The staff are all very professional, kind, and understand how to work with memory impaired patients. They also worked seamlessly with hospice to meet moms needs quickly, and we were so appreciative !

Dad is doing very well here. He is not doing as much as I would like to see him do but the staff is always encouraging him to do things but that is not really his personality which is fine. The food is good...

Kind and Communicative

They were really great with letting me visit my mom during Covid. I just had to test in the vestibule & I was granted access.

Nice And Clean

I thought that Nazareth Vista was a very nice and clean facility. It seems like all of the residents there receive good care and also there is a lot of options for activities for them to participate in.

Sales Oriented

They were definitely more sales-y. I felt like I was buying a used care from there. Also, they weren't very efficient in getting back to me for a lot of things. That kind of worried me because if this is how they're treating me now, how are they going to treat my mom?

Horrifying care facility ! The worst

I had liscensing get involved ! They were fined ! They are a terrible facility ! They allowed the innocent residents to have scabies for 6 months ( maybe more ) I finally was able to take my mom out of there with scabies but they were being treated from head to toe for 6 weeks straight ! They told loved ones it was a rash they get every year ! All the residents who sit close together were scratching and scratching ! That’s abuse ! That’s just one incident ! I called A place for mom 2 times after I took my mom out there ! Telling you how horrifying it is there ! I trusted you when you suggested me take my mom there ! Maybe you should read all the complaints against them At liscensing ! Please stop endorsing them ! They also had the district attorney trying to file charges on 2 men attendants there . A man passed away and he has bruises ALL of his body ! Shame on you if you don’t look Into this facility !

The State needs to make regular visits

The staff that directly cares for my dad at this facility receive 5 stars because they are very caring and respectful of the residents and their families. Management have proved to be dishonest. Exercise/music in the morning is the highlight of my dad’s day here as well as the volunteers that come to sing and play the guitar periodically! The facility itself is in need of maintenance and overall facelift. More staff is needed to monitor patients. More often than not guest must call phone number of facility to gain entrance or exit facility, the button does not seem to work. Central heat system blows cold air through bedroom vents (63 degrees over vent). Space heaters are used in residents rooms and activity area. Receptacles don’t hold heavy plugs and need to be replaced. Doors to patio are sprung too tight and propped open with rocks when the weather is warm. Emergency call buttons in rooms don’t work. Electronic bed remote sits on headboard but doesn’t work. This is supposed to be a memory care facility and a staff member let my dad have scissors They need a nutritionist. Roof leaks on left side of lobby near hallway door.
