My mom has been at Azura since Jan 2018 and my family and I are so pleased with everyone's help. We never worry about her or her needs being met. Much care and attention is provided to her and all the other residents. We are definitely "stress-free". A big THANKS!

[Name removed] was the main deciding factor to my choosing Azura Memory Care in Oconomowoc. I visited 8 different facilities including one in Watertown where we live. Azura was my number 7 in my visits. I has called and talked to [name removed] on the phone and made an appointment to visit the facility. I arrived and she was expecting me and met me at the front door. We greeted each other and she told me she would show me the facility and answer any questions for me but first she asked me to come into her office and just have a time to get acquainted. We sat around the table in her office and she asked me to discuss and tell her what was going on in my life that I was looking for a place for my wife. We talked for the better part of an hour and it was very obvious that [name removed] was interested and concerned about the situation with my wife. She took notes and asked me questions as we went along. She was very encouraging and told me if I chose to place [name removed] at this facility she knew her health, my health and our marriage would improve. She told me that was probably hard for me to realize but assured me she would be well taken care of and my health would improve because I wouldn't be under the constant 24/7 care giving. Of course I was crying some as it was difficult to talk about without my emotions getting involved. We talked about the care I was currently giving to [name removed] and the fact that I was becoming very stressed with the routine. The was the first facility I had visited that I sensed a real interest and concern in my wife and my well being. [Name removed] then explained the care that would be given to [name removed] and answered my many questions. Then she said lets go tour the facility which we did. She took her time and explained things as we went. I was very impressed that she was greeting residents as we toured and calling them by name. They were responsive to her and she certainly was not a stranger to them. I was very impressed with the facility and especially the focus on memory care. Most of the other facilities I had visited were combination nursing home, skilled care, assisted living and a section of memory care. I met a number of the staff and they were all very pleasant and friendly. Aster we toured the facility [name removed] invited me back into the office and asked if I had any more questions. I thanked her for taking time to talk with me and answer many questions and show me the facility and explain the care program. I sensed a real concern for the well being of all the residents and genuine concern for them by all the staff. We talked a little more and I was having a hard time holding back the tears as I thanked [name removed] for her time. I told her at that time I was most impressed with her and the operation than any place I had visited yet. She gave me a packet of material and an application if I was interested. When I got up to leave she asked if it was alright to give me a hug which I told her would be welcomed. I toured one more facility after that but I was convinced that when i needed to place my wife this was by far my choice of all the places. [Name removed] told me to give her a call any time if I had any more questions or concerns she could help me with. She asked if she could call me in the future and keep in touch and I told her I would welcome that. I told her [name removed] and I were going to try another bus trip together and [name removed] said she would call me after the trip to see how it went. She was true to her word as she did call and follow up more than once. She assured me I would know when it was time to place my wife in a facility and she would be glad to help me with that decision. When I decided it was time to place [name removed], I filled out the application and called [name removed] to make an appointment to bring in the application and discuss it with her. We went over the application and she was very helpful with my questions and gave me help with some items I needed help with. She told me she did have an opening at that time and asked when I was wanting to make the transition. I told her I wanted to wait till February which was just over 2 weeks away. I told her this was my personal desire because February would be 11 years since she was diagnosed with Alzheimer's and I finally decided I needed some help with her care. She said that would work and we listed all that needed to be completed and said if I had any questions to please call. We communicated more than once during that time and [date removed] we proceeded to get [name removed] all set up in her room. [Name removed] and her staff were there to greet us at the door and make my wide feel at home which I greatly appreciated as she has not been to Azura before that day. This was a very humbling experience for me and the most difficult decision I have had to make in my lifetime. My wide and I have been married 58+ years and it was not easy to place her away from home. [Name removed] was absolutely the deciding factor in my choosing Azura. She gave me complete confidence that even though this was a difficult decision on my part, it was in my and [name removed]'s best interests and assured me she would be well taken care of.

Awesome staff! Wonderful place, kind and loving staff members. Staff would always go beyond caring and give from their hearts. They gave us great comfort and peace of mind, knowing that our loved one was receiving excellent care that he so needed. We were very grateful to be able to be a part of the Azura family.

Well managed, relatively new memory care facility. Very clean and procedures in place so it appears to be operating efficiently. At the same time, great emphasis on personal attention and kind and gentle approach to patients. Patients are at various stages of illness, so this is challenging. The head RN is excellent and an important asset here. However, in addition, I think Azura should add more men to the staff.

I highly recommend Azura Oshkosh! From the moment I stepped foot in Azura Memory Care Oshkosh, I knew I had finally found the right place for my father who had late stage dementia. The entire staff was amazing right from the start. My father's stay was, I would say, the best it could possibly be, right up to his passing. The staff was caring, compassionate, and attentive to both my father, my mother and all his moved ones when they visited. It is a warm and inviting place set up just like a comfortable home with activities created to include each and every level of care needed. The food, the rooms, everything were fantastic!

This community has a very warm and welcoming atmosphere. It is very clear that the staff and leadership have a deep commitment to their residents and show genuine care and compassion every day.

Finding the perfect place for your loved one to walk through the stages and trials of dementia is an arduous task. Looking for caregivers in the home who display knowledge of the disease, unconditional compassion for their residents and fervent passion for the task before them is critical. Desiring a loving home where every resident and their family members feel welcome and are treated with respect and dignity is essential. Hoping for marvelous meals and music, daily devotion and exercise time wrapped in a blanket of safety and comfort is paramount. We were blessed to find all this and more for our mother at Azura.

My mom was at Azura Memory Care in Wausau, WI for 19 months. Our family was very pleased with the care she received. The staff was amazing to our mom and our family. They were very sensitive to her needs, provided a loving and caring home for her the final months of her life. The staff was very knowledgeable and professional. The Azura staff became apart of our family. We are so thankful that our mom was at Azura and the staff will always hold a special place in our hearts. ??????

[Name removed],, letting you know that when I brought [name removed] back today I saw her smile for the first time since going to the ER last Sunday and then the hospital and [removed]. It was so satisfying for me to see that!! [Name removed] has difficulty expressing her feelings via language, but I understand her feelings by her eyes. Azura is "home."

We provide therapy services and home care services in Senior Living Communities. It is a pleasure providing services to some of the residents at this community. The staff are all very friendly and caring to the residents, fellow staff and visitors. It is a lovely community that goes above and beyond to take great care of their residents.

The community is so welcoming, it feels like home. The staff are friendly and so caring. I would recommend this community for my own mom or dad!

I have multiple sclerosis. It has progressed over the years. So, I've lived in 4 different facilities. Some are too expensive. Some are understaffed Some are too big. Some are poorly organized and badly run. Some have good chefs in kitchen. Some order only processed food. Some have young caregivers with dangerously limited experience. Some have highly skilled professionals. BROOKDALE of Appleton Senior Living is all of the best. Fun activities, residents council involved, hair care, bus trips, delicious nutritious food, organized management, skilled staff. I love it here!

Azura got it right! As a guest of Azura from Hometown Hospice and Home Care for the past two years, I'm proud to suggest this community to my friends for their loved one. They have the right approach when caring for our loved ones. This is not just a facility it's a family with a vision.

We are extremely pleased with all the care my mom receives from the staff. Everyone is caring, thoughtful and watchful. We have no stress whenever we think of her care and how lucky we are.

This place is first class in all respects. Love it! And Mom is happy.very reasonably priced.

I placed my mother in Azura based on several things. I dropped by without an appointment so i could see how the staff interacted with the patients. I had been to other places with appointments and felt like I was watching a staged event. When we arrived, the patients were making their own pizzas and bouncing balloons around. Everyone was happy, clean and appeared very content. The rooms were adequate, not fancy, but I learned that should not be how you choose a place to care for your mother. I transferred my mom to Azura and immediately she loved it! She had lost interest in eating and now had her appetite back as her happiness increased. She was treated with love, respect and kindness. They would wake her up in the morning by saying "Good morning Beautiful!" She adored all her caretakers and began to relate to them. I was out of town and she was admitted to the hospital after a fall and she was very frightened. I could not be with her, so her main caretaker came in from home and stayed with her that night so she wouldn't be afraid. I have never seen a more loving place for advanced dementia patients as Azura. The staff was so devoted. They let mom set her own schedule and if she wanted to eat Lucky Charms in the middle of the night, she could. My mother was blessed to live out her last days at Azura.

My mother, [name removed], received excellent care, that is as much as she would allow to be cared for. The staff was always welcoming and was able to give a quick update on how she was. Because the facility is open for residents to move around, I quickly found her because the staff was watching out for her. Mom received excellent care for her physical needs. While [name removed] refused to participate in the many activities, there were many for her to choose from.

Excellent care, very respectful of him. The rest of the family was also made to feel very welcome when we visited.

I only visited my brother two times. The facility was clean the staff has lots of smiles and very friendly. My brother was clean and didn't seem in any kind of stress or unhappiness. My 1st visit with my sister we noticed the smell was very strong from a plug in fragrance. Knowing these are toxic to a dying brain we mentioned it to faculty, the next visit it was gone. Thank you!
