A Place for Mom has scored IntegraCare Corporation with 9.3 out of 10 using our proprietary review score based on 237 reviews in the last 2 years. Over all time, IntegraCare Corporation has 644 reviews with an average "overall experience" of 4.5 out of 5.
There was only one aide in memory care. So care was not even minimal at best. There were NO activities except for sitting in front of a tv in a very dark room I had to argue to have her clothes washed, even though I was paying for that service. They "lost" 5 pairs of her pants - never to be found. The food was awful. The one day I went looking for an aide to help me get my mom out of bed and nobody was to be found. I asked at the office and was told that the aide assigned there has to leave early to catch her bus. I guess it never occurred to them that someone needed to cover. The week that my mom died the med techs on nights refused to give my mom her pain meds as ordered. They said that they had a 1 hour leeway- 1 hour before and 1 hour after the ordered time. I tried to explain that it was a pain med specifically ordered every 2 hours, not a as needed dose of a daily med Once my mom was deemed "actively dying " no aides came into the room. I had to go out and search for someone to help me turn her so she would be comfortable. I reported all this to the supervisor and administrator but they had no interest and no solutions. If my mom would have lived I would have gotten her out of that hell hole ASAP.

This place has changed so much. It smells awful, and residents are walking around unchnaged in filthy briefs. Staff is so rude and has been a revolving door. Management changes have had a negative impact on morale overall. Very disappointing. Very expensive.

Do not send your loved one to this AL. Don't be fooled by the pretty front lobby - Fresh paint & new furniture cannot fix poor management. Call response metrics are false- they turn off bell & return an hour ltr to keep high response metric. One med tech uses racist language & her domineering attitude permeates the place affecting other staff. Management does not reigns her in despite formal complaints & notices to corporate offices. In fairness, there are a few good aides, but overall there is no "care mentality" like other places-it's more like warehousing. My loved one waited 3 hours to have her brief changed. Medication management is a ripoff forcing you to use their 3rd party service and the "Wellness" Center is a joke-run by an unchecked incompetent unqualified administrator. Medication errors are not uncommon. Staff trash residents rooms & do not clean up after themselves. On the plus side, activities programming, facilities maintenance, front desk, & food service are good.

I would not recommend this community to anyone. My loved one was only there for 4 days. We found our loved one in clothes that weren't his. Our loved one was left alone. The staff said that it was shift change and he would have to wait, since he was alone. My loved one got an infection due to him not being checked on. They wanted to charge for the whole month, even thought my loved one was there for only 4 days. I feel they are short staffed.

[Name removed] REVIEW' Glen Mills Senior Living-EDITH W. (continued 'part 2' after"because if all the bells and whistles going off!") NOTE: Facts contained herein can be verified thru PA Dept. of Human Svcs. and/or PA State Police. ....I'll NEVER forget that comment of disinterest as long as I live! After reaching Dad's room, I found him in ACUTE RESPIRATORY FAILURE!!! No one had given him the doctor prescribed albuterol because no one had looked in on him to see if he needed it or not, violating specific doctor's orders to "keep him on a short trigger"..keep a very close watch on him. I didn't even need a stethoscope to hear the gurgling of the accumulated fluid in Dad's lungs. He had all the covers up on him, all his clothes were on plus a sweater and a towel was folded in half over his head, he placed there to keep the light out so he could sleep. No checks on him, no care, no albuterol, NOTHING! Dad's Death Certificate reads at letter 'D': Cause of Death- RSV as the originating cause of Death.Glen Mills Senior Living even failed to get him to the Emergency Room as they had done for Mom without doctor's orders. I had to drive him to the Energency Room myself, as no one offered to do anything for him. And we're paying over $6K per month for this? As for Five Star parent company...they're no better. The Reg. Dir. of Operations, the Reg. Dir. of Healthcare Svcs. and the Pres. and CEO did nothing. After Mom's return from the hospital, we reported incident after incident in accordance with our Residency Agreement. One such incident whereby a nurse who was giving a breathing treatment to Mom by mask, strapped the mask over my 91 1/2 ye old Mother's face and left the room, for getting to turn on the air compressor that supplies the air to breathe in the mask along with the albuterol and budesonide medication. When the nurse saw me arriving downstairs, it surely reminded her of something important, BECAUSE, she literally raced up the stair tower steps(I took the elevator as usual),arriving together at Mom's front door. Both of us entering as the Nurse exclaimed: "Oh my God, I forgot to turn it on!" as I watched Mom's rib cage rising and falling quickly, with her gasping for breath I saw the mask on Mom's face. Both Corporate and the facility disavowed the fact that Mom was almost suffocated ; instead promoting the statement that "the nurse never put the mask on Mom's face and had been called away on another emergency! AND that "she didn't suffer from any clinical issues!" Our family experienced total denial! WHAT A LIE! I was actually there and I saw the mask on Mom's face with her gasping for air when I entered the room with NO AIR COMPRESSOR RUNNING, just as the nurse had exclaimed. with my own two eyes. Do you want LIES AND COVERUPS as a main ingredient of your Loved One's care? Even the PA Dept. of Human Svcs. did nothing as they were trying to get the facility's 'Provisional License' restored to 'Regular Lic.' by October,2019. Because of this, the incident has been referred to The Insoector General of Pennsylvania's Office with Senator [name removed] spearheading the investigation into the PA Dept of Human Svcs. inaction concerning the facility negligence, including our Father's wrongful death and our Mother's breathing treatment gone horribly wrong and an additional matter I have first hand knowledge of, per the PA Dept. of Human Svcs. report to Corporate Pres. and CEO of Five Star: of a woman who choked to death on a hotdog while eating in the dining room at GMSL. A report was filed with the PA Dept of Human Svcs. indicating that firstly: no staff members in the Dining area at the time had training in performing the 'Heimlich Manuever' and secondly as I read the detailed report that: no one had called for EMT assistance until 17 minutes after the female resident lay aspirations on the floor. When EMT's arrived in the dining room, she was deemed dead at the scene. Is this the type of facility you want your family member staying at? Don't chance your Loved One's lives at this despicable personal care facility. Yes it is a 'personal care' facility and it is NOT covered under the statues of 'Assisted Living' facilities covered by the Bureau of Hearings and Appeals in Harrisburg, PA . Our own attorney didn't even know this, but now YOU DO!! The Five Star Corporate Attorney Firm took advantage of this and had our Dear Mother evicted..yes, evicted at 91 1/2 years old because the Judge declined to render an opinion since his Court did not cover 'Personal Care Homes' and a PA Dept. of Human Svcs. Licensing Manager, who wanted to restore the facilities 'Provisional' license to 'regular' license wrote a letter stating that our Mother was "a burden to the facility" and our family did not have further funds to fight the case. It cost us $5K in attornies fees, a life changing upheaval for our Mother and a New Community Fee of $3,500 at Sunrise Assisted Living which, by our personal witness, is far superior in EVERY way to what we experienced at Glen Mills Senior Living. The Director at the PA Dept. of Human Svcs Headquarters in Harrisburg has been given the assignment of investigating all of the elements of this horrible Senior Living experience...one of total incompetence! Please check for yourself, if you desire further substantiation prior to making your decision. Go on the PA Dept. of Human Svcs and enter in the appropriate fields: Personal Care Homes, Southeastern Office Delaware County, Glen Mills, 19342, Flen Mills Senior Living and go from there, perusing all the violations submitted to The Corporate Pres. and CEO's office. Thank you for caring enough about your Loved One to heed this honest, but painfully accurate review [removed] Mills Senior Living now appears to be under new ownership..No longer Five Star Senior Living, but now SNH Penn Tenant LLC, also located in Newton, Massachussetts, however. Recommend investigating further into their 'new' operating license that runs from [dates removed], keeping in mind that it's only a piece of paper. Based on your knowledge of all the lying, corporate deceit and governmental philosophy "it's better to cover up negligence and evict one person, than to shut down a facility for negligence and displace a hundred peoole!"; you are left with the decision.. "Should I place my Loved One at Glen Mills Senior Living?" I think not.. anywhere else is where you should go. Goid luck to you... and God Bless.

* 4th degree bed sores and a hole in the roof of her mouth that extended to her nasal passages (no one reported this to me -- I had to wait for Hospice to). * sitting alone and in a soiled diaper (uncovered and unattended to for hours). Filed a report and nothing was done. * war between hospice and The Pines over hospice decisions made by family/hospice. The Pines wants to extend the life of a suffering client to profit (everyday a patient is in that bed means hundreds of dollars in profit for them). *Accusations from The Pines that I wanted to rush the process of ending My Aunt's life by pushing Hospice to prescribe more morphine. *Sad and unforgiving experience. I told my children to never, ever, pay that much money to place me in a facility that puts profit over care.

The Pines of Mt Lebanon is unbelievably understaffed and unclean. The residents are neglected and usually either smell of urine or are dripping with urine. Their is little to no afternoon staff. Memory care smells horrible with no help to the fact that laundry is only done once a week. The corporation does not care for their residents or staff members what so ever.

The review is of the Memory Care Unit. It is understaffed, especially on the overnight. Aides will take a break while leaving the remaining aide alone. They encourage hospice because they do not have the trained staff to deal with Alzheimer's patients. Hospice could get my parent to shower every time they visited; only the Woods' social director could get my mother showered. No aide or MCU supervisor could do it. The activities schedule looks full but out of the 20-30 times I ever visited, at all different hours of the day, there was only one time that the scheduled activity was actually occurring. A resident was able to escape through the patio gate and was missing for at least an hour. They claimed there was an electrical problem with the alarm, we think they ignored it for too long. All of the wheel chair bound residents are gathered in the dining room every day and spend the day sleeping there. This place looks great, but looks are deceiving!

Our tour was awful, I would not recommend the facility to others. We walked in and it was so dirty that we wanted to leave right away. If the man showing us around was not so nice we would have. When staff were wheeling residents by a lot of them smelled like urine. It was not a good tour experience.

I was appalled with their choice of movie for the residents to watch. I visited in the afternoon, right after lunch, and the residents were all there watching TV, which I don’t object to. But the movie was smash 'em/clash 'em, just very violent. And residents with certain conditions can’t be exposed to things like that because they can’t separate reality from fiction. That would have been extremely distressing for my loved one. I asked what the criteria was for picking movies, and the person on staff answered "Whatever keeps them quiet." Which to me indicates that the staff there are undertrained. The memory care outdoor area was also just a little outdoor space carved out of the larger patio area and gated off. Memory care seemed to be more of an opportunity for them to make money than to truly care for and support the residents.

Very disappointed in this place!!! My father did not at all received the care he needed, even with doctors orders that needed to be carried out to ensure my dad’s health! Even basic care such as showers and hygiene was neglected on many occasions! Despite voicing my concerns multiple times, nothing ever changed!!! No one ever helped my dad with incontinence care, and a bedsore resulted. I very often had to perform care for my father due to their neglect in doing so. The executive director acknowledged that there were “MISSES” in my dad’s care, but that he would speak to the nurses to try to rectify this. “MISSES” in care caused my father pain, lack of dignity, and health issues. The place is very understaffed and uses agency nurses constantly to try to keep the place going. Call bells go off and are unanswered for long periods of time all the time. I hear the call bells ringing in the halls for extended periods and no staff are even around checking on people or answering them! The lack of care my father received is unacceptable! We moved him out last week, and he already is doing so much better! Shame on Glen Mills Senior living!

Absolutely HORRIBLE care. Mom has been here almost 5 years and her care has gotten worse as each day passes. They had a remarkable nursing staff here when we first moved her here but because of them always being short staffed and overworking all the good employees that they had, they all started leaving and finding better jobs. They have lost many good, caring aides because of them being overworked. The housekeeping is absolutely HORRIBLE now too! Soiled furniture everywhere and no place to sit when visiting. Strong smell of urine. Again, they lost a great laundry person and a remarkable housekeeper for the same reason. Whole building is filthy! Kitchen staff keeps quitting too, so much that managers are serving meals most of the time. Activities lost a great employee whom all the resident adored! Same pattern in each department...it’s tome to start looking for new options for Mom. Explore your options and did deep before bringing a loved one here!

Do NOT put your loved ones here!! My sister and I found ourselves in a legal situation with a hateful stepsister who placed our father in the Exton Senior Living facility. The staff was so uncooperative it was unbelievable. They had no respect for the fact that we were his only two children (who were in no way estranged from our father). We had to get our 91 year old father to insist they put us on his Emergency Contact List. And even after he did that, they never contacted us when he was put into the hospital. He died the next day and we had no time to get there. We could barely get them to bring him a phone so that we could talk with him. Both my sister and I live states away. It was a horrible and depressing experience to say the least! Do NOT put your loved ones here!!!

I have seen first hand multiple times the miserable and at times rude staff at this facility. I am able to witness this because I am not a family member. The dining room staff is beyond miserable and rude. I have never seen a pleasant worker in the dinning room and the food and service is horrible. You do find a few pleasant and helpful employees but they are few and far between. I have been to multiple facilities where the staff and quality of food served is wonderful but not here. God help those that can't communicate in a place like this and for those that can they may think all facilities are like this but that can't be further from the truth. They are continually understaffed which effects what little quality of care they do get making the situation even worse. I will say that the facility is clean but that is about the only positive I see. The few daily events are basically the same every week. So there is very little to do. Please take the time to look around before placing your loved one here. This facility needs to take a hard look st who they are hiring starting in the dinning room and insure they have enough staff to meet the residents needs. There is barely a warm welcome when entering the facility which should be the first warning sign. I see what they don't want families to see or know. Please please please look elsewhere.

I am not sure where to begin. My Mother is now in Lancaster General Hospital, because she fell at Magnolias on Tuesday, the 1st of March 2016. She has bleeding in the brain and we are deciding if we should put a feeding tube in for her. No one from staff had looked in on her for three hours. No one from the staff called the children when she fell for two times .They wanted her to eat breakfast before calling the ambulance. They were not going to call the ambulance, but Thank God, my sister-in-law was there and made the call. The laundry was not done at all for the week she was there. She had no bath. They were suppose to order a bed rail and a fall mat and that never happened. They told us that on Monday a psychologist was going to come to evaluate Mom. That never happened. They made us stay away for four days. That was precious time we could have spent with our Mother and they would not let us. Now my Mother needs skilled care and not sure how long we have with her. She cannot swallow at this point. Does anyone at assistant living places care. I spoke with Heather for an hour. She never apologized or would admit that these things were not right. She wrote some things down and said she would check into it. If she calls me back with what she has found, I will be very surprised. We could get a lawyer, but what good would that do, Mom is not good and I feel Magnolias has failed us. I really would like all the money back that we had given for Mom's short stay. Some of the people were very friendly and we appreciated them. You need more staff to be able to run a more safe and caring environment for dementia patients. Heather made it sound so wonderful and great when we met with her for the first time, but we will never recommend your facility to anyone we know. I am sorry for this negative note, but my Mother is in very bad condition because of the negligence shown to her and it makes me sick to my stomach.
