My wife was in Sonoma House memory care from [dates removed]. I finally decided to move her to another facility as, in my opinion, the quality of her care declined significantly, especially after January 2023. Also, in December, she fell in her room and was not discovered for some time. She had hit her head on something and had lost a fair amount of blood, and had to got to the ER. Turnover of staff and caregivers is a big problem at Sonoma House. My advice is to look for other alternatives. Find a facility that has separate cleaning and cooking staff, so that the caregivers can better watch the residents instead of doing other duties. Ask about how your loved one's care is communicated on a regular basis. I eventually concluded that Sonoma House was the not the right place for my wife. My responsibility - I should have done more homework before choosing Sonoma House. I now have her in a facility where I get daily updates on her condition.

My mother and father lived here and the care was substandard. We were promised housekeeping, my parents room was always filthy. They had no activities that were also promised. It is next to impossible to find assistance when needed and when we did find an aide, they were usually in the break room on their phones. Not pleased with the care.

My loved one and family had an awful experience. I wouldn't recommend the community to anyone based on the horrible experience we had. The administrative staff and head nurse kept telling me that they had plenty of staff, and there were times that I showed up and only saw 2 staff members and one of the staff members was the chef. The care wasn't what they promised. The showers and medication weren't given to my loved one for several days and one time it was two weeks for them to shower and changing my loved one's clothes. The community never called me down for a care conference for any concerns they had about my loved one. My loved one had to be sent to the hospital and the community never called me to let me know this. I ended up finding this information out when the hospital called me to let me know. The community promised me one fee and a bill came that was much higher than what was promised. The nursing assistants really tried to assist my loved one. The Activities Director and Janitor were very kind to my loved one.

When my family was hospitalized because I had to explain to the staff to send my family to the ER because their nurse wasn’t available! ( I am a nurse and this lady treats her job as a vacation! ) the hospital assessed and said “ she is coming from country meadow place ? We can tell!” Due to covid it was hard to move anywhere else . Our other family ( yes my other family died ) won’t move now because they feel they need to be there to watch their friends . Because no one takes care of the clients ! When you bring up complaints the manager acts like a car salesman ! Oh and he has no training in healthcare. Ask when everyone has had Alzheimer’s training and watch their face Iowa law says you need it within a year of employment! And don’t even bother asking about the meds they are giving your family they have no clue what they are . My family got am meds at 1pm Iowa law is two hour window before and after scheduled time . Please reconsider elsewhere !

Do not put family here! I had to beg to have the nurse get an order for a urinalysis. ( my family member had such a severe UTI it went septic The staff make tik toks in the halls or rooms with their masks sitting on their chin or off. One med manager told my family the med he was taking for a - fib meant he needed fiber.

It is really nice setting in the country but there was a very strong urine odor. It could be a better fit for someone more capable who is mentally and physically more independent. We were under the impression that they would provide more care. They told us that they would only be able to care for him if he was a one person assist. He would fall at least twice a day and they would find him on the floor.

Looked great at first, but our mother said the care was not what you paid for meals were terrible, chef quit, fed Mexican food 3-4 times a week. Elevator broke all the time and she was on second floor. Bed bugs on lower level by bingo room found in some ones room. how does this happen in an assisted living facility??? The maintenance man was the nicest man there and the most caring. The little gal doing the interviewing doesn't have a clue what is going on, or the Manager for that fact either. I would NOT recommend this place to anyone. We have since moved our mother , where she is getting great care.

They don't do anything they say they're going to do. My Mother in law had fallen right before moving in, and the doctor told us she had liver cancer. The Dr. in the ER told us that when she got to the facility, they needed to do a blood panel on her to see how bad it was. The facility assured us they would have a doctor come in within 2 days of her arrival to check her out, do blood work and have hospice come in. They never did. When we pressed the issue, the head nurse that told us this would be done right away, changed her story to “I never said that, I don’t have the authority to just call a Dr. and tell them what to do. The Dr comes once a month and if they feel it’s necessary they will order bloodwork. They never addressed the issue EVER. They didn't give her a life alert button around her neck in case she fell, so she couldn't get ahold of someone. She was in the last room of the facility, farthest away. If she fell or needed help, nobody would have ever heard her. They said they would check on her every hour, which they did not do, ever. They only came and got her for meals. We were there for, four hours. Nobody came to check on her. So there was no way for her to contact anyone when she needed help. The reason they said was so they wouldn't have to run down there all the time, but how would they ever know if she would ever abuse it? She never had one to use. It was a joke. They didn’t want to take time for her to walk that far to the dinning room, so they put her in a wheelchair there and back for every meal. After a month of them doing that, she had to have physical therapy because she couldn’t walk but a few steps anymore. We told them no more wheelchairs. One time my son and I went to visit, and they couldn't locate her. It took them 45 minutes. Finally found her in a strange bed, with no clothes on from the waist down. And they didn't look into it, report it or do anything about it. They shut the door so We couldn't see, but We had already seen and heard more than enough. I should have called the police right then and there. But I honestly didn’t know what to do. I put in several fix it tickets for her thermostat. It was over 90 degrees, and she couldn't sleep it was so hot. They opened the huge window in her room all night during wintertime. She could have easily pushed the screen out and fallen out, walked away into the timber, and they never would have known she was gone. There were no safety measures on the window, and it opened all the way, which was a good 3 ft wide and 4 ft tall, starting below her waist, So she easily could have pushed the screen and fallen or gotten out. Food looked fine, they are supposed to give the residents options with the food. But they just sat it in front of them and said there it is. Nonexistent activities, would play music and sing, and that's about it. The activities director was probably the only nice person in the place, and I really liked her. At least she tried. They just didn't give her anything to work with. I did have to call the police because I went to see her and I blew a gasket because her room was still 90 degrees and her bedroom window was wide open again. She didn't have any clothes in her closet - they were all gone. Nothing in her dresser drawer either except a pair of poopy underwear. There was no clothes basket. She went into the facility with 20 outfits. She was wearing a bra that wasn't hers, was 3 sizes too small for her, and was literally cutting into her skin. She had nothing to wear so they took someone else’s clothes and got her dressed. They told me that other residents come in and steal her clothes and that they couldn’t do anything about it. They had not given her showers for a month since she had gotten there. Their excuse was because they were "training staff." A worker stole all the brand new towels I brought for her. They looked all through the facility and couldn't find them. They didn't replace them either. We did file with the state of Iowa. The police found pills in between the refrigerator and the cupboard. We don't even know what kind or where the pills came from either. And it was filthy dirty. The apartment had not been cleaned at all. They also didn’t change bed sheets often. Visitors would notice that her sheets were stained with poop or pee and would get a clean set out of her dresser. She had 3 new sets. Thank God we found another place for her before they shut everything down for the pandemic. They were dirty. They were not doing aything for the residents. The entire resident population just wondered through out the facility. Other residents that we personally knew remained there and got sick and died from Covid. The facility death count was never reported to the state, ever. They never did any bloodwork on my mother in law until the new facility demanded it before she could be trsferred to the new facility we wanted her in. I or a family member went to see my mother in law every day that she was in there because they were not taking good care of her. They didn’t even brush her hair on a daily basis. They completely stripped her of her dignity. Even with us checking on her daily they were not doing their job of taking care of her. I can’t even Fathom what it would’ve been like when they locked down and wouldn’t let anyone in. That is an absolute horrifying thought. I wouldn’t put my worst enemy in this facility let alone a loved one.

The Activity Director [name removed] was one of the worst Activity Directors I have ever come across. Do not bring your family here if you want them to be or remain active. Not only does she not have the personality you would expect for an activity director she doesn’t even put forth an effort! We have had our mom at Emery for four months. As of today we are at 27 days with none of the electrical outlets working in her room. We have brought this up eight different times and are told that they are looking into it. They reported an injury to her foot but had no idea how it happened. Her doctor thought it was broken and did x-rays because of the extensive bruising. She has had a black eye, a bruised cheek and arm and we were the ones that had to point it out. We found moldy food in her room. We are trying to find a new home as soon as possible.

Very disappointed in the treatment of my M-I-L at Emery Place. She was one of the 1st residents to move into Emery when it opened 2 years ago. Emery Place made an independent assessment of her a few weeks ago and decided she needed to be moved to their new locked facility because she burned a brownie in her microwave and "failed" an assessment they gave her. The nurse at Emery Place appears to make all the decisions and is not open to any other opinions (including family). She gave us the choice of moving to the locked ward or moving out. So...we are moving my M-I-L into a facility that respects her independence and fulfils their commitment to increase the level of care as needs increase instead of warehousing our loved ones in a locked (and understaffed) facility. Do not heed their promises of being able to increase the level of care your family member needs as they age. What they mean is that they will require them to move to the more expensive locked ward in order to remain at Emery. My M-I-L has been "assessed" at several other facilities that we are looking at to move her to, and has passed all their assessments. I'm confident the reason Emery Place required her to move was that they have a waiting list for her type of apartment, and they thought they could fill their empty rooms in the locked ward with residents who currently occupy the coveted rooms. Our story is not unique...in the last 30 days this has happened to 3 residents. Unfortunately, they didn't have the family resources to advocate for them. Beware...

I think the facility is nice, however, the promises and claims made about the memory care unit, and what they are set up for and trained to handle are VERY exaggerated. This can have disasterous results for families who trust in those promises. Though some of the staff is amazing and exceptional with the residents, there are at least as many who have minimal training and little or no desire to learn ways to help your loved one through the difficult road that dementia takes them down. When they become overwhelmed with the needs of a given resident, you will be given a notice of 30 days to find another facility. I want others to be aware of this policy, since moves are so devastating for people with dementia. Every move is a monumental set back. Be wary!

She quit milestone because of them doing things illegally and didn't want to loose her license. So we have no one to help us with medical issues. They tell you that housekeeping is included but your apartment never has been cleaned because there is no housekeeper. Rip off!!!!

Over priced. Under staffed. Is run on a bonus system for management. They will tell you anything to get you in th door so there census increases, therefore personal bonuses for community directors go up. Run on a care point system. When you first come in care points are low but after 30 days they will increase drastically. Therefore another big bonus for management. Uneducated staff is responsible for medication administration. Have a track record of getting wrong medication or not getting it at all! Meals are not healthy or nutrious as management also get a bonus to keep meal cost down. Per day tenants are allotted$3 or less for all three meals. For what you pay to stay here you are beter off to stay at home and hire a personal caregiver
