The room was clean. Food was great. Staff was poor - Paid for help for additional help needed. $[fee removed] or stage 4 help over what room charge. Only got help for wheelchair transport to meals and shower and when help button passed for bathroom help it can take up to 30 minutes. Then they will set on couch and watch him walk. What if he did lose balance!!! Never was a gated belt put on. Was given daily meds. He had cornea transplant in both eyes and needed eye drops daily for anti rejection. I asked Daily why he was not getting them. Answer was " I'LL CHECK. " FINALLY I ASKED for a meeting. Was told that computer said drops NOT AVAILABLE. It was in the Refrigerator and was at Astoria the day of admitted but had never been put on drug cart for use. I decided that was it! Brought home home.

I wish I didn't have to put my wife in a memory care facility, but this is the best I could find.

Staff seems good. Although there's a few that don't seem to do much. And some that do quite a bit. They seem to have enough staff if everybody was hustling, but a few of them don't ever hustle. So I would give their staff 3 also. Management I would give a 4 to. [name removed]has addressed my concerns when I've raised them and [name removed]the head of the memory care unit has also addressed a few issues. The nurse manager was fired a few weeks after [name removed] moved in because she never put in an order for her medication. They used up the medication I took in when she moved in and then she ran out of a couple meds and the nurse never contacted the doctor or the pharmacy the cleanliness of the units are good. The size of the unit is nice.

Poor quality of care.

They just have too many staffing changes. New people that are not qualified and do not like their job. There are a several high quality caring staff but the lesser bring the community down as a whole.

Disappointed and Sad!

I was so excited when I learned my father was going to be placed here. Now I am crying as I write this. Although we are paying for services I haven’t seen one person check on him. It’s been three hours. The place is a ghost town after 5pm. When I went to ask for help, the care giver was on her cell phone and didn’t acknowledge me. She then stated she’s the only one on the floor and it’s not even her floor. It breaks my heart to think that if my father falls (and he has) that he’s on the floor until someone finds him. Pretty much a staff member confirmed that’s the case. Where is the dignity in that? Where is the service for the money we’re paying? Where’s the care? I thought this place would be different, but I see that it’s not. At least not for the Memory Care Floor.

Actual cost is not given, They give you the base price, then you receive the bill!

Before you decide to move here ask for additional cost. You can quickly go from the base price to twice or even more. Seems the "assisted" part will cost you more! They refuse to tell you what they are charging you for when they double or even more each month. To be able to live here and to keep the base price you to have no help of any kind. And the will bill you monthly for giving you your required prescriptions. [amount removed]a month even if it one or two prescriptions, and even it over the counter! That is not counting the cost of your drugs they obtain from a local company that supplies your presciptions. Also, if you are ill, or having loose bowels, etc., you have to go down to the dining room or be charged [fee removed] for each meal they bring to your room. In other words you can be on your " death bed" and you are suppose to Get out of you get and go down to the dining room! All in all they will find ways with excuses to increase your bill. Ask them to itemize their charges. Good luck!

A Trial and Error

I placed my husband here at Country House in Elkhorn on Jan 10, 2022. Its very new and well kept and the staff was very kind. But the cares that were promised did not happen from the beginning. He has edema in his legs , therefore leg wraps needed to be put on every day and edema pumps were to be on his legs for an hour a day. This did not happen for at least two weeks. He was to be cued to go to the restroom and I never saw that occur (and I was there most days for long periods of time.) nor did any of his visitors witness this. Joe did like it okay , always wanted to go home, but I think that was a normal reaction. The most compelling reason for our decision to find another facility was the significant increase in cost over a one month period. So we gave notice on Feb 11, 2022 and moved him as soon as I could research and visit other facilities and make a decision. And also I reached out to AGEMARK, the corporation that owns CH , never a call back. That’s inexcusable!!!

It's all really a phony façade. It looks and feels like a luxury hotel yet it is only the outside appearance. We spent three months at Sage and never once did I feel that my father was being taken care of properly at a [Amount Removed]/mo price tag. They were constantly understaffed and poor/slow to respond to any questions or concerns. The executive director position changed three times in the three months we were there and other management positions changed out as well. It was difficult to get my father's room cleaned at minimum once a week... in fact, it was three weeks that it took when we first moved in. I ended up having to hire a caretaker to stay with him, and to assist him with his medications, his shower, and care of his living space. When getting the initial tour, and even afterwards, all the wonderful descriptive words were used to convince me, and I'm sure others that they cared. I'll leave it at that for now since just writing this summary has made me quite sad. After moving my dad to another facility, he passed a month later. It is quite hard to remember those last few months. Traumatic better describes it.

They remodeled (cheaply I might add) to try to distract potential residents into thinking it’s a clean and modern facility to accommodate future residents. They are NEVER adequately staffed. Once you’re in, all niceties go out the window, with the exception of a few employees. The caregivers they hire is anyone and everyone to have bodies in those positions. You press a call button, it can take up 1 hour for them to respond. They doctor their records in order to be in compliance with the state. Their only focus is money!! I advise against this facility VERY STRONGLY!!!!

Dining room is open from 7:30 to 7:00 but it seems chef has never opened a cookbook and doesn't care. My 1-br costs <fee removed>. $150 worth of gift cards disappeared from my room. There are no black residents so they just hired a second director of sales who is black in hopes of solving that problem. I estimate there are only About 50 residents and half are in Memory Care. 4 floors, 27 rooms per floor. Food was just as bad as [community removed] where staff was friendly but laundry was stolen. Management didn't care. Older, uglier building, smaller rooms, some without overhead lighting fixtures, one resident didn't even have a fridge in her studio. I'm 81 and the thought of moving again is out of the question. Never thought I'd die hating where I live. They all just want your money.

Please do not put your loved one in their care

This place charges a lot and half the time, the resident doesn’t get the care they pay for. Recently moved our loved one out to a new facility (and cost less) and he is so much happier. No problems at all like he was having at Country House in Granite Bay. No more constant phone calls of incidents and the new facility knows how to work with people with dementia and Alzheimer’s unlike Country House. So thankful we were able to remove him from this toxic place that says they care but it’s really all about the $$$

Rude and uncaring

There is a login system for guests to register at. If you are a first time visitor and don't know the system, they roll their eyes at you and say just enter your phone number and answer the questions. They definitely clean because all you can smell is the odor of cleaner Workers are usually bunched together and hard to find if a resident needs help Nurses don't know what is going on with patients. I texted my sister in law that my mom in law had a rash. We showed the nurse and she took pictures to send the doctor. Apparently it was a few weeks going on yet the nurse knew nothing about it. Activities person gets frustrated when telling residents how to play a coloring game and walks out when no one knows what to do. My daughter and I both had no idea what she was trying to say so how would challenged residents?

When Astoria purchased Sunrise, they did not disclose entire information, lost initial paperwork, and did not communicate changes. My mom's room was always neat and picked up. Not prestigious. There was always dust. The staff that I encountered was always friendly. Sometimes we're notified and sometimes not, about an incident. Hospice does most of the work with my mom. They are the one's that keep us up to date. We very seldom get any news from Astoria. The staff has changed significantly. I only know 1 person that was originally there from Sunrise. No one seems to know what belongs to who. It took 5 weeks to locate my mom's new glasses. She's not able to do a lot of activities. With Sunrise, she was never left alone. They said whatever communication she could ingest would keep her brain healthy. Now when I visit, she is always alone in her room. Sunrise never left anyone alone in the room. They had them out at least listening. I feel she is set in her room to waste away. She's there because I can't be with her 24/7. Astoria obviously doesn't care about memory care. What she is paying (18,000 a month) and with hospice doing most of the work and now they've said she can't stay there on Medicaid, once again I have to move her. Sunrise said that she could die there. Astoria should not be recommended to anyone. They are not truthful.

ude staff, do not recommend, not a good place for a loved one

Their staff is very argumentative, unhelpful, rude and will sue you if you have any issue. Would not recommend sending my dog to this facility let alone a family member.
