Overall, a good experience; Inconsistent caregiving

I give Cranberry Park Assisted Living points for its atmosphere and setting. It has welcoming floor plan and furnishings. Breaking things up into neighborhoods is also a smart idea. The activities director goes above and beyond- creativity in offerings. I can't say more about them as my mother does not typically participate. She does love the music in the evening's once a week. The level of caregiving is inconsistent. Some staff are very aware and check in on Mom. Often. Others do not. It really has no rhyme or reason in my opinion. The wellness team is responsive, however there's not always follow-up. Overall, it's a great place for Mom. The negative remarks have more to do with my high expectations. I believe staffing challenges are everywhere you go. And it's something I'm trying to learn to live with.

I'm happy with it. They are short staffed at times and that brings bad caretakers. For the most part it is safe and everyone is friendly. My mom does not like the food, there are not many activities she likes to attend. For the amount of money we pay, which is less than others, we do not get a lot.

Turned into SNF no Assisted Living. I do not recommend.

After my mom moved in to assisted living they decided to make the entire place a SNF. She will not be allowed to have her coffee pot or microwave and she will not be allowed to self administer her own medications.. I have serious concerns about loss of independence for these residents. There is not enough staff, rooms are not cleaned very often, trash rarely taken out and let's not even begin to talk about the food and the terrible food service. Looking for a new place. Mom actually is advocating for other residents who cannot advocate for themselves. I can say that the nurses are great, the aides as a whole not so much. Some are great others don't speak, are not friendly and I have seen them walk by an active call light.

Could be better.

Support staff was at times slow to respond to calls but kind and effective when responding. My wife particularly appreciated [name removed]'s assistance in all respects. My wife was there 5 weeks and made satisfactory progress with physical therapy help. Occupational therapy staff was helpful. Very clean and comfortable room but television usage was overly complicated for a healthy adult and impossible for my wife with Parkinson's.

I am writing this review because I strongly feel it The Grands of Dublin has shown a great deal of lack of responsibility and not taking any interest of an issue that occurred while my father was a patient/resident there. On Sunday, [Removed], I picked my father up from room 324 at The Grands and drove him to visit my mother who was a patient at another rehabilitation facility. After the visit, I drove my father back to The Grands. I made sure he was back in his room and noticed his dinner was ready and waiting on his tray/table. On the following day [Removed], I picked my father up for an appointment at The James Cancer hospital. It was then when my father let me know his electronic dentures (talking dentures) were missing. He can’t wear them while eating and has to use his regular dentures. He explained to me that he took the talking dentures out and placed them in his case before eating and laid it on his table. He then fell asleep after eating and when he woke up later, he noticed they were missing. He told his nurse early that morning that they were missing. After returning from his appointment I looked everywhere in his room for the dentures with no luck. The only thing that we strongly feel that happened, is that the dentures with case were removed and thrown away by whoever collected his food tray. The staff available then had searched the laundry and other parts of this facility and no luck. I cannot say enough about how well my father was treated during his stay at The Grands. The staff was always on top of his medication and treatments, taking great care of him and making sure his stay was comfortable and clean. I am writing this because after talking to other staff members, I had no results. On my father’s discharge date, [Removed], I talked to the director about this situation who told me that he would turn this in to corporate. Since then I have left three messages with the director and not yet heard back from him. My first message was on [Removed], and then on [Removed]. I tried calling corporate myself and all the offered prompts do not have any options to leave a voicemail, so I’m guessing you have to get lucky for someone to answer or go in person, which is hard to do while working. These dentures are very important to my father. This is his only way of communicating if he had to call 911 or talk to anyone in an emergency situation. Whatever the outcome of this situation is, I feel my messages should be recognized and returned. Just seems strange that someone is not wanting to take responsibility for this and doing a great job avoiding whatever consequences that could follow. Respectfully [Removed]

Current Management is Poor at Best

They really try and sell you on hoe wonderful they are and everything will be handled. Not the case. Many families pull meds from the pharmacy so they can track orders and ensure their family member is receiving them properly as the LPN does not take a proactive stance. The current LPN likes to play the blame game with her employees and never takes responsibility, she's also is very unprofessional towards family members. It's unfortunate many of the caregivers the families like do not stick around due to the unprofessionalism exhibited by the LPN. Giving the excuse "it happens at other facilities" then saying "I know that not an excuse." Is unacceptable behavior. I highly recommend video camera placement in any facility. It helps when making complaints with the state.

Take your loved ones elsewhere

First impression is good. Beautiful place. Oddly presented with fake stores or ice cream shop that is only open a couple of hours a week. When you look at patient care, the facade falls apart. Too few staff, nurses not sufficiently educated or trained in patient care, terrible food. PT seemed the one bright spot. I do NOT recommend this place for any type of treatment. Many of the staff were rude. A couple were really nice but completely ineffective. Social worker on staff was rude and condescending. No one seemed to be concerned about getting my mother better. Take your loved ones elsewhere.

Beautiful in Concept BUT Lacking in Practices

I just left the rehab facilities at The Grand and feel I need to share this with anyone contemplating using The Grand for the same services. Things to be made aware of, 1) IF you enter the Grand on a Friday afternoon your rehab may not begin until Monday or later, they may EVALUATE you over the weekend in Occupational Therapy (OT) and Physical Therapy (PT) and call it rehab but physical and occupational rehab in the real sense did not actually happen until Tuesday after I was admitted on Friday. 2) Make SURE you or your loved one is fully assessed immediately upon entering The Grand by their transitional team. By that I mean sit down with the Trasitional Care Coordinator, The Director of Nursing, and/or the Social worker. I believe after my experience they will really try to make this right and will work hard to not have your loved one fall through the cracks as I did. The number one priority being there was to receive in-patient rehabilitation services for my replaced hip making “staying ahead of my pain” my goal. Make SURE that you or your loved one is immediately placed on a pain medication schedule that ensures that will happen. Do NOT allow anyone to just say, “just tell us when you need it.” You are in no condition two days after major surgery to do that even if you think you are. There needs to be a 4-6 hour regiment that continues 24/7 that ensures that you will be prepared for your pain, treated, and then physically prepared for therapy. It does you OR the therapist zero good when you are receiving your pain meds, as I was, 10 minutes before receiving therapy. Also, while being put in the ICE room to ice down my hip after therapy feels good, I consider that not part of my therapy and would much rather my limited time with my therapist be spent strengthening my hip than me sitting alone icing it. 3) You have a call button for a reason. When you push it...time the response, if it goes beyond 5 minutes for the aide OR the nurse dial 800 (if you can reach the phone) and have them page the nurse to your room. If you STILL have a problem getting someone (like maybe a nurse that does not respond for 1 hour and 24 minutes) either leave the next day as I did OR dial 800 in the morning and ask for The Administratior who I believe absolutely wants/needs to know what is happening in the facility. 4) The food....improved once the director of foods spoke with me and found out that mine was coming stone cold causing my husband to pick up food and bring in several days in a row. 5) If your room does not get swept or trash emptied each day during their housecleaning, call 800 and request it. Then finally, while I was not there for the nursing care as much as I was for the therapeutic care, the nurses are who you see the most frequent while you are there. Beyond your pain level which they do ask, Make SURE that they check your incision EVERY day and may have to rebandage it, and that your vitals are taken a least once every day. For whatever reason I carried a low grade fever the first several days I was there and that needed to be monitored. I WILL say that one of The Grand’s most redeeming features are that many of their nurses are very caring. I am an active 63 year old female that just wanted a head start on therapy and wanted to be close to my daughters’ homes in Dublin. While I, like everyone who has reviewed The Grand found this place to be beautiful and the concept noteworthy. The actual PRACTICES were found well below expectations. What concerns me most is I KNOW that I wasn’t the only one receiving this same care but many older people are hesitant to give a review for various reasons.

Not a good experience!

Mom stayed here first before we had to get her out of here. They were not properly staffed with the people to take care of my moms needs. They just did not care about the residents and the care they needed. This was the worst experience that we have had.

A life-long regret

More emphasis was placed on building a pretty facility, though most not used, and not enough attention to caring for patients. My mom, who was there to recover from a serious fall that resulted in a compression fracture, did not get info communicated correctly. I let them know she was seen at Dublin Hospital a few times and that's where her records are. I would share. She was taken to Mt Carmel East and they didn't give her shots to avoid blood clots because Mt Carmel didn't say anything. Mt. Carmel DID give shots. They ASSUMED many things and after two weeks I discovered after getting my mother to the hospital nonresponsive, that she was having a heart attack over two or three days. A staff of RNs, a NP, and LPNS and 24 hour care supposedly and not one of them noticed anything. They frequently said, "I wasn't on duty" and never took responsibility for anything. My mom had a heart attack and died November 2018 at Dublin Hospital. I would NEVER let a loved one go to this place. I was hesitant about her going there but the hospital said I had to transfer her or private pay the hospital bill. I will have to live with this regret and it's hard.

Terrible

I have many many complaints. The phone does not get answered. I have to physically drive over to speak with anyone!! I'm not happy. They gave my papaw for dinner carrot soup 1/2 cup. And ONE slice of lunch meat on bread. Smh. I came there and it was absolutely disgusting.

No bed alarms??

Why in a rehab which includes many people who are there for or have a history of falling, would you not have any bed alarms whatsoever. My 94 yr old grandma was found on the floor 3x in one week in the morning. No staff sure of how long she has been there. My grandma was not confused and said one night it was hours. To top it off she was fine one day that I was visiting her, breathing/lungs sounding clear (I'm a nurse and notice these things), then 6 days later of myself not visiting, DIES of pneumonia, suspected aspiration pneumonia, [date removed]. MD stated the x ray looked terrible and not like new onset which means they must've noticed her sounding bad in the days prior and failed to do anything. In fact my mom who was there everyday pushed for an x ray the day she passed and their doctor claimed it "looked fine". Hospital MD was stunned he said that. She became septic and died 3 hours later.

Incompetent medical care - Don’t take your loved one here

This is a review of the “skilled nursing care.” My wife arrived at The Grand on a Friday afternoon, 10 days following back surgery. It took more than a day for them to get all her prescription meds in. There were no physical therapy or wound care nurse services on the weekend. Nursing was understaffed and not well trained. A doctor did visits in the mornings, usually before the facility was open to visitors so family miss these visits unless you get in early. I tried to communicate with staff about my concerns regarding my wife including confusion and weakness. The doctor did not make rounds at all on Monday so I was determined to speak with him on Tuesday. My wife passed away during the night, about three and a half days after arriving at The Grand and more than 40 hours since their doctor last saw her. In my opinion The Grand’s incompetent medical care was responsible for her death. I would never ever recommend this facility to anyone needing skilled nursing care for a loved one.

Disappointing

My husband was here after spinal decompression surgery. The staff was very nice but did not follow through with recommendations from the doctor. Medications were not given per doctor's request. At 2 weeks from surgery he was sent via wheelchair to doctor's office to have stitches removed. He had not sat upright for any length of time.He was dehydrated and had a very low blood pressure so he was admitted to the ER for possible sepsis which he didn't have.. When he was sent ho.e I was told he needed a lift and hospital bed which I declined. He arrived home via ambulance and was incoherent. He was again dehydrated and blood pressure was low. After 3 days at home, his blood pressure stabilized. He is able to transfer himself with little help from bed or chair to wheelchair. He is much stronger, oriented to himself and location. He is getting home PT and OT. Both women who evaluated him could not understand why he did not receive better therapy.

The Good, The Bad and the very much UGLY!

The Good, The Bad and the very much UGLY. I am about to post a very fair review of THE GRAND'S "SKILLED NURSING FACILITY" in Dublin. My wife suffers from Parkinson Disease and as a result of a fall needed a Skilled Nursing Facility to recover in 2019. I was given a Medicare. Gov. listing of Skilled Nursing Facilities in May 2019 to select a location. At that point The Grand was very highly rated. I will only discuss one category "Staffing" and it was "Above Average" for me to have selected that location. The current rating for "Staffing" at The Grand published by Medicare.Gov. is "Much Below Average". My wife was there for 7 months until [date removed], when she suffered a seizure and arrived at Riverside Methodist Hospital with a temperature of 102 degrees and PNEUMONIA in both lungs. Most of the Nursing Staff who I have met scheduled for the evening shift are undertrained at best. In seven month, The Grand, can only now be described as an "UNSKILLED NURSING FACILITY".

Need more staff and better training

Family member did not improve and responsive rate was very slow. Was frustrating for us to try to get people to listen and more importantly to care about our requests and reason for being there. The stay did not accomplish what we needed and we disappointed with the level of expertise and compassion there.

I scheduled my tour on line. They had no idea we were coming. They charge extra for many things. Eventually will need memory care. Was not even offered to be shown that area. It is a very pretty new building.
