This place is awesome. I'm not positive the smaller environment will be for dad, but I'm taking him there to look around for himself.

A very lovely small facility. The rooms were great with large closets. I like the smallness of the facility which allows more intimate knowledge of each resident. I have only two negatives: first the lighting is not as bright as I would have liked since my mother has macular degeneration and needs bright lighting; also, residents are checked on in their rooms ever hour. I'm not sure I understand the need for checking so often in an assisted living facility (nursing home, yes) and I know my mother would really dislike such frequent checks.

Overall we're very pleased with how things have gone at Autumn Oaks. It's a very nice looking facility, they do a great job keeping everything clean. Overall the staff are very friendly and helpful, and I like that since it is a smaller community the staff and residents all get to know one another. They've got all sorts of activities going on to keep the residents active and engaged. One area I'd like to see them improve is their food service. It started off very well, but they got a new chef since she moved in that seems to be much less willing to accommodate specific dietary needs. I'd also like to see them offer a bit more in terms of individual transportation for doctors visits.

Good ratio of caretakers--3 + 1 Mgr for 16 people. End of life facility. Specializes in Memory care. Assists with feeding and prepares soft/puree foods. Private rooms.

They have had a lot of turnover. The community is currently without an administrator and there were two previous in this positions. The activities director is excellent, and I think she should be running the community. Her name is [name removed]. It is the small things like the door bell not working...this might be due to the pandemic. The community is very clean.

It's an attractive place. The building, the grounds.

This community has a new director and things are starting to turn around, and they are making improvements. For the most part the staff members are good, however for the level of care our loved one needs, she does not always receive that care. It is not always consistent. There is just not a lot of care takers for the amount of residents they had. The director is a nurse and the nurse practitioner are very good, but are certainly spread thin. The communication could use come improvement as well. At the moment they are offering bingo on Friday afternoons which our loved one really gets excited about. The main chef at this community is wonderful, he is very accommodating to all of the residents. The overall cleanliness of this community could also use some improvements.

We toured quite a few communities and pretty much they all offered the same things but there was some difference that made us choose this one over others. We really liked the floor plan of this community the best it is setup where my mom can wander as much as she wants and not go anywhere. The community is very clean and well kept. The staff is nice and attentive to the patients they don't have too many assigned to them so they are able to give the attention that is needed to the residents. The food is good here we have eaten here a few times. The price might be a little more expensive than others in the area but for the piece of mind I have, I don't mind.

New Haven has a wonderful activities director, who is very engaging. I have learned from comparison to other homes how important this is for the residents. They have finally addressed the food being served problems. I have found the present staff to be very caring and good with the residents. The decor is beautiful and very comfortable for residents and visitors. Overall New Haven is off to a great start being new.

Residents and family members desire to have the serious issue with the food being served to be addressed. Residents are not served what the menu always states and what they are served is of poor quality with little or no meat. My family member has left the table numerous times without eating due to the food. Whoever cooked the Christmas Family Meal did an excellent job. It was delicious!! Please address this on going concern. It has gone on far to long! Otherwise the staff is great and the facility beautiful.

When Mom left Encompass rehab I was glad to find New Haven in Tomball. It was a smaller facility and they offered Hospice in her assisted living if needed. The staff was friendly and the facility clean and welcoming. Her room was small but adequate and hospice provided a hospital bed. But Mom stayed bedridden and never used the bathroom facilities or was able to eat a meal there so can't comment on the food. The staff was cordial and helpful when called. I hired caretakers to be with her round the clock because she couldn't do anything for herself and probably should have gone to skilled nursing. The biggest complaint I had was the lack of staff especially at night. We couldn't get Mom her meds and she was in a lot of pain. There was slow response to help and they had to call in someone in the middle of the night to come in and help administer her meds. She was in the wrong place to get the help we needed as she passed away shortly after checking in there. Hospice had to be repeatedly called and slow to respond. There was only one person in the main office and she was not always available. The nights were difficult for us with the lack of help. New Haven would be good for someone that was able to care for themselves with some assistance. Mom was beyond that point. Thank you for what you do to help families needing help during this very difficult time.

Ask for some things about Meds inquiry and then having to wait to for answer to your questions at their earliest convenience. Some staff members are just and others wait on you at their earliest convenience.

The staff is great, but they are terribly understaffed. The food is ok, but they don't cater to people who have to have special diets. I happen to be gluten free so it's hard to find something to eat

Management is not easy to meet with. Manager doesn’t want to be bothered. She even shared she has to much to do between administration and other duties. I witnessed a specific employee being unkind to residents in Memory Care Unit where residents need more assistance. I was even told no color/flowers etc were needed in lawn area as they “don’t recognize anything” anyway. Food that is served to memory care residents is not warm. Times I have been there food is cold. Everyone deserves a warm meal to eat. Many residents cannot ask for water. I have observed only once when water was offered. It is unfortunate that this is the situation and attitude of the manager and some of the staff. No matter who you are and what your current mental/physical state is everyone deserves respect and care.

Well it's hard I know to keep good workers and it's a tough job! I believe language is a a barrier...one some workers wear mask two they speak a different language......

There is a lot of isolation. There is some independence required to be here.

I toured this community and was not impressed. It wasn't nearly as clean as some of the others we saw and the rooms weren't user friendly. The staff was ok and our tour guide was very knowledgeable. But I just didn't get a great feeling about it.

With a community I feel like you get what you pay for with my mother I felt that there was just different care needed. Nothing negative about the community at all just not the right fit for our loved one.

The rooms are small bedrooms with a bathroom attached. The whole facility itself is nice, and there are about 16 rooms per building.
