Our mom came here in Feb 2019. She has dementia, but medication has helped her manage her behavior. She walked, talked and danced when she came in February. She was in Personal care, but enjoyed going to dances in Memory care. She started using a walker early April and then she fell later in April. She was first sent to the hospital and then to rehab. That's when we learned she suffered muscle weakness due to lactic acidosis because she laid on her side for a long time. In rehab she walked with a walker, but with someone behind her, as she feared falling again. She came back to personal care at the Terrace in early May, but we all agreed she needed to move to memory care for full care. New mgt took over mid June. Things started to go downhill. Staff not responding to phone calls, concerns, etc. Our mom shared a room, as this was all they had in memory care. Finally in early August she moved into her own room on a different floor. That's when the new floor's aide noticed she had a blister, covered with a band aid. We knew nothing about it. No real explanation given. Our mom became incontinent because she would call for bathroom assistance that didn't come in time. We would find urine drenched clothes in her hamper. Urine stained sheets on her bed. She doesn't even try to walk anymore, just stays in her wheelchair. On [date removed] my sister and I were both visiting. We assisted our mom to the bathroom and saw she had a bedsore. I went to get the nurse in charge and the memory care director. Both said they knew nothing about it. How can that be? Our mom has full care. Someone is bathing her, dressing her, changing depends. We called the new Director that day. When she finally called back she was defensive and rude. I waited until Friday for her to follow-up. She didn't, so I called again. She called back and had no info as to how this happened. Finally Wound Dr came that Monday and said she has a stage 2 bedsore. That blister we saw in August was stage 1. He had to debride the area and gave treatment instructions. This is so not a place to put your parent. Our mom went from walking and dancing, to a fall that left her scared to walk, then a bedsore, dirty linens and weight loss. I would never recommend this place to anyone.

Beware of the memory care wing. Use of "pharmaceutical restraints" is a common practice -- happened to our loved one. Caregiving staff is grossly untrained and uncaring -- turnover is constant. Management is most concerned with shirking responsibility, while "contracted" nurse practitioners dole out heavy psychiatric meds to dementia sufferers. This resulted in a fall and injuries for our relative. These people and this company care about profit not people. They paint a pretty picture, but the care is so sub- par. Also have a problem with bed bugs.

Mama is in assisted living and she hates the food there. Another of her complaints is that when she leaves her room and locks her door, she will find it unlocked when she returns. She suspects that night time workers are entertaining boy friends while on the job, because the chatter and laughter outside of her room is sometimes loud. Considering that she pays four grand a month for her room, I guess that she has the right to complain. Her overall health is much improved since living at Somerby, and that is really important. Mama's biggest problem now seems to be forgetting and losing things. It is great having her medicine administered on a timely basis, because of her forgetfulness. She recognizes that her health is better since comming to Somerby, but she doesn't like it there and wants to move out. If she had a bigger apartment, she would likely be happier.

This place should be absolutely shut down !!!! Never and I say never should send anybody in their right mind should ever send a loved one to this treacherous place. she was sent there for Alzheimer’s care they completely numbed to her out!! They had her on Xanax I believe more than one time a day obviously to the point where every time we came she was completely asleep and out of it and completely high almost, and obviously Xanax is one of the most often do use drugs for people to abuse , Xanax is an Anti anxiety drug and causes major memory loss and extreme drowsiness. she was abused by other patients one Stabbed her if a fork when she was unprovoked and another man pushed her down for no reason it it broke her hip and she died a few months after. This place haunts me and like I said should be shut down never send a family member there!!!!!

I would not recommend this facility. I moved my mother into an apartment when they first opened a few years ago. I was told because her care changed and “she needs more care” she couldn’t stay. I wasn’t told why or what they could not provide. Overall the care isn’t great. She doesn’t need to be in a nursing home with that level of care, so it didn’t make sense to me what I was being told. She is now living at another assisted living and is much happier.

Although the facility is beautiful, the day to day goings on appear to be very unsettling. I am very involved with my Mom's care, so I admit I can be a bit "pressing". The nursing staff overall is great, however, I've noticed that staffing always seems to be an issue and it is nearly impossible to get return calls from management. It is very expensive, and continues to gradually increase in price because of what they call "service tiers" meaning that the more care your Mom or Dad requires, the more expensive it is to stay there. My Mom has recently declined, and among dealing with those changes, I was hit with an added charge through MAIL. I'm becoming very disappointed with the overall experience and I am looking into other options for my Mom.

I will give 1 star due to the caring aids, nurses, and non-administrative staff at the Bridges. However, when an extremely dangerous situation occurred resulting in police returning my mom to the Bridges, the administration was [removed] and nontransparent. After several phone calls, a trip to the Warwick police station and obtaining the information from my moms fitbit that she showed had walked 9.6 miles, I was able to get to the bottom of the events. Not only did the staff not investigate the event after she was returned by the police, they had no intention of calling her family to notify us of the situation. In response to my mothers getting lost was a mandatory "wander guard". Which meant every time my mom got close to the door the door would lock. Therefore when she would want to smoke an aide would have to take her out to smoke. But because the Bridges is close to full capacity the aide was unable to stay with her, so she would be left locked out of the facility and wait until someone could let her in. I recently found out that she waited and waited, and during that time went for a walk to the giant! This became a great source of stress, not to mention that the only option was a bracelet that look her independence away. The newness of this facility leads to a lack of policy and procedures. I have since taken my mother out of the Bridges and not one member of the administrative staff has reached out to my family to express any remorse for the situation. Yet, the day after she left $6,325.00 was taken from her account.

I would not recommend this facility. We showed up and outside there were cigarette butts all over. It was filthy when we went in and the tables were all sticky and it smelled like urine. We saw the room and it was very tiny and only had one small window so it was practically dark. Everything is an add on price. We did not see any residents while touring and we found the person we spoke with to be unprofessional.

Don't do it. It's all about the show. It looks nice but if your family member needs extra care, they don't want to do nothing. They kept sending her to the hospital and the hospital said my loved one wasn't getting her medication. The facility was responsible for providing her medications and they were not clearly. She suffered a fall (sliding out of her chair) and the staff left her on the floor until the ambulance came to pick her up off the floor and put her back in her chair. The staff had notified us of a few incidents except when she suffered bruised ribs and we were never contacted about this incident. Our loved one told us, not the staff. She had visible bruises. They gave a 30 day notice that she had to leave after this. Half the time she was there, she was in the hospital. If your family member doesn't need much care, it's nice. If they need anymore attention, this is not the place to go.

I moved my mother into the Bridges about 6 months ago. It's extremely poorly run and they never have enough help. The staff never returns to called to anxious children and they have had several cases of Covid recently. The nurse in charge never returns calls when you are calling to report a sick loved one. She apologizes that she is far to busy to return her calls. Corporate that owns facility doesn't care about anything but their monthly payments.

Very non-responsive to requests, calls and support. Their COVID19 actions were late and subsequently at least a dozen residents were infected along with several staff members. Getting information on our loved one at the facility is non-responsive. They recently were sold and they have BIG issues here. Stay away and if you can, remove your loved one immediately. Very expensive care and you definitely don't get what you pay for!

The worst experience of my life. We looked around for a place for my mother with Alzheimer's and at first glance it looked great.. The Blake is new, updated, and didn't have that old urine smell that most of the other facilities had. We met with sales and he was very nice and helpful, we thought wow this is going to be great. Well, it was a nightmare!! My mother was left in soiled (feces) clothing all the time. We would come to see her and have to shower her every time, which was everyday because we visited everyday!! The sheets on her bed were always soiled and never changed. On days where they striped the bed, they would replace them, some of nights there were NO sheets on her bed at all and I would have to go to the laundry room to get her sheets and put them on myself. The floor in her room had feces all over it, which we have photo's of. The residents are locked out of their rooms unless they are sleeping and the common area bathrooms are pretty hidden..Since my mother couldn't find the bathroom and they did not take my mother to the bathroom she ended up just going in her pants. She came into the Blake in regular underwear and within 2 weeks was completely incontinent. Funny part is, whenever she was taken out for a day visit with us she never had an accident . She ended up coming down with 2 UTI's and the worst raw rash in her private area. It took them 10 days to get UTI test the first time and the second one my sister took her to urgent care because between the UTI and rash, my mother need emergency attention. Most of the PA staff were completely rude to the patients and family. It was like, we were inconveniencing to ask for basic care. The staff would sit around on their cell phones not even paying attention to the residents. They would yell at the residents all the time.. As I said before, they keep the residents doors locked at all times and the residents are not allowed to go in their own rooms. Once, there were a couple resident is one of the rooms and one of the PA staff started yelling at them to" get out and that they know better than that".. Well, guess what? they don't, they have alzheimer's.. I realize alzheimer's can be frustrating but they, the residents can't help it. The final straw was when one of the staff twisted my mother's arm in front of my sister and another staff. Thank goodness the staffer was an honest person and spoke up to her superior. That Abusive staffer was let go, but she was a manager and can you imagine what happened there when no one was around, if that happened in front of my sister.. Now the food, it looks like bad 1970's TV dinner food, NOT restaurant quality food!! Frozen crap that they would heat up, I would be ashamed to call myself a chef and the Blake.. My family and I have tried to contact the corporate office and have never heard back. I called Phil Benjamson myself and also have heard nothing.. You pay a $3000 community fee and if they don't live up to their end of the deal, you're still out your $3000.. Now, I have had to come up with another 3k to put her somewhere else. After only 6 weeks, they should apologize and give me back the community fee at the very least.. Since, I have not heard back, I will be calling the state!!! because no one should ever go through this, EVER!!!! SHAME ON THEM!!!

Mom went to the Terrace after coming off of rehab from a fall. Unfortunately after 3 months, she fell and spent 7 days at Chestnut Hill Hospital. They wound up not treating the fall but the multiple bed sores that had been allowed to increase and get worse while at the Terrace, one so bad it had to be operated on. Overall, the terrace is okay if your love one doesn't need special care.
