The staff at Sage Meadows are always friendly and very helpful and caring, it’s always clean and all the residents appear to be well cared for. I would recommend to anyone who has a loved one in need of a place

Sage Meadows is a very clean facility. My loved one loves it there. The staff is super friendly, Denise goes above and beyond! The meals are excellent, and the care is just as good. The activities are good, and I feel very happy my loved one is there.

I am definitely new to touring and assessing assisted living facilities, but Sage Meadow was a great entree to this new adventure. The staff could not have been more knowledgeable, helpful and caring. The facility was clean and looked to be well-equipped for this vulnerable population. The staff did a wonderful job of answering my questions and guiding me in this new endeavor.

Everyone is very caring and attentive to my husband. He seems very comfortable there. He does have problems concentrating on activities and they try to get him involved. At the beginning they observed him to find what he likes and what his abilities are and worked with that. They have also added 2 new activity directors who are very kind and friendly. I am glad he is there!! I have peace of mind knowing that he is safe. They also let you know immediately if there are any problems or concerns. My husband is also a veteran and they work very well with the VA for medications and follow up. Wonderful place!!!! Great staff!!! Great leadership and administration!!!

This quiet community was nestled beneath a large oak tree which is visible from dining room windows - providing a nice view while they eat. We want to thank the staff for a lovely tour and their time on Saturday with us. The menu for the day looked yummy. the rooms were a nice size!

From what my sisters tell me, they are surprise with how much better mom is doing since moving into this community. The picked them because it was the cleanest and nicest place. Now my mom has been taken off hospice and when I talk to her she sounds so much better. She likes activities they they plan out and she loves her room. the cleanest n nicest.

We like the care of this community. We like that the community is always clean. The activities are offered when she is able to participate. The staff is very nice and always helpful. She seems to be happy with the meals and the variety. We would recommend other families for the care of a family member

We're very pleased with this place overall! I'd give them 5s across the board, I can't really think of anything they need to work on. Their social programs in particular are fantastic, Mom had become a bit of recluse in her previous apartment, and they did a great job here of getting her out of her shell and involved in the activities!

She has done a complete 180 from before she was here to now. She seems happy and having the time of her life. They are extremely comforting to the residents and take really good care of them. The food here is excellent and she can't stop talking about it. She loves the activities and loves being here. She has her own little space but also has people around so it provides her with plenty of socialization. We are very pleased with them and have already recommended them to others.

Spoke w/Luann. They are a small 19 room home & have a nice private room with bath overlooking the garden available. Rates are $3400 and up to around $4000 depending on level of care needed.Only charged for number of days used, and no long term contract required. Are equipped to provide full service to 'end of life'. Generally schedule a family appointment with Luann to tour facility, then Luann would come to home to do an evaluation. This could be scheduled for weekdays or weekends.

I have moved my mom and dad into the cornerstone. It's a very nice place. Mom and dad are very happy with it.

When we were looking for a new place to move my mom, cost was a factor. We really liked this community. Her apartment is in the remodeled wing and it is supposed to have extra staff available for the population. The communication has been great, although, due to the current situation, we would like if they would put out a weekly newsletter with building updates. We have only had limited ability to visit, but when we were there the staff were nice and friendly.

This is a nice community, but they do not serve meals except for the dinner. My mom is not able to cook for herself. The staff was friendly and the community was beautiful. Families would be happy with the staff here and how clean it is.

I was doing some advanced planning and toured this Racine facility for another perspective. My guide was [name removed] and she was both calm and articulate in explaining procedures and accommodations, as well as honestly answering my specific questions. The place is smaller with limited gathering areas and small, basic private rooms, but very clean, nicely appointed and well organized. Although security measures are in place for those with memory issues, the facility is not a locked memory care unit. The level of care the team is able to provide in this assisted living facility does seem to be more limited compared to some (i.e. transfers, equipment, skills, etc.) but seems a good, quiet place for those needing less assistance and more independent.

We are very pleased so far with The Cornerstone of Oak Creek community. The staff is friendly and caring and attentive to her care needs. It has a very warm and homey atmosphere. There are not a lot of options in the meals for my mom to get the most out of what she eats for nutritional value. I know it is hard to please everyone's palate but we are working on it. There are plenty of activities that the staff encourages the residents to participate. Right now my mom is adjusting to the staff members and hasn't yet ventured into making new friends with the residents. There is a bus available for the residents to go shopping and different outings. The landscaping is well-kept and there is seating available in the front and back for the residents to get some fresh air. So far we would definitely recommend this community to others.

Very nice. Like the room layout and space. Top so far ! No private pay requirement and doesn’t have to switch rooms when family care kicks in !

Most of the caregivers but 1 were very good. They treated mom with kindness and respect. The manager always sided with one of the caregivers that was not very nice to some of the residents. The amount mom paid went up twice in a year. That 1 caregiver, who was very unprofessional, said that the place was peaceful until mom came. There were not too many of the residents that could talk so mom spoke up for them and the 1 didn't like that.

Clean, efficiency apartments but not for assisted living if your elder is more independent. Small 24 bed facility. Homey feel.

I took a tour of this community I wasn't very happy. The staff there was not very friendly. The person that gave the tour was nice. The cleanliness on this community was okay.
