter the harassment got so bad. When I reported what was going on there they retaliated making empty accusations targeting me . They are mandated by law to report unexplained injuries, they are mandated by state and Federal regulatory law to keep up the standard of care and prevent injury and diminishment but failed to do so so they were cited. They are also mandated by law to allow the resident access to family. This corporation instead of admitting the now cited deficiencies, isolated my mom for three years now and then again INJURED her by spiral fracturing her leg and again in the same pattern and practice not EVEN investigating or reporting it, the hospital had to report it. So even with others reporting them they hold their ground fraudulently calling the targeted family member an EMPTY adjective "disruptive" I was told I had to adhere to their "culture" (while they don't adhere to the law) The Pa Health Department cited them stating " Based upon clinical record review, review of documentation provided by the facility to the State Agency and review of facility policies and procedures, it was determined that the facility failed to implement its policies and procedures regarding injuries of unknown origin for two of two residents reviewed" SO IT WAS NOT JUST MY MOM. I would challenge a cognitive response from this corporate giant to why your "culture" attacks people who report you and does not take inventory on the internal patterns and practices that DID FOREVER HARM my mother and me. In addition how do you plan to return three years of our lives? Your nursing staff went out of their way to cover themselves at the expense of your resident and family. I have called your office and was hung up on MULTIPLE times, is this the "culture" your nursing staff refers to?

But my in-law were promised they would NEVER be kicked out! That was a lie. Do your research. They paid I believe $260,000 for their apartment plus they had to pay $3500/month. My FIL passed away in [date removed]. My MIL stayed living there. Last yr she started suffering from dementia. Monarch landing had her admitted to 2 different psychiatric hospitals. They finally said she can not return to Monarch unless she had a full time cna. That would cost my mil $1700/week!!! My mil volunteered in their memory care for years. They said she would not be fit to move in to their memory care because it would confuse her. She had NO choice but to leave. Of course after you leave you still have to pay that $3500/month for 3 months!!! They are also trying to charge her $10,000 in repairs! We moved her in with us in May, it is Feb and have yet to see her $260,000! Of course Monarch is still making interest off of her money! Of course she can't get the money until her Unit is sold then we have to wait for the grace period! It is a joke! Please be careful when choosing the right place.

Specifically, but not limited to: -- (Every Morning) Assist him into the shower and standby while he showers, remove his dirty clothes and place them in his hamper, lay out clean clothes and assist with dressing if necessary -- (Days) Incontinence prompting when he is in the TV area or dining area (there is one bathroom with no visual indication of what it is on the door, so your loved one would not know where the bathroom is, my grandfather was OK as long as he was in his room), assistance with going to the bathroom/cleaning up as needed -- (Every Evening) Assist him with his pajamas and place dirty clothes in hamper, remove dentures and soak them next to his bathroom sink, help him put on a clean undergarment My mother or I visited 4-7 times a week over the past year and regularly found him in dirty clothes from the day before (or a soiled undergarment), soiled sheets (or a few times sleeping on a bare mattress even though he had two sets of sheets). They did not remove his dentures nightly or help him into his pajamas (we would find him in bed in his day clothes) or help him clean up after using the toilet (we would). Everyone who regularly visits their loved one(s) has similar complaints (not receiving the services contractually paid for), and every time we voice concerns they go unaddressed. There is an extremely high turnover in the care giving staff, and unfamiliar faces from one day to the next. And to fill these gaps Clarendale of Mokena uses a service, which only makes things worse (these temp workers never know the lay of the land at Clarendale, let alone the residents and their needs). When you visit, your loved one will more often than not will be parked in front of the TV or sitting in the dining area, unengaged and abandoned. It got so bad that we had to go every day. Last week my mom and niece arrived shortly after dinner and were told my grandfather had gone to bed (he goes to bed right after dinner). They encountered him on the commode, helpless and soiled, and in his day clothes. So yet again no one had provided a service we pay for (i.e. assisting him with bedtime activities). He was in distress and they had no idea how long he had been there, but he pitifully said, "Thank God you came." My mom cleaned him up and got him into bed, then went to speak with the nurse on duty (who supervises the caregivers). As she passed a group of caregivers who were sitting together in the dining area, one asked how my grandfather was doing and my mom replied that he was not good and had been in distress. One caregiver (a temp) rudely replied "He is always in distress." The next day my mother called and spoke with the head of nursing, [name removed], on the telephone. [name removed] did not acknowledge that what happened was unacceptable, and went on to make excuses. When I chimed in that the situation was elder abuse and could be grounds for a lawsuit (I spoke calmly and was not yelling nor attacking, nor using profanity), she replied that that was uncalled for and HUNG UP ON US. This is not our first round of caring for someone with Alzheimer’s, my grandmother also suffered from it. Fortunately, my grandfather, mom and father were able to rally together and keep her out of a facility (including my grandparents moving in for the last two years). By the time my grandfather was diagnosed my stepfather had battled and passed away from cancer and my grandfather had been ailing for five years under my mother’s care and with a caregiver we brought into the home daily to give my mom a break when I could not. My mom had been a caregiver for almost 20 years at that point, and we finally had to investigate outside resources because his sundowning got really bad. So we know what love looks like in action. And we know what memory care should look like outside of the home. He is now in a new facility ([removed]) and the difference when you enter is palpable. The head of every department has been there 10+ years, as has much of the caregiving staff. They have won multiple JD Power awards for assisted living and memory care. There was energy and the residents have been engaged whenever we did an impromptu site visit before moving him. Clarendale of Mokena had lots of bells and whistles your memory care loved one will never be able to take advantage of, and you will pay for services you will not receive.

DO NOT put your loved one in memory care here. They say they offer it, and they have lovely marketing materials, and you pay for it, but they do not actually provide memory care. My grandfather, who is suffering from Alzheimer's, was a memory care resident for a little over a year (we moved him yesterday). We never received the services we contractually paid for (which were based on the assessments Clarendale required). This facility is not what it seams, if you want to know how this place really operates go after hours , you will get a real taste of this joke of a place , what they are doing is trying to sell u a room so they can keep the place full they are fake and don’t care about the residents from my experience don’t be fooled , we got Mom out!
