Evergreen Valley Assisted Living DOV: 7-16-15 Positive Aspects • 15 residents both male & female • Physical environment o Clean o Adaptive equipment such as hospital beds with bed rails, ramp entrance to residence, stair glide, hand rail in bathrooms, toilet hand grips, shower chairs, mats/nonskid strips in bathrooms, and wall rail on 1st level. • Staff’s interacted in a caring manner and was able to redirect residents when necessary. It appeared that this came from training as well as their skills developed from experience. • It appeared diets were followed. • Residents that were not actively eating were assisted with being fed. If further reluctance was displayed by a resident they were offered an alternative food. • Younger group & older group o Younger group appeared more independent o Staff maintained closer supervision with older group o Variety of activities on the calendar Other aspects of concern o Concern if my parent did not have a bedroom on the 1st level o Staff ratio 2 staff to 15 individuals

The visiting times can be limited even before covid. I also would like to see more activities to engage the residence. The communication with the families needs to improve and be more proactive. The staff is very loving and kind and are doing the best they can.

This was a very nice place. It is kept very well by the staff. My mother was very well treated here, despite her difficult behavior. She was very difficult to deal with but the staff was very good with her. The Manager stays on top of things, and does an excellent job supervising the staff, and being attentive to the residents. My mother was very well cared for and I am very thankful to have found this place.

I truly was happy when I got [name removed] out ot there because I wasn't sleeping because of worry & he wasn't happy there either.Unless they have been more selective of their staff I wouldn't recommend. Most of the food dispensenced on Corell, a tablespoon @ a time. examples oatmeal applesauce finely chopped stew, portions were small for a man.

The main concern we have is the overall communication with the families, but we do know that was probably caused by the staff being short handed.

The community was clean and the providers were all very friendly. There were no openings at this time, so I will continue to look.

They could improve on the activities. I never see any. I feel like they are just put in front of the TV and that is it. But i'm not there in the morning, so it may be that they do some in the morning, I am not sure. It's okay, but my mom's hair is always oily and I think they just have too many residents and not enough staff. They do care about the residents though.

This is a large older home. While the care seems to be good, and the staff attentive, the space seems very tight. Furniture is arranged around the perimeter of the activity room to fit everyone. There are stairs to be navigated. Bedrooms seem large enough. Most are shared.

This was a nice looking community, and we were looking forward to using them on a long term basis, but we were told only a few days after moving in that our loved one was not appropriate for their community. We had been very explicit about her issues, and they assured us it wouldn't be a problem, which did not turn out to be the case. It was very upsetting having to go through the whole moving process and then have to leave a few days later.

This place was a lot smaller than what we have seen. The staff were nice. It had a homey feel, though the building itself is older. the outdoor space was a small deck. there wasn't much activity at the time.

My Mom had a long journey with Alzheimer's disease. We chose Dulaney Valley because she was beginning to fall into furniture and walls at her home. Dulaney Valley was small and homey, and close to us for visits. Despite the frequent turnover in help, one constant was Mr. T . Mom seemed to feel comfortable around him, all the residents do. His wife is the nurse and travels to the other facilities owned by her Son. She told me Mom would not have to go outside the home for nursing care or end of life hospice care. When Mom began to have trouble eating, The nurse advised that she would be going downhill. While I was visiting my daughter, apparently Mom began to fail, but I did not know that until a phone call from the nurse when I returned from my travel. I went to visit and was shocked at her condition compared to when I saw her two weeks before. She was in obvious pain. I had to arrange to meet the intake nurse to sign paperwork for hospice care, wait for the doctor, (who was out of town), while my Mother was dying and in terrible pain. Where was the "end of life" care we were promised when we contracted with D.V.A.L.? This process took far too long and was a horrible experience to live through. I found out through the literature from the hospice that arrangements could have been put in place six months ago! My opinion is that since they added more facilities and were in the process of remodeling Dulaney Valley while my Mother lay dying, everyone was too busy to get her the comfort and care she was due at the end of her life. My Mother could not speak very well since one of the symptoms of the terrible disease is aphasia, but the last thing she said to me was the most gut-wrenching and will haunt me always, she said "I hurt".

Due to unqualified aides not paying attention, Dad fell and resulted in hospitalization then rehab. Shortly thereafter was in hospice care all seemed from his fall. [name removed] the office manager is good, but there is lots of staff turn over. Unqualified aides who do not interact with staff. Never ever did I see activities. Snacks were never given unless you brought them. Everyone sirs in living room. Literally no interaction. My father hated every moment he was there.

This community is owned by the same community that another community my loved one was in and we did not like this community. The appearance of the community was very old. The manager was very nice but the community was just very old. We had to leave and come back due to lack of communication with the tour

The quality if care was poor because their caregiver turnover was weekly. They weren't trained properly and never engaged the seniors. Wanda the Nurse was nice and attentive but no one else. My dad would ask to go to the bathroom as he had a weak bladder and he would have to wait for about 15 mins before anyone got to him. He would get tired of waiting and go himself and had falls.

Staff seemed to have more patience with the women. People who are fecal incontinent this facility not a good choice. Food portions small. Facility suitable for someone who is able to go the bathroom unassisted and who isn't incontinent. Perhaps someone who has to have surgery or needs respite this would work for temporary placement of their loved one. The admissions administration fee equal and in addition to monthly fee. They are trying to impress by the name "Dulaney Valley Assisted Living in Hunt Valley MD" they are located near, but actually, address is Cockeysville, MD. It is convenient to Cockeysville & Phoenix, MD, but I think they are understaffed by polite, caring, compassionate individuals.

They are not able to keep staff, every time I go it is someone new. My pastor has ben trying to go and every time she tries to go they deny her and tell her that she can only come Tuesdays and Thursdays which she is unable to do. My loved one has lost a significant amount of weight. He said he is unable to feed the food because of his false teeth. They were supposed to change his diet, but they have yet to do so. One of the aids who is supposed to be watching them, when I go visit she is just sitting in a chair sleeping, she does not help the residents when they need it. I have been in a lot of places and most of them are great and have a lot of activities going on, but there is no mental stimulation going on here. We have also been disappointed with the diaper service, it does not include wipes and is very expensive. If you choose this facility, I would tell you to make sure to buy your loved ones their own diapers.

This community sucks and should be shut down. My loved one had to move due to the care that she was receiving. You also have to make an appointment to see your loved one and we did not like this.
