There are very few activities offered. And when they are maybe a handful of people attend. That is very disappointing. Otherwise it's a nice building - it is very clean and quiet.

Expensive places aren't necessarily the best.

A friend's relative resided there in her later years so I looked at the York's website and reviewed a marketing package. I did not proceed further with a visit because my friend's relative had her medications [Removed] as well as other issues such as inaccurate billing. It was also more expensive compared to others I was considering.

Terrible Experience

My mother-in-law was there for respite care and it was terrible. They lost her meds. It was supposed to be smoke free blinding but the staff smoked downstairs so when you come up the elevator you can smell it. When we checked her out it took 10 minutes to get a nurse to come help us. Nobody took accountability for the meds. They insisted that we didn't bring them. Then later they magically showed up. When we were checking her in it took 35 minutes for someone to bring us to her room. Overall, it was not a good experience.

Inadequate Staffing Levels

My grandmother was living here for over a year. The main problem seems to be the high turnover in staff, constant shortages of staff, and inability to provide the services that they charge you for. Sometimes when she would press her button for help, it would take an hour for the aide to show up. They told her to just urinate in her Depends. That is disrespect in my opinion and makes the elderly person feel terrible about themselves. That could also cause a rash. We finally had to move my grandmother to an assisted living in Burnsville where the staffing levels were better. She is 100 times happier now that she is moved out of Nine Mile Senior. I do have to say that the activities were great, and the meals seemed pretty decent at Nine Mile. But you cannot run a senior facility without adequate staffing!

Not Happy

When we moved mom in the staff was very friendly, since then some staff don't even say hello to her in the hall. The front desk doesn't seem to know much either and isn't very helpful. The food I was told is bad, small portions and bread/deserts are stale. Definitely not worth the price they are charging. Grocery store outings are too early, sb changed to afternoons, to accommodate the seniors. Very miss managed and lack of communication between staff and residents.

Memory Care Unit at Fountains

We moved our mother into the Memory Care Unit at Fountains at Hosanna In May 2013 with high hopes that this would be her forever home. The facility looked beautiful, and we were promised a safe and caring environment for our mother, who had symptoms of dementia. However, we rapidly learned otherwise that this facility did not live up to its advertising. We took our concerns to the management team of Fairview/Ebenezer on several occasions, in person and in writing, and found them to be uncaring and unresponsive. It was always their opinion that our mother was at fault or we just did not understand her condition. . . After events leading to an unattended fall and a femur fracture, we asked for a full case review. However, the conclusion by Fairview was that they were not at fault. Our mother is now wheelchair bound and incontinent, never recovering from the fracture, which worsened her dementia, but thankfully she is now at another facility where she is well cared for.

Big mistake

Memory care is dismal. Staff constantly speaking very loudly in foreign language. Largely inattentive and did not interact with residents. Care plans not followed. Non responsive management. Poor housekeeping. Very few activities. No choice in food. Had to get home health care involved to get my mother bathed. Missing items; clothing, hearing aids, fake jewelry, etc. Maybe a lot of places are like this but keep looking so your loved one is happy.

Don’t put your loved one in memory care here

Reading the other reviews just prove that nothing changed at MW since 2019. Dirty. There is either a communication barrier with some staff or it is used as an excuse not to answer questions. Very loud conversations between staff in foreign language. No choice of food, and it is lukewarm by the time it gets to memory care. Male staff is very gruff with residents. Cell phone/personal device use by employees is off the charts, staff sitting in chairs charging their devices surfing the net. One male staff member even takes his shoes off while at work. Very little positive interaction between staff and residents, even at meals. One evening staff ordered in pizza and sat in the living room area with their feet up on the furniture completely ignoring residents. Limited activities during the week and none on weekends. No nurses in the building evenings or weekends. Serious concerns with physician orders being processed in a timely manner. DON’T PUT A LOVED ONE HERE.

So far a nightmare in memory care

They place people in memory care based on SLUMS assessments. No activities after 2. They are not allowed a choice of food, and what they are served looks terrible. Residents discovered in my loved ones bed and room consistently, clothes missing, awful!!!

Run Far Away

Extreme staff shortages, poor food quality and food served cold. The activity director refuses to plan activities outside the building. Washers and dryers not maintained at all, emergency generator is not connected to apartments, so residents in wheelchairs, on oxygen, or needing lifts are in danger during blackouts. Currently managed by [name removed], these serious issues have gotten much worse.

My tour experience

They never showed up for my virtual tour. They felt really bad about it and tied to get me to come back, but I was done at that point. We were also considering moving our loved one to a different state, so that also influenced my decision not to reschedule.

Poor customer service and business relations

My loved one lived at meadow woods for about 1 year and we were very displeased with care and services received during their stay. There was a high turn-over rate among nursing staff, some of the home health aids were very rude and neglectful of responding to my loved one's needs. Some of the staff had difficulty in understanding english so communication was a challenge with nursing staff, and caregivers, cell phone use was frequently noted by caregivers and breach of privacy was observed regarding resident issues and concerns. I found this to be a very chaotic, dysfunctional environment regarding the communication and interaction between management and staff. High need for improvement in communication, resident cares, and managerial structure.

The representative for tour was more like realtor salesman. Memory Care is not a sense of home in the layout.All 36 units are divided between two hallways. Floor is divided up in 6 to 7 areas. An individual with memory issues would get lost on the floor plan. They are do not allow 2 assist with their residents. The individual handling the tour wasn't concerned with my loved ones needs. He was just out to pitch a apartment. He didn't seem open or knowledgeable to questions on how your loved ones cares would be handled. Well decorated. He was more concerned with the bells and whistles of the building. Memory Care level just seemed like a well decorated nursing home environment. With lot extra rooms for different group activities. But where on their own you loved one would just get lost. More for the individual in the early stages of Dementia/Alz. Seems like a place for independent senior living.

Things did not work out there

Things did not work out there at all. He stayed there one week and he could not handle it there. He just could not stand being alone. He though he could do assisted living, but he could not. I was kind of disgusted with the whole process because there was a lot of red tape to get him out of there. He was only there for 5 days and they continued to bill us and the also billed us for pills that he couldn't possibly have used. They are after every dime they can get from you and it is just a racket.

The tour was great. We were given 3 tours and [name removed] was very helpful in filling out our paperwork ( some forms were not completed correctly). The people making the decision not to accept him were totally unprofessional. I get being nervous about his previous behavior but the overall feeling was they were punishing him (family) for causing them trouble. (long story there) Their words to the hospital staff "he is only behaving because he knows he is getting out of here if he behaves. He is going to be a troublemaker and we don't want him." The staff from the hospital felt the nurse from Sanctuary making the assessment had already made up her mind before she did her assessment. If the administration treats the residents like this I would be nervous about having my father in law live there. [name removed] says it takes 4 to 6 weeks for her to process the application and she handles one application at a time. She moves on when she runs into a snag.
