My mother has Alzheimers-type dementia. This facility has proven to be professional, caring and has a staff very well trained in handling Alzheimers.

My sister and I visited our father and his wife shortly after they moved into Arbor Terrace. They had been living in an assisted living facility that no longer was enough care for them. When we arrived they were sitting around a table with other residents playing bingo. I was happily surprised to see them socializing with others since they had been very reclusive. The staff there is helpful, friendly and very professional. The facilities are perfectly suited for people suffering with memory issues. Everything is on one floor and centered around a "great room" where most the the activities take place. This room opens out to an enclosed outdoor area where there is no possibility of them wandering off or getting lost. I highly recommend Arbor Terrace to anyone looking for a memory care facility in the Jacksonville area.

My husband has lived in Mt. Pleasant Gardens since they opened last summer. I highly recommend it for anyone with Alzheimer's or dementia issues. The staff is professional, yet warm and caring. The many daily activities are geared to each patient's ability. And the facility is truly beautiful -- with a gorgeous enclosed garden. Management is accessible, and ready to hear any request or concern. If you have the misfortune of a loved one suffering from dementia, you will at least be comforted by the care he or she receives at Mount Pleasant Gardens. [name removed] Mt. Pleasant, SC

They have been doing very good. The food is actually pretty good, but they don't follow up with me as quickly as we would like. If I call or email, nobody gets back to me and I have to call back.

Kinda rough the first few weeks for all involved - Mom/dad, us & the caregivers. Things have settled down to a point where we would provide the following ratings: staff-5; quality of care- 4.5; activities-5; food-4.5 (dad 5+; Mom 4 as she is gluten free); cleanliness-5; room-4.5. Bottom line once the staff/care givers get to know the patients they are trending toward 4.5 to 5.

Ponte Vedra Gardens is just what we were looking for to care for my wife. Nice size and specializing in memory care. The staff is caring and professional and has been understanding as my wife has had a difficult transition. She seems to be settling in a little more after 60 days, but is not completely acclimated just yet. I expect after a few more weeks she will start to feel like it is home. That is my hope anyway. PWG has been great!

When it comes to the meals, the food should be a lot better and the quality of the staffing has been an issues. However, the nurses and management team at Mount Pleasant Gardens are great.

My loved one is currently a resident at San Jose Gardens Alzheimer's Special Care Center. Overall, her experience here at this facility has been very good. The friendliness is great! The cleanliness, staff, and quality of care are good. The value for cost is also good. I would recommend this facility to family and friends.

We picked this community, because for my needs, it was the best choice. It was a positive experience. They have the willingness to work with you regarding the needs of your loved one. It is not the newest of facilities but it is a good choice, if your loved one doesn't care about fancy or newness of facility.

I had called this community to get some information for my mother on possibly living in there facility. The staff where very helpful and I appreciated there patience with all my questions. The only problem I ran into was they did not allow cats in there memory care unit and that was something that she had to have. Otherwise we have no complains and everyone was very professional

The meals there have always looked very bland when I have been there. For example, I was up there just yesterday on Christmas and rather than a Christmas dinner they just had chicken nuggets and sandwiches. The food is the main area in need of improvement. Other than that it has been a pretty good experience. The people there are very helpful, friendly, and nice.

The tour here was a good job. It was a clean and beautiful community. Seems like a suitable facility but I felt my mom would not fit in. They were more active and social. They were helpful, kind, and considerate.

They have not done what they said they would once my loved one moved in. We have not had the best communication. The majority of the nurses are super. I have not seen any activities. The food I have seen looked good. The community is beautiful. They advised us they were a memory care only community, however there are some residents that seem to be more assisted living.

Found that they made a lot of empty promises when I was researching the home that are not being up held now that I placed my Dad there. Staff is very friendly, but I have found they socialize with one another too much leaving residents to fend for themselves. Over all this was one of the better places I found.

This is a very clean and well maintained facility. The staff was very approachable and caring. The problem is it is a for profit facility, which the cost is roughly 5000.00 and up. They charge extra for everything. Diabetic meals, extra help with bathing, etc. Which means when her money ran out she would have to leave as they do not accept Medicaid or public assistance .

Many times we have visited my father in law, and he has been walking around without his glasses. He needs his glasses. He is a fall risk. There have also been many instances where he was walking without his cane, and he should not have been. They need more helpers for the nurses. Dad has a roommate. It was less money. He receives extra services that we pay for. It all adds up.

My dad was diagnosed with Frontotemporal dementia 3 years ago. We wanted to keep him home as long as we could, but eventually, we couldn’t manage him at home. We spent months visiting facilities to determine which would provide the best care from him. We chose Mount Pleasant Gardens, because we were sold on a family feel and quality care. We are giving 2 stars, because some of the employees and the activities director/assistants are outstanding. The activities people are trying hard to provide a variety of activities for the residents. We knew that no place would be perfect, but we did expect a certain standard of care. In our 7 months at Mt Pleasant Gardens, we saw many things that caused us concern in the level of care being provided. Overall, there is a lack of professionalism and respect for the residents and their families. I’m addition, it seems that many employees are not properly trained on how to care for residents with dementia and mobility issues. Often during our visits, employees were sitting on their phones and didn’t even acknowledge us being there. We asked ourselves-if they aren’t attentive when we are here, how bad is it when we aren’t? The strict visiting hours concerned us as well. We were made to believe we could only visit during visiting hours, and were not treated well if we came early or stayed late. We now know that we legally have a right to visit 24 hours per day. We brought several concerns to management, but never felt they were truly addressed. We lost confidence in the managements’ ability to ensure quality care and we felt we had no other option but to move our dad to another facility. We are very pleased with the new facility and have seen a huge improvement in Dad’s mobility and attitude. We have chosen not to provide a laundry list of concerns on online review sites, but we have addressed those concerns to the upper management of Mount Pleasant Gardens, and hope that they will be working to improve the experience so that other families and residents don’t have similar experiences.

Our initial experience with Arbor San Jose started well enough, however as my fathers demensia progressed to where more direct care was required, he was refused to be accepted back following a hospital stay, despite being told specifically they could care for him even if he wasn’t mobile. We felt like they were shopping for patients that require the least amount of direct care resulting in our having to move him a couple more times with little notice and or options. My recommendation is to ask pointed questions.
