We toured this community when looking for my mother. The staff needs more training on awareness of each resident's ability levels and accommodating that. Offering an environment that recognizes every resident is different. The atmosphere should be more fluid.

A beautiful new place, but they apparently didn't write down we were coming and were not prepared. I did have a confirmation email however. They didn't seem to have things organized, and didn't have anything going on for the memory care residents, and weren't able to answer that there is much organized for them. We were not able to sample any food, but the menu was less than inspiring when I viewed it on the tables in the assisted care area. The receptionist seemed put out that we were there when we first arrived. Not at all welcoming. We didn't see much to sell it to us.

Our tour of the Sylvan Crossing Hunter Ridge made us reakize we would never put our mother there.The place was not clean and was very opressive, I was there with 2 of my sisters and we walked away thinking we couldnt put our mother there.

This community is a newer community and they have already had a high level of turnovers. Things are left out and they are sloppy. I have come in to find dirty diapers on the floor. My mom does not get her baths as often as she should. There have been some growing pains with the kitchen and the food. I have had nothing but issues with this community since she moved in. They are slow to open and slow to fill. It is a nice community as it is about 6 months old.

The facility may look beautiful but the care is not. Our loved one has been there for several months, as the residents level of care increases it seems that care is not provided. It appears if he resident is independent they will be ok but god forbid as your level of care increases the care will not be there. Family must advocate for their loved one. Even though the care plan includes a detailed plan it is not followed. The hourly or two hour checks are not done has per plan of care. Showers \/bathing not given as indicated on plan. We have been very disappointed with the cleanliness of the room and bathroom. Phone calls are returned and plans made but not followed through by all staff.

A bunch of young kids trying to run a nursing home with no compassion no caring about the residence the food was horrible I mean horrible and the care was so bad that my mom had to hire a private nursing company to come out they say they do do their stuff and check make bed checks they don't they just mark off that they do stuff we couldn't get her out of there fast enough and the turnover of that place should stand as a witness that nobody should be putting their parents in there. My sister died of cancer in the facility and they barely checked on her they give you pendants that you push if you fall my mom laid on the floor for over an hour at night because her pendant was inoperable if you have to have your food delivered they only give you the leftover scraps if there's anything left and they don't even care if you got a special a diet that you tell him verbally everything has to go through the doctor in order to get special meals. Her neighbors were moving out as fast as they could and the day we moved my mom out of there there was a lady that said that she was so glad that they finally got her parents out of there they have a high turnover rate there and out we're still waiting to drop some reviews on Yelp and their website as well sorry this this is such a long thing but people need to know there are bad places like this

My moms husband is there. The care is terrible the management dont do their job judt sitting in the office. The caregives walk around on their phones they are lazy dont give showers dress the clients in clothes that arent their own and dont even fit. I would not let a loved one live at this place it should be shut down.

Poor communication, nutrition, treatment. They do not follow doctor ordered plans in physical therapy. They do not tell you when your loved one has been ill ar report it so they can be recieve doctor care in a timely manner. Few speak english so communication barrier. They do not keep them hydrated and take care of some other basic needs. So frustrated my loved one is being treated this way. They need to do much better! Also this place is hard to reach anyone. You get a recording.

Staff members sat at "nurses" station on their phones, most were unfriendly, and non-engaging. There was only 1staff member that was "qualified" to administer meds. Had 1 experience where my family member ran out of pain meds and it was not recognized by staff. My family members room was kept fairly clean but we (the family) swept, mopped cleaned bathroom when we visited. The kitchen, dining room and hallways were dirty. Food was not cleaned from the dining tables or the dining room floor after meals in a reasonable time frame. Food was palatable but not very warm. Daily "snacks" were nonexistent as far as I know. The entrance to the AL area gave a "convincing" portrait of good aesthetics. But the moment u entered the"Dungeon" (what staff called memory care) everything changed for the worse. Administrative staff changed 3 times in the short time our parent was there. It was next to impossible to reach any one of them by phone. Quality of care--all I can say is I never witnessed mistreatment but I could be there for hours and no one would come in the room to check on her. My parent was attacked and injured by another person living there, yet that person was allowed to wander as usual. Activities were nonexistent. Value for the money--absolutely unbelievably Horrible. That place should be ASHAMED!! If there had been another choice I would have moved within the first month but I needed my family member close to me. It is very shameful how the "owners" are padding their pockets and have no remorse for taking advantage. The residents in Memory Care are not afforded the DIGNITY that they rightly deserve I WILL NEVER recommend this AWFUL place to ANYONE!

I was disappointed. It took me a long time to come to a decision only to have made the wrong one for my husband. I think he was more than they could handle. Would have rather been told that before the move in. After two weeks, he was sent to [facility removed], at which point there was little communication regarding their expectations. Then, they scheduled a time for a re-evaluation, but did not show up. I have moved my husband back home, fearing trying any other facility,especially since he is still cognizant enough to know if I am there or not. I would fear my visit (leaving) would cause further agitation.

If you have a loved one and you want them to have top-notch care Terra Bluffs is not the place. This is the first memory care facility opened by terra bluffs and the layout is very confusing. It is definitely not appropriate for memory care. Every time I went to visit they were sitting and sleeping, no activities. The care is very sub par. I saw my loved one detoriate more in the 4 months there then in the last 2 years. High staff turn over, very dirty rooms and an overall lack of caring.

Our loved one has received subpar care since they moved in. Medication errors (that they have not reported to the state of Colorado per regulations), constantly being locked out of their room due to inadequate staff available to care for and keep eyes on the residents, inappropriate meals for those with dementia and eating/chewing problems. The caregivers are WONDERFUL, the management is HORRIBLE. Many, many families are upset, and our concerns seem to fall on deaf ears.

I have lived here for one year and it has been hell The food is awful, staff rude. Nothing gets cleaned ,there are bugs and I have to help everyone. I had to empty a residents dirty diaper pail for her. They constantly screw up my medications. Management is rude,dismissive and gaslight me. I am looking to get out but moving is a big job. Worst year of my life. I absolutely hate it here

This was a premiere facility when owned/operated by City of Eyota. Sold to one management company ($1 million) under contract, who quickly sold it to Cornerstone Mgmt. for $4+ million. Smell a skunk here? Since taking over, they have gone through no less than a dozen directors and nurse coordinators, cooks, and more than I can count NA's. They advertise "End of Life" care... Couldn't be further from the truth. As soon as medical issue arises, they hold a Care Conference, and you are out!!! Refused to give baths beyond 1-2 per week, even when doctors orders stated it. I gave mom a bath in their tub, due to bed sores and bottom issues. The doctor stated issues caused from sitting in a soiled adult garment too long. We threatened to turn them in. And when we did...we got kick back and told "we cannot accommodate her needs". We honestly feel terrible for the staff. Cornerstone Management is evil to the core. ALL ABOUT MONEY, MONEY FOR PROMISES AND NO DELIVERY! STAY AWAY!!!

We removed our loved one from this facility after an awful blonde nurse whose name starts with [name removed] and her sidekick bullied my boy. Things started off badly when the key we were given by the facility didn't work. Instead of apologizing for the mistake, the nurse doubled down on scolding us because she had to get up from her chair to let us in. Another disabled resident discretely indicated he was not happy with staff either, and this nurse especially. My boy turned down some food, so the nurse labelled him "eating-disordered" and wanted him out. Think twice about putting your special people here with these lowlifes.

On [date removed] my family and I moved my Dad to Kruse Village, hoping he would get more care as they promised. I was wrong, from the first day nothing was what I was told. First I was informed that my Dad's room would be ready on [date removed] and he could be moved on that day. My sister and I walked into his room with hands full and saw that the room wasn't ready because the commode was sitting in the shower and no running water. Since hospice had been assigned to my Dad a bed was already set up and his two recliners were already in the room. My sister and I should have taken my Dad back to where he was staying or to his home, that was a mistake leaving him there. So, from [date removed] till [name removed] my Dad was not taken care of properly. Examples, I had asked if Kruse personnel would help my Dad during meal time feeding him. I was informed by employees that they were not allowed to feed the residents, even the hospice nurse assigned to my Dad requested them to do the same, she was told the same. I asked employees that were contracted to Kruse and they told me to contact their boss and they would gladly do it. My Dad was left a lot alone in his room, of which, he slept a lot. On days I would visit many times other residents would walk in his room and look in his closet, etc. I also saw a resident leaving his room as I was walking down the hall to my Dad's room and when I came in they had been in the bathroom using his toilet. Not happy about these things and asked why residents were allowed to walk into any other resident's room and I was told it was okay. I did not agree and looked up in the residents' rights, of which I signed for my Dad, and read that a resident must be invited to another resident's room. I did not pursue the matter any further until one day, in September, I walked into my Dad's room to visit and smelled a foul odor coming down the hall. I saw my Dad laying in his bed asleep, and I thought someone had come in and used his restroom, as I turned to examine the room further, I saw someone had used my Dad's recliner as a toilet and a huge pile of human feces was in his recliner. It took me about a week to realize that it was not going to be good for my Dad to stay in this environment any longer and contacted the hospice nurse assigned to my Dad to get him out of Kruse Village and in two days, [date removed] my Dad was brought home to his residence in Brenham. The day he came home he had the biggest smile on his face and was so grateful I brought him home. My Dad lived 13 more days and died on [date removed]. I will not recommend this facility to anyone, especially the memory care department.
