I would tell others to make more than 1 visit, come during a meal time. Called late evening and no one to answer phone. Some staff said short handed, management said they were fully staffed. Just something a little off putting to us. Couldn't put our finger on it. Don't think it is bad just didn't work for us.

The community had really good activities for residence. The staff is very good and so is management But when we left some of her items were missing. Rooms sizes were good she was in a double room. The food was good when I had a chance to eat with her. I would recommend this community.

The community itself, the decor and furniture and set-up, is all very nice. The food was not too great at first, but they did improve. My loved one was in memory care, but their staffing was always way too light and the residents there needed more supervision.

I suppose that Orchard Park is rated a 5 star if your parents are able to have assistance. Memory Care side, have all stages of Alzheimer's. My frail parent has had 2 room mates that has abused her. She is not a full blown Alzheimer patient, but has room mates that are. One, use to steal her stuff and the other one has broken my Mom arm and has pitched feces onto my mom's bed. I picked my Mom up at 9:30 a.m. and her room was a mess with feces and clothes all over and the smell was awful - when I returned Mom back at 3:30 p.m. - the room had not been touched, still smelling and a mess. This is not a nursing home, but to get into a Nursing home by Obama Care you have to have a broken hip or back that way Medicare doesn't have to pay for it long. So with a broken arm you have to learn yourself to eat with your left hand or go hungry. Even paying out lots of money - still have to be there to feed and dress her. I would take her home with me but I don't have Power of Attorney - just doing the best I can. Don't know what Captcha means. Not in my dictionary.

My mother in law is no longer at The Lodge. It did not work out for her. There was inadequate staffing for her needs. It is fine for people who are independent though.

The facility is beautiful but staff rude and dismissive to my parents. Food was ok, but they regularly ran out of even basics. The care is very substandard. When my mom was vomiting and unable to take her medication, no one followed up on how she was doing or what they could do to help. They just left us a phone message that “Your mom refused her medication.” A very upsetting and demoralizing experience for my parents. They moved after less than two months. No one at The Lodge asked why.

We moved my parent into The Lodge at Glen Cove and were promised services that are not delivered. They are severely understaffed and the turnover rate is high among employees. The Dining Room continually does not have the food in the kitchen that is listed on the menu. There are problems with activities listed that are cancelled. The maintenance has been a continual problem and again there is no one to fix no hot water in the kitchen. The transportation as promised to appointments does not happen. I have to grocery shop to augment the food that they are not receiving. What happened to the promises that have Not materialized!!!

My mother has been at cornerstone for sometime & has gotten to know each of the caregivers on a deeper level. These caregivers are UNDERPAID for all the work they put into this company which would explain the mediocre care they provide to their residents. I hope they one day get the pay they deserve it explains why they can never keep staff.

I moved my mom to Silver Oaks to have her closer to me, expecting the same high-quality care she had previously received for four years. However, my experience has been deeply disappointing. During her stay, she suffered multiple serious UTIs, had recurring unexplained bruises, contracted COVID-19 twice, and experienced several bouts of the flu, all of which contributed to a decline in her health. These issues led to complaints from the staff about her behavior. Tragically, a severe UTI hospitalized her, filled with extensive bacteria, which doctors attributed to poor hygiene practices at Silver Oaks. She never returned to the facility.

Since the trinity didn't test [name removed] when he had symptoms of covid he wasn't given any medication to treat it. They eventually did test him but not till 2 weeks later. By then the results were negative. So with getting no care/ medication for covid [name removed] passed away. Prior to leaving the Trinity I took [name removed] to the doctor they did blood work and his sodium levels were so low because of the covid that they had to give him sodium pills. He also had pneumonia at that time. He continued to have respiratory issues. But because of the lack of treatment at the time he had covid he was not able to recover fully and passed away. This place needs to lose their license. On one of my visits his roommate was in the TV room crawling around on the floor with just a diaper on. The staff was standing 20 ft away doing nothing about it. I was there for about 45 minutes and they never did attempt to put clothes on him. Like I already said this place needs to be shut down. It's absolutely terrible their lack of care. According to [name removed] one of the male caregivers was handling him roughly and [name removed] threatened to knock his block off. [name removed] then said the man told him if he did he would hit him back. This is no way to treat seniors with dementia. Well that's about it.

My mother-in-law enjoyed Lake Park for many years until the recent purchase of it by Pacifica. After the purchase, the internal communication and quality of services declined rapidly, including decreased security (random people entering the building and apartments), and regularly running out of food, which is so awful! My MIL decided to leave, as have other long-term residents. Now, Pacifica refuses to return these residents' deposits in a flagrant breach of contract and won't respond to any inquiries. Out of necessity, we now have to explore legal avenues to get back the money my 85-year old MIL is owed and needs for living expenses. It's unclear what the new owners do care about, but it's obviously not the seniors they are supposed to be serving. It is a tragic and deplorable situation.

The facility is new and the staff in the Memory Care is incompetent and has no idea how to deal with difficulty. My husband was there for one week before he unexpectedly died. We were promised over and over that the staff would be able to keep him busy and redirect him if he became difficult and wanted to leave. This was a complete lie; they had no idea what they were doing and each shift would call us to either speak to him on the phone or come down there and speak to him/them. Although I filled out a ton of paperwork explaining who he was and his likes and dislikes, no one read this material and they knew absolutely nothing about him except for he wanted to go home. One evening they lost him (he had wandered into another room and was asleep) even though this is literally one half of a floor and each exit has alarms on the doors. The staff called us in a panic and said that they had left one person in charge on the floor and the other two assistants were out in the parking lot looking for him. It was a horrendous experience, thankfully we only had to deal with this for a week. He was taken from the facility on the 6th day after having suffered a seizure and NO ONE from that facility ever contacted us to express their sympathy

My loved one lived here when it was brand new so we didn't have any reviews to go by. Whenever I visit they are understaffed and ask me to help them. My loved one is never bathed and always smells like urine her hair is always dirty. They said they would have a salon and nail services but never has had them. The staff don't know the residents names and calls them by their room numbers only. Very impersonal. The staff is often sleeping on the job when I come to visit. My loved one got an infection from their lack of cleanliness/cleaning her and they never even told us we just happened to find out weeks later from the doctor they called in. Very bad place please don't send your loved one here!!!! It's abuse especially since they cannot tell you what is going on! And yes they have had many outbreaks of covid as well.

This facility seems to provide you with a really con job in making you believe that they care for their residents and have the best care for them. The dining room is really nice and they do have great food, Memory Care, on the other hand, is like putting your loved one under the bus. There are no activities that you can observe, staff is not always with them although if you need someone, you can find at least two or three in the medication room laughing and talking. Food is horrible and it looks like it is left overs from the food in the main dining room. I would not recommend this facility as a caring and loving place for your loved one
