Very good, personalized care, good activity, clean and decorated nicely. Staff was efficient and very caring. Meals were good. This was all related on experiences from when my parents were there (2 years ago).

My wife has been a resident since [date removed]. I visited 8 places and this was by far my choice. I was and am still impressed with the facility. The staff is exceptional and made the choice for me. My wife is receiving great care and has adapted well at Azura I would highly recommend anyone needing such service to visit this facility.

I am very pleased with the staff and services provided by this facility. My mom gets the attention and medical needs that she requires, without leaving the facility. The various programs offered are all great!

Mother is so happy to be at Azrua! She tells me every time I visit how nice everyone is. She says they keep her so busy she's "exhausted" by the end of the day and she sleeps very well. I'm happy that she's happy! I'm thankful for the entire staff who are always friendly and greet me like I'm family. I'm glad mother is getting such good care, 3 meals/day and snacks, has friendly people to visit with all day, a variety of activities to keep her busy. She can go outside in the beautiful courtyard for fresh air and exercise and is safe and happy!

The caregivers were so conscientious and caring for my husband. He was here 2x and only left to be closer to home. Later I moved him back to Azura for the better care they gave him. I also loved the outside area and that we could always take our loved one outside for a walk. Compared to others, this was the absolute BEST! The food was very good also.

I really liked that family members could come and visit anytime day or night. My dog brought smiles to many men and women in the community. She was always welcome after certain she had all her shots. Many looked forward to all of her visits! Whenever I was visiting during meal time, Azura gave me a serving free of charge. Thoughtful!

I can't say enough good things about the staff and facility. I can't imagine anyone taking better care of my parents. They truly treated my parents like family. I was touched to see how many of the staff attended my parents funeral.

My sister has lived at Azura for almost a year. She is getting great care. Everyone there - staff is very helpful and caring. I know my sister is in the best possible facility.

From the first visit with a welcoming assurance and understanding the difficulty for both of us to make this decision to the tears and comforting support with his passing, I found the staff of Azura from the top management to the caregivers and assisting staff to show both [name removed] and I their smiles and presence. I would and do recommend them to all I encounter needing this help. I am indebted to them all.

I very caring place that had always treated my Mom with dignity and respect. She thought of the caregivers and staff like they were her family.

I had power of attorney for a lady's health. I was at Azura many times during her stay there and would (and have) recommend Azura highly.

The staff here is wonderful. My wife has been a resident for over one year. I visit her almost every day and I have never heard one staff member loose their temper. They are great. The only shortcoming is that once Dad a while back they didn't finish making [name removed]'s bed. (Very ready and is already flipped promptly).

When we moved Mom to WI from AZ and chose Azura, we were entirely uncertain about her cognitive state. We didn't know how much independence she wanted or how much assistance she would need. Azura provided the exact blend! Mom loved being there - said "I don't ever want to move again." Fortunately she never had to. We love Azura!

Other than continued staffing shortages and turnover, I find that the remaining staff does an excellent job in meeting the needs of my wife and other residents. I have and continue to recommend Azura Oconomowoc to my friends even though I live on the property of another "memory unit."

Good location, can see dad living there, has a large number of residents to find things in common with like sports, their careers, books, card games, etc, there were more lunch options than at other places including a soup and salad bar that had a lot of his favorite items on it. Relaxing areas to socialize including a back deck where the deers and birds are fed, looked just like his backyard at home. Rooms were good size with assistants there to help him at certain times as needed. Seriously looking at this one as being the right fit for our dad.

It is a delight to work in this facility. Staff is friendly, space is clean, well decorated, safe and warm for residents and families.

Staff always willing to help. Residents are first priority at Azura M.C. space is clean, safe. Great communication.

I walked in cold and was met by [name removed], she stopped and gave me a guided tour for well over an hour. I moved my mother from another memory care within the week. She and her staff ([names removed]) made me feel so welcome and so at peace that my Mom was in the right place.

My mother was a resident at Autumn Leaves GURNEE from April 2019 until her death in June. Her Alzheimer’s had advanced to the point where I could no longer care for her in my home since I was also caring for both my disabled husband and son. I can’t say enough good things about the staff of Autumn Leaves. They understood my circumstances and gave my mother wonderful care while also giving me support. They allowed me to be just my mother’s daughter again which was the greatest gift! I wouldn’t think twice about placing another family member there if the need arose and I highly recommend them to anyone who needs care for a loved one suffering from dementia!
