My father needed some assistance and friends so we choose John Evens in Pekin. It seemed like a perfect fit. My father soon discovered he had terminal cancer and they said they would still take him even if he went on Hospice. He was there let then a month. He fell 5 times in less then 24 hours after a med change. They never called me. When I went to see him his room looked like a murder had occurred. He had fallen against the wall and broke a nightlight. The tines were still in the socket with masking tape to cover it. He was clearly not in the condition to be left alone. Very disappointed in the administration and nursing director.

I didn't know that the residents were allowed to have people living with them that aren't listed on the lease. Isn't that illegal and violation of a lease?? I wouldn't want my family living there knowing that anybody off the streets can come and shack up with the residents at any time for as long as they want.

Do not put your loved one in this facility. Horribly understaffed. My Mother has been here for a few months and many times her meds were mixed up. She was given a shot that was not for her. Luckily she didn't have a fatal reaction. Food is terrible. Dangerously understaffed and the few staff there are underpaid and doing multiple jobs that they're not trained to do. This place is dangerous for your loved one.

This place looks nice. They have nice apartments with room for furniture. Don't let that fool you! Besides a couple caring staff members, it is horrendous. We find bed made with poop in it. Poop left on the floor. Bottom rash not being addressed. Hurt ankle and bruises on my dad's body. One time they did not check on him at all. The next morning he was found sitting in poop from his head to his toes, never getting him ready for bed or checking on him all night. I'm sorry his stomach got so off that it is taking awhile to get him back to normal bowel but really?! At the beginning he got so dehydrated my dad had to go to the hospital because his kidneys were failing. Another time he was taking a shower and they wanted him out before the shift change. Dad purposely put himself on the floor. The nurse call threatening to call the fire department unless he gets up. I told them to give him a couple minutes. He got up. Lack of training and terrible management.

Just from walking through this facility I was not at all impressed and I did not get a good gut feeling at all. We toured during dinner time and the caregivers seemed like they could have cared less about the residents. The grounds at the facility are not kept up, the area where residents are able to go outside looked like the grass had not been cut in 2 months. I think a lot of people are drawn here because it is very inexpensive compared to others, but you get what you pay for.

The people who provided the care, the aids would be about 6 stars out of 10, and the administrative team was horrible. They lied to us, they never assessed our loved ones needs. They never cleaned our loved ones room, the call button did not work for the first two weeks that our loved one was there. It took them a week to brush our loved ones teeth. It was degrading, they only cared about filling a room and nickel and dime families. The only person we liked, was the activities director. She was the sweetest person ever, she should be running the place. I would never recommend this community ever.

We visited Evergreen and found it to be a lovely place, my mother in law would have loved to move in there if there was someone who really handled the business side of this facility. Don't bother to apply if you have a Medicaid waiver and limited income, they won't be calling you back as that was our experience. Very disappointed that we handed over private information that took time and effort to compile and have no idea who now has access or what has happened to it. No calls back from business office or Administrator. Very poor way to run a business that caters to seniors but doesn't really care about the person, just the dollars they may have. As a person who works in the Nursing home field, I would not put my loved one in a place where the names and faces of employees change faster than your or I change our clothes.

We never really did see any issues with the cleanliness of the community. Our loved one did seem to enjoy the meals and dining, and they did offer a good amount of activities. However when our loved one moved in [date removed] they offered to put him on a medicaid wait list for a room. They said they never had a room available for him with the medicaid but we were never made aware of that. They changed management and the new managements excuse was always, "we do not know what the prior management agreed to." We thought within that year he was going to get the medicaid room, but the eventually forced him out and moved him into a long term facility, they gave us no options at all. When signed a 30 day notice in August, they told us to not worry about paying rent, but we had to pay two days into September on top of late fees they charged us.

I would tell people not to go to this community to anyone who asked. My loved ones had a not great experience. We thought this would be a safe environment in their hometown. The staff was less than helpful to our loved ones. My loved one used a walker, and we requested that activities would be incorporated for assistance with those who are not as independent. The food was terrible. The community said they would have snacks and coffee all the time. The snacks that were out, some residents would horde the items and there would be no snacks left for other residents. Due to one of my loved one's declining health I requested the food be delivered to my loved one's room. The food was cold and seemed to be leftovers. During the grief process we decided to take my loved one home to assist with this process. We had plans of bringing them back, and the community decided to give an eviction notice on our loved one's TV stand. The administrator was not sympathetic towards us during the dying process of our loved one. He seemed like he didn't care about the families or residents.

My parents reside in Gull Creek. The kitchen and nursing are understaffed. The meals are inedible at times. Most other times the portions are so small that the residents are still left hungry. Meals are not well balanced with little or no vegetables given. You fill out a meal plan for the week and still don’t get what you asked for. Hot dogs and hamburgers are sometimes the only thing offered for both lunch and dinner. Meals are so bad, l as the son, have to keep their appt stocked with food otherwise my parents would starve I’m sure. We’ve complained but Gull Creek doesn’t care. It’s all about money to them. Nursing is almost as bad. They often bring up the medications at wrong times or forget to bring them up at all. $6000 a month for such terrible care

My mother was diagnosed with Alzheimer's several years ago so we decided to sell her house and find Assisted Living, I hired attorney [name removed] to help with Medicaid, he recommended that I hire a company called [removed] and they introduced us to Carriage Court in Grove City, we were taken for a tour and we thought we had found the perfect place for my mother. We met with the Administer and explained Mom's situation with Medicaid, she had around $80,000 in the bank so we were in a "spend-down" which mostly went to Carriage Court for monthly rent. Once we had spent all of Mom's money on rent we we're relying on her SSI payment to get her by until Medicaid kicked in, her SSI payment doesn't get to her account until the [dates removed] of the month but Carriage Court kept sending late notices, I explained to them several times about the SSI payment but the late notices continued. My attorney kept contact with Medicaid and Carriage Court throughout the situation and they were able to file a motion of "Act 52" to get the arrearages paid up by Medicaid. While on a visit today to see my mother and take her a few items I was approached by the administrator [name removed] who asked me what I was going to do about the arrearages and that he was planning on moving forward with evicting my mom, I calmly explained the "Act 52" to him and that it would be paid up, he said he was moving forward with the eviction. When I arrived to start moving mom I was met by the administer [name removed] and two police officers who were there because of the way I "got a bit angry" while in the office of the administer. After explaining the situation to the officers they were shocked and left after about 20 minutes. I wasn't prepared to move everything so I asked the administrator. [Name removed] if I could come back and pick some things up later and he told me if I didn't have everything out that he would put what was left on the porch of Carriage Court. One of the maintenance men/bus drivers from Carriage Court saw what was happening and offered to help, he got the Carriage Court bus and helped me load my mom's things and deliver them, if it wasn't for [name removed] helping me I don't know what I would have done that day. Thank God that my mom's best friend was the owner of a nursing home and her children run it these days, it only took one phone call and my mom is now in a place that she knows and people love and care for her.

We moved mom in on Jan 24, 2019. Someone stole her new clothes we brought for her on monday the 4th and friday the 8th then over the weekend that we did not show up which was Feb 9th and 10th when we come to visit her on monday the 11th, most of her new clothes were replaced with old clothes and only 5 to 6 pieces of her new clothes were left. That was less then 2 weeks that we moved her in and We were very disappointed. They are looking into this situation at this time. We were happy with the service they provided for mom until this happened. We are fair about the whole situation and hope this can be resolved soon.
